Backup Storage Agreement between Jackson County Health Department and Jackson County Sheriff’s Department Purpose The purpose of this agreement is to establish off site storage of data tapes for the Jackson County Health Department and security of these tapes which contain protected health information. The Jackson County Sheriff agrees to: 1. Provide storage of data tapes belonging to the Jackson County Health Department in the

vault located within the Tax Department. 2. Ensure that stored data tapes are only exchanged with those individuals listed on this

agreement. If Tax Department staff is not familiar with either of these individuals, they will request to see identification.

3. Ensure, to the best of staff’s ability, that these tapes are secure and not available for release or review.

The Jackson County Health Department agrees to: 1. Deliver data tapes to the Tax Department on each Tuesday unless the Health Department or

the Tax Department is closed for business on that day. If this occurs, tapes will be delivered the next business day.

2. Refrain from sending anyone not listed below to deliver or retrieve these data tapes. 3. Only deliver and pick up data tapes during normal operating hours of the Tax Department

unless there is a declared emergency. Data backup tapes will only be delivered or retrieved by: 1. Tena Casto 2. Sandra Golden 3. Wendy Barnette

This agreement becomes effective this _________ day of _______________________, 2005.
