What is a Cataract?Cataracts develop when there is a buildup of abnormal protein in the lens of the eye that makes it cloudy. This distorts or prevents light from passing through the lens, causing some loss of vision. Since new lens cells form on the outside of the lens, all the older cells are compacted in the center of the lens and a cataract is formed. When the lens be-comes cloudy it is then called a cataract. Cataracts usually form slowly and cause few symptoms until they noticeably block light. When symptoms are present, they can include:

• Vision that is cloudy, blurry, or foggy; difficulty reading street signs or a newspaper; difficultly seeing TV captions or following a golf ball, etc.

• Progressive nearsightedness. • Changes in perception of color because the discolored lens acts as a filter. • Problems driving at night, caused by glare from oncoming headlights. • Glare problems during the day from sunlight. • Double vision (appears as a superimposed image). • Frequent changes in glasses prescriptions. • Needing more light to read.

How are Cataracts diagnosed?

An eye exam should be given to test how well you can see while wearing your glasses (be sure to bring them to your appointment). The doctor will dilate your pupils in order to examine the condition of the lens and other parts of the eye. Cataract surgery will not correct other causes of decreased vision such as glaucoma, diabetes, or age-related macular degeneration.

Cataract surgery is justified only when vision loss interferes with your everyday activities such as reading, watching TV, or driving safely. Poor vision has a negative impact on a person’s mental health and quality of life, and can also lead to social withdrawal. Fortunately, most individuals are able to once again engage in productive activities such as business, sports, or maintaining the household after cataract surgery.

Cataract Surgery Extends Life:In a study of more than 350 people with cataracts, those who had cataract surgery were 40% less likely to die from any cause within a 15 year period than those whose cataracts went untreated. A possible explanation is that the better eyesight that results from cataract surgery can increase a person’s optimism and motivation to pursue a healthy lifestyle.

Lens Selection

Each lens has different attributes that makes it a choice for certain patients. Dr. Tyson’s experience with many of these lenses during their research stage gives him knowledge of their strengths and weaknesses before they even come to market.

There are many considerations to discuss with your doctor regarding the selection of the best lens for your eyes and your lifestyle. In order to determine which lens is right for you, consider your lifestyle, and ask yourself if you focus more on near, far or intermediate objects. How important is color and contrast sensitivity to you? How often do you drive at night?

The exact health of your eye will also help determine which lens is best for you. Some people mistakenly believe having cataract surgery will enable them to see perfectly without glasses. The conventional artificial lens, covered by Medicare, provides a single fixed focus. It cannot give far distance focus one moment and near focus the next, like the eye’s natural lens does in a young person, so eyeglasses are still needed in order to shift the focus of your eye from far away to close up.

Monofocal IOL:Monofocal, or single vision lenses are the standard lenses that have been implanted at the time of routine cataract surgery for many years. These lenses take the place of your cataract and can help you see distant objects. These lenses will not correct astigmatism and will not correct near vision, so it is very likely that you will need to wear glasses at least part-time for distance activities and full-time for near work. These lenses are covered by Medicare and most insurance plans.

Special Intraocular Lens Packages:1. You can have a standard monofocal intraocular lens for distance vision and continue to wear glasses or contact lenses to also correct the astigmatism.2. You can have a special intraocular lens (toric IOL), which can correct astigmatism. Since most insurance companies will not cover the cost of a toric IOL, there is an additional out-of-pocket charge for this type of lens technology and professional services associated with its use.3. You can have refractive surgery such as LASIK or pho-torefractive keratectomy (PRK) several months after cata-ract surgery to correct not only astigmatism, but also any residual nearsightedness or farsightedness. There is an additional charge for LASIK or PRK that is not covered by insurance.

Presbyopia and Alternatives for Near Vi-sion after Surgery:At about the age of 45, all people who have good distance vision begin to have difficulty seeing at near. If they require corrective lenses to see at distance, the same lenses do not produce good vision at near. This occurs because the optical system of the eye must change its power to change its point of focus. This is normally accom-plished by changing the power of the eye’s nature lens by contracting the muscles inside of the eye and changing the shape of the lens. As we age, the lens becomes stiff, so it cannot change the focal point of the eye from distant to near. Bifocal glasses contain lenses that have different powers in the top and bottom of the lens. The top of the lens focuses distant objects, and the bottom focuses near objects. If no correction is needed for clear distance vision, non-prescription reading glasses correct near vision.

