Sabbatical Leave: You will accrue one month of leave for each year of service, and you are eligible for sabbatical leave after three years with Northwest Permanente, P.C. During a sabbatical, benefits remain in force and salary continues at one-half of normal pay. Sabbatical leave requires approval of the Board of Directors.

Long-Term Disability: Long-term disability is provided by Northwest Permanente, P.C. The benefit will provide up to approximately 50% of your salary, with a monthly maximumof $10,000 (non-taxable), for the duration of the disability or until age 65. An optional supplemental disability policy is also available.

Long-Term Care: Physicians and spouses are eligible for long- term care coverage at their own expense, without proof of insurability, if enrolled during the first 30 days of employment. This plan is also available for parents and parents in-law; proof of insurability is required.

Retirement: Northwest Permanente, P.C. offers two retirement programs. The Permanente Physicians’ Retirement Plan, a defined benefit plan, provides a scheduled income at retirement for those physicians who have completed five or more years of service with Northwest Permanente, P.C. The income is based on average salary (within Federal limits) and total years of credited service.

The Northwest Permanente, P.C. Retirement Plan has two components: 401(k), which requires approximately one year of service, and defined contribution, which requires approximately two years of service. Between the two components, Northwest Permanente, P.C. makes contributions of an amount equal to 11% of salary (within Federal limits) to the plan. In addition, participants may elect to make pre-tax or post-tax contributions to the 401(k) portion of the plan. You may withdraw the funds at termination of employment or retirement.

Life Insurance: You are provided $200,000 of group term life insurance plus $200,000 accidental death and dismemberment insurance at no cost. Within 30 days of your hire, you may purchase up to an additional $250,000 of life insurance plus$50,000 of spousal life at group rates, without proof of insurability. After that period, proof of insurability is required for additional insurance. You may purchase a total of $450,000 of life insurance for a maximum coverage of $650,000. Maximumspousal insurance is $200,000 with the remaining $150,000 requiring proof of insurability. Note: There are times when the basic, supplemental and spousal coverages will automatically reduce due to age.

Dependent Care Reimbursement Program: You may pay for qualified dependent care expenses with tax-free dollars. A predesignated amount of up to $5,000 annually is deducted from your pre-tax salary and deposited into a reimbursement account. These deposits are reimbursed to you when receipts for dependent care are submitted. Dependent care reimbursements may be claimed for children under age 13 or spouse or other dependents living with you who are unable to care for themselves.

Health Care Spending Account: You may pay for qualified health care expenses with tax-free dollars. A pre-designated amount of up to $3,000 annually is deducted from your pre-tax salary and deposited into a reimbursement account. These deposits are reimbursed to you when receipts for health care expenses are submitted. Dependents whose expenses are eligible for reimbursement from the Health Care Spending Account include any member of your household that you claim as a dependent for tax purposes.

Educational Programs: You are invited to participate in programs offered by our Department of continuing medical education, accredited by the AMA’s Accreditation Council for Continuing Medical Education since 1979. These courses are provided for the Northwest Permanente, P.C. professional staff and the community at large. Each medical department also offers a variety of continuing medical education programs throughout the year. To pursue continuing education, one week of educational leave is available annually for the first two years of association, and two weeks are available annually thereafter. An annual educational allowance of $650 is also provided for the first two years. Physicians who have been with Northwest Permanente, P.C. for approximately two years receive $1,200 professional allowance per year to use for continuing medical education and other professional expenses. The allowance is established annually by the Board of Directors.
