Background

• Radiation leak discovered during annual white-light survey

• Surgical inspection revealed that:– Lead had shifted slightly– Corner section was not built

to specification

• Actions taken:– All lead lined hutches are to

be inspected and reviewed– All lead lined hutches moved

to Pre-Commissioned status

Shielding Inspection Criteria

• Verify that lead shielding is either:– Contiguous– Properly overlapped

• Ensure that shielding is secure and will not move

• Document and Archive

Shielding Defining Process

• Determine types of panels and shielding– Exploratory surgery– Compare to mfg drawings

• Develop repair plan:– Procedure for panel repair– Process for documentation

Shielding panel types

• Three types of shielding panels on the hutch:

–Structural (S)–Non-Structural (NS)–Corners (C)

• Additional regions–Doors–Support Structure over door

Shielding Repair Approach

• Documentation– Generate tracking methods for each panel– Record panel information and repairs– Archive documents, drawings and photos.

• Three basic shielding repair “actions” to hutch panels:

– Skinning (structural & corners)– Securing (non-structural & doors)– Surveying (roof, H – panels, labyrinths)

8.3.2 Shielding Documentation

• Documentation note– Includes explanation– Defines numbering

system for panels and references drawing

– Provides repair forms for each panel

– Will serve to record repairs with forms and photos.

Open issues…..

• Complete documentation note with deails, process for repair and drawings for each panel on hutch

• Review of Commissioning radiation survey procedure – All concerns must be addressed regarding actual

process.

• Structural Calculations– INC is reviewing hutch design with added lead– If additional structure is needed a design could be

added to the corners