Lens selection is one of the most important steps in your cataract surgery. There is a large number of lenses to choose from, and as new lenses come to market the selection is ever changing.

Multifocal IOL Packages:Multifocal or multirange lenses are appropriate for some patients who have a strong desire to see distant and near objects without glasses and are willing to potentially compromise intermediate and low light vision to obtain freedom from glasses. Using the TECNIS™ Multifocal lens package for appropriate candidates can improve distance and near vision, and can reduce your dependency on spectacles at these distances. No lens restores the natural vision at all distances like you had as a youth, and you may experience some limitations with these lenses, including glare and halos, less sharpness of vision than may be obtained with a monofocal IOL and spectacles, and inadequate interme-diate vision that still may require the use of glasses. Multifocal lenses may require some time for adaptation, and in very rare cases the vision obtained with multifocal lenses may not meet expectations.

Extended Range of Vision Intraocular Lens:Tecnis Symfony intraocular lens is the first intraocular lens that improves visual acuity at close, intermediate and far ranges and, may reduce the need for patients to wear contact lenses or glass-es after cataract surgery.

This lens is designed to significantly reduce glare and halos as compared to multi-focal lenses. This lens is also available for those patients requiring astigmatism correction.

Accommodating IOL Packages:The Crystalens®, modeled after the human lens of the eye, uses the eye muscles to flex and accommodate to focus on objects at all distances, to provide good distance and intermediate acuity and a moderate amount of near correction. Crystalens® is designed to allow you to achieve a more natural range of vision. Many patients find that they still require reading glasses for most near tasks. Advantages of this lens include better night vision and less glare. It is a better choice for patients with glaucoma or macular degeneration or have a family history of macular degeneration.

Posterior CapsulotomyIs a simple procedure that is sometimes needed to restore clear vision after cataract surgery. It may be recommended soon after cataract surgery or several years later. It is a quick and painless procedure that should not cause you worry since the clouding or wrinkling of the posterior capsule is part of the body’s normal healing process. Using a YAG laser, your surgeon will make an opening in the posterior capsule allowing light to pass through again.

Faster Healing Times: Decreased endothelial cell loss translates di-rectly to increased long-term corneal health. In a series of 433 cases, the reduction in endothelial cell count was lower for the LENSAR™ laser cases compared to the phaco-only cases. Faster healing times result because less energy in the eye equals to less cell loss. LENSAR™ is the only femtosecond laser with FDA-labeling demonstrating a significant reduction in cumulative dissipated energy in the eye.

WHAT TO CONSIDER

What to do Before Surgery

• Start using your eye drops three days before your procedure in the surgery eye as directed. • On the morning of your surgery, you should take any medications approved by your surgeon and

which you normally take in the morning. These may include seizure, cardiac, high blood pressure, or breathing medication. Take them with just a sip of water.

• You should bring a responsible adult to provide you with a ride after your procedure. • You should wear a short-sleeved shirt or blouse that buttons in the front. • You should take a bath or shower prior to your procedure. • You should not wear makeup or false eyelashes. • You should not wear jewelry or bring any valuables to the surgery center. • You should not wear contact lenses. • You should not wear any restrictive clothing that day; do not wear panty-hose. • You should not eat or drink after midnight on the night before your procedure.

What to Expect After Surgery • You will need to rest the remainder of your surgery day. • Protect your eyes at all times. • Do not rub your eyes. • Tape a shield over the surgery eye during nap time and at

bedtime for at least three days to prevent rubbing it in your sleep.

• No driving of a vehicle for the first 24 hours due to the intravenous medication you were given.

• Avoid alcohol for the first 24 hours. • You may experience mild discomfort for which TylenolTM or ibuprofen can be taken. • Wear your sunglasses when outdoors. • You should continue using each of the eye drops as directed until further notice from

your doctor. • You will follow up with the doctor the next day and will be given additional instructions for the

care of your eye and the use of the eye drops.

