MECHANISM OF ACTION
Azithromycin prevents bacteria from growing by interfering with their protein synthesis. It binds to the 50S subunit of the bacterial ribosome, and thus inhibits translation of mRNA.

Demonstrates unique pharmacokinetics
Oral azithromycin is rapidly absorbed in the blood stream and quickly penetrates into tissues and phagocytic cells where its concentrations exceed the serum by 10 to 100 fold.

Tissues act as reservoir of azithromycin from where it is taken up by phagocytic cells for delivery to the site of action. Phagocytes containing high concentrations of azithromycin migrate into infection sites and then release it precisely where it is required. Presence of bacteria stimulates release of azithromycin from phagocytic cells.

As azithromycin has an extremely longer half life (2 -4 days), a 3 days dosage regimen is expected to produce drug concentrations in target tissues above the MICs for target pathogens for a period of upto 10 days.
Post therapy tissue concentrations may persist even upto 10 days

Adults: For most indications, the total dose is 1.5g which should be given as 500 mg once daily for 3 consecutive days.
For sexually transmitted diseases caused by Chlamydia trachomatis the dose is 1 g as a single dose.
For typhoid fever 500mg once daily for 5 days. For children 10 mg/kg/day once daily for 5 days.
Children over 6 months : 10 mg/kg as a single dose for 3 consecutive days.
