2. Has examined the site and all Bidding Documents.

a. To hold the Bid open until 1 calendar year after the Bid Opening.

c. To provide to the Owner a Performance Bond and a Payment Bond in an amount

4. Proposes to accomplish all Work in accordance with the Contract Documents for the Base
Bid prices as outlined in the following sections.
5. Bid Award Requirements:
□ All Bids must be submitted electronically to Maureen Gorski via email at
<[email protected]> on the attached Bid form.
Form shall include Lump sum pricing for Base Bid and Alternates as well as
requested Unit Pricing as defined on the bid form.
□ Certificate of Liability Insurance in accordance with the requirements stipulated in
Section 8.1 of ISFA’s Short Form Agreement between Owner and Contractor.
□ Payment and Performance Bond as outlined in Section 8.3 of ISFA’s Short Form
Agreement between Owner and Contractor.
□ Compliance with MBE/WBE participation requirements as set forth in Section 10.6 of
ISFA’s Short Form Agreement between Owner and Contractor.
□ Compliance with all other requirements of ISFA’s Request for Qualifications and short
Form Agreement between owner and Contractor.
6. General Requirements/Note:
□ The Work requires field measurement and verification of dimensions. Dimensions
shall be measured at each location prior to installation of equipment and materials.
The Contractor shall report any inconsistencies, variances, obstructions, and/or
interferences to the Architect/Engineer prior to proceeding with installation. If field
dimensions indicate that the work cannot be installed per the Drawings, notify the
Engineer for design modifications. Do not scale Drawings.
□ The construction schedule and staging shall be coordinated with the Owner to
minimize interference with scheduled events. The Contractor shall provide all
temporary controls as necessary to accommodate the building operations.

□ The Contractor shall secure and pay for all permits, licenses, and fees as required. The Contractor shall comply with codes, ordinances, rules, regulations, orders, and other legal requirements of Public Authority, which bear on the performance of the Work.
□ The Contractor shall at times protect all finishes against damage resulting from the
work performed. Any damage caused to the existing building elements shall be
repaired or replaced to the satisfaction of the Owner at the Contractor’s expense.
□ The Contractor shall comply with all security procedures.
□ The Contractor shall provide Proof of Insurance prior to the start of work.
□ Contractor shall promptly submit verbal and written notice to the Engineer of
observed variance of the Contract Documents from actual on-site conditions.
□ The Contractor will limit on-site storage of materials to those areas identified by the
Owner.
□ Water and electricity may be taken from the building for construction purposes only,
and at areas identified acceptable by the Owner. Toilet facilities in building shall be
designated by owner.
□ Contractor shall provide and maintain required dust barriers, canopies, barricades,
protection and warning lights in good condition until the completion of the work
requiring such protection and then remove the same. All canopies and barricades shall
comply with federal, state, and local laws and regulations.
□ The Contractor shall maintain premises free from accumulations of water, material,
odors and rubbish. Precautions should be taken to minimize blowing dust from
entering the building.
□ Costs caused by ill-timed work, defective work, or work not conforming to the
Contract Documents are the responsibility of the Contractor.
□ The Contractor shall provide shoring, bracing, and support as required to maintain the
structural integrity of the existing construction during the work. Construction debris
shall be removed in a manner that avoids overloading adjacent structural members.
□ Contractor shall be responsible for the restoration of finishes affected by work

A. BASE BIDS:
Base Bid #1 – HWS and HWR Replacement
Scope Provide pricing to disconnect, remove and replace all domestic hot water
supply branch piping and all hot water return branch piping indicated on
drawings dated October 26, 2018. Replace all galvanized piping present in
service corridor areas and all surrounding locker room, office, warehouse,
kitchen, catering, toilets, janitor closet, nursing, etc. areas. The piping shall
be replaced up to the last accessible fitting or union either a) up to floor
penetration above or drop down to fixture below. Provide dielectric unions
or dielectric waterway between dissimilar pipe materials. Include insulation,
hangers, valves, and demolition of existing pipe insulation, hangers (as
necessary), valves and components such as pressure reducing valves (PRV).
Existing PRV’s for clothes washers shall be replaced with new PRV’s.
Existing PRV’s for showers may be demolished without replacement.
Provide new copper pipe connected to piping drops serving fixtures or group
of fixtures.

Base Bid #6 – Water Heater Replacement
Scope Provide pricing to replace existing electrical domestic water heaters per the
drawings dated October 26, 2018. Replacement shall include new localized
pipe insulation, dielectric unions or dielectric waterways, new replacement
expansion tanks (of existing expansion tanks where present), and all necessary

Base Bid #7 – Replace Existing Lavatory Chrome-Plated Escutcheons For Existing
Floor-Mounted Wall Carrier at 100 and 500 Levels
Scope Provide pricing to replace all existing lavatory chrome-plated escutcheons and
carrier bars for existing floor-mounted wall carriers at 100 and 500 levels per
the drawings dated October 26, 2018. Include all necessary scope to
temporarily dismount and disconnect the existing lavatory fixture, the drainage
piping, and the water supply tubing to perform this scope. Provide work to
remount and reconnect the existing fixture, the drainage piping, and the water
supply tubing also. Maintain and restore existing drainage and water supply
tubing insulation for ADA accessibility. This would include adequately storing
the lavatory fixture and replacement of any lavatory fixtures, drainage piping,
water supply tubing, accessibility insulation, etc. which happen to get
damaged. Provide floor protection as necessary to protect existing floors and
dismounted fixtures. Furnish and install new chrome-plated escutcheons and
new carrier bars for all existing wall mounted lavatories at 100 and 500 levels.

Area Drain Cover Replacement:
Contractor shall include pricing to furnish and install replacement top/cover for existing area
drains as defined and scheduled on sheet P3.002 if the top/cover is damaged. There are numerous
area drains throughout the park. Base dollar numbers shall be based on base bid product indicated
on drawings; alternate dollar numbers shall be based on alternate product. Submit alternate
product submittal sheet for review. Successful contractor shall provide a sample product for
review prior to acceptance of any alternate product.
