I faxed a request for a CD of imaging done at your facility but never received it. I suppose the fax may not have gone through. Anyway, I would still like a CD copy of imaging and radiologist's reports for my lumbar MRI (2011) and my breast US (2007?). My DOB is 7-22-73 and my old address was 41755 Cynthia Terrace Aldie VA 20115. My new address is above. Please call me at 202-509-2445 if for any reason you cannot send the CD/reports as requested. At the time of my visit I requested a CD but for some reason they handed me a bunch of images on real x-ray film-very costly to you all and not what I requested. Anyhow, thanks for your time and effort fulfilling my request.
