It had a laryngoplasty, left ary-epiglottic fold removal, and right ventriculo- cordectomy (‘tie-back surgery’) performed on 04/06/18. During surgery it became apparent that this horse had a congenitally malformed muscular process on its right arytenoid, and had no lever arm for the muscle to pivot open the arytenoid with. Since the prosthesis replaces the muscle, the prosthesis also had limited ability to perform this action. However, by changing the angle of suture placement, I was able to achieve some abduction. Since the arytenoid is pulled over midline by the negative pressure generated during inspiration during galloping, even fixing of the arytenoid in this position will improve airway diameter during exercise, as will removal of the vocal fold. Post-operative endoscopy (below) showed minimal abduction, but if it remains fixed in this position it will likely be a good enough airway for a sprint horse. Instructions regarding the post-operative care of the horse will be provided with the horse at the time of discharge. The horse should have a three month spell before returning to work. An endoscopic examination prior to being put back in work may be useful to ensure adequate arytenoid abduction at that time.