Important: Do not rub your eyes after surgery.

It is normal for your eye to be slightly scratchy or irritated as the numbing medication wears off. You may even have double vision for a day or two. If you experience severe pain, excessive bleeding, or a decrease in your vision, call us at (239) 542-2020.

Our surgery staff will contact you the day prior to your procedure to confirm your arrival time and review your final instructions.

Frequently Asked Questions

Q: When should I have cataract surgery? At what age should I consider the surgery?A: When your vision starts to interfere with your daily activities or is less than the Department of Motor Vehicles requires for driving. Each situation is unique and should be evaluated individually. We have done cataract surgery on all ages - young adults, adults, the middle-aged and the elderly.

Q: Will I need glasses after my cataract surgery?A: That depends on the type of lens you choose and the overall health of your eyes. There are many variables which include your selection of cost level and your current vision issues such as Astigmatism ( a condition where there is a slight oval shape to the eye). Most patients are less dependant on eye glasses after cataract surgery.

Q: Will the cataract come back?A: Once a cataract is removed, it does not grow back. However, a clear membrane is left behind the IOL at the time of surgery. Over time, this membrane may haze or become cloudy, causing decreased vision and increased glare. This may occur as soon as a few weeks after surgery, but it usually is one to two years after surgery before the haze or cloudiness becomes significant. When the haze is significant, a laser pro-cedure can be performed (YAG capsulotomy) to clear it. This is a one-time, quick and painless procedure, which is performed in the office.

Q: What will my “down” time be?A: On the day of surgery, it is imperative to rest the remainder of the day and to protect your eyes from rubbing, water or injury for several weeks. It is normal for your eye to be slightly scratchy or irritated as the numbing medication wears off. If you experience severe pain, excessive bleeding, or a decrease in our vision please contact us at (239) 542-2020. Once your eye doctor releases you (from one week to one month after surgery), you can resume all your normal activities.

Q: Can I have both eyes done at the same time?A: We currently recommend the common practice of having one eye done at a time. Your eyes are one of your primary senses so it is not advisable to have both eyes done on the same day. Most of our patients have the second eye done as soon as one week after the first eye.

Q: Will I be asleep during surgery?A: You may be awake during surgery but you will be given medication to help you relax which causes many people to fall asleep. Prior to your procedure, your surgeon will discuss your anesthesia with you.

Q: If I have Glaucoma or a Retina Disorder can I still benefit from a premium lens?A: Based on your pre-surgical diagnostic testing, Dr. Tyson will recommend the lens that he feels will give you the best visual outcome. One of the premium lenses pictured below may be a choice for you.

Dr. Tyson’s World View of Eye Care began very early in life. He first began working after school at his father’s ophthalmology practice, building the practice computer system. As his interest in medicine evolved, he participated in most of the medical mission trips his family took to Mexico, Honduras, Panama, China, Africa, Ukraine, and Turks and Caicos Islands as a trained and certified ophthalmic technician.

Dr. Tyson’s first introduction to LASIK surgery was working as a biomedical engineer at Wilmer Eye Institute at Johns Hopkins University doing the early research on the VISIX 20/20 Excimer Laser.

Lecturing and training other doctors in the latest surgical techniques has taken Dr. Tyson to such far away places as Russia, Dubai, Saudi Arabia, Europe, China, and Singapore.

Dr. Tyson has authored a monthly article for Ophthalmology Management Magazine and also writes for many other magazines in the profession. He has served on many editorial boards such as Cataract and Refractive Surgery Today, Review of Ophthalmology, Ophthalmology Times and many more.

Dr. Tyson completed a degree in biomedical engineering from Johns Hopkins University before entering medical school which helps him stay on the leading edge of high-tech innovations in ophthalmology. He consults and lectures for AMO, Alcon, Aerie, Bausch & Lomb, Allergan, Dionysus, Chroma, Glaukos, Icon, iOptics, Optivue, Omeros, Katalyst Surgical, Lensar, Marco, Transcend, and Zeiss.

His eye on technology keeps Dr. Tyson involved in many clinical research trials which gives him experience with many new innovations and products before they come on the market.
