An umbilical hernia occurs when part of the intestine protrudes through an opening in the

abdominal muscles. Umbilical hernia is a common and typically harmless condition. Umbilical hernias are most common in infants, but they can affect adults as well. In an infant, an umbilical hernia may be especially evident when the infant cries, causing the baby’s bellybutton to protrude. This is a classic sign of an umbilical hernia.

Many umbilical hernias close on their own by age 1, though some take longer to heal. To prevent complications, umbilical hernias that don’t disappear by age 3 or those that appear during adulthood may need surgical repair.

Symptoms ¾An umbilical hernia creates

a soft swelling or bulge near the navel (umbilicus). If your baby has an umbilical hernia, you may notice the bulge only when he or she cries, coughs or strains. The bulge may disappear when your baby is calm or lies on his or her back.

Umbilical hernias in children are usually painless. Umbilical hernias that appear during adulthood may cause abdominal discomfort.

When to see a doctor ¾If you suspect that your baby

has an umbilical hernia, talk with your child’s pediatrician. Seek emergency care if your baby has an umbilical hernia and:

Your baby appears to be in ªpainYour baby begins to vomit ªThe bulge becomes tender, ªswollen or discolored

Similar guidelines apply to adults. Talk with your doctor if you have a bulge near your navel. Seek emergency care if the bulge becomes painful or tender. Prompt diagnosis and treatment can help prevent complications.

Causes ¾

During pregnancy, the umbilical cord passes through a small opening in the baby’s abdominal muscles. The opening normally closes just after birth. If the muscles don’t join together completely in the midline of the abdomen, this weakness in the abdominal wall may cause an umbilical hernia at birth or later in life.

In adults, too much abdominal pressure can cause an umbilical hernia. Possible causes in adults include:

Staying healthy during the monsoon season requires particular care to be taken. The rain and water lying

around makes it easy for mosquitoes to breed. With the rains several potential health problems arises. By protecting us with the right precautions is a great deal of worry.

Seasonal concerns �Coughs, colds and flu, Fever, Dealing with extreme

weather, Mosquito-related problems like Dengue, Malaria, Mosquito repellents, and so on… So avoid unhealthy diet. and have only recommended diet.

Recommended Diet: � Eat light foods like vegeta-bles, fruits, cereals, Eat foods which are drying in nature, the most popular being makka (corn), chana (chickpea), besan (gram flour), jow (oats). Consume a lot of bitter vegetables like karela (bitter gourd) and bitter herbs like neem (basil), methi (fenugreek) seeds, haldi (turmeric) as it prevents infec-tion.

Unhealthy diet: � Potatoes, cauliflower, cluster beans, and ladies’ finger among vegetables, pigeon pea, kid-ney beans, urad and sprouted grains should not be consumed. Avoid heavy salty food as it promotes water retention and bloating, avoid too much of meat or fish, Avoid foods which are very sour like tamarind (imli), chutneys and pickles as they promote water retention.

Measures for boosting the energy: �In this season digestion is generally weak. Therefore one ªshould eat less. It is essential to fast once in a week or fortnight.It is highly beneficial to walk barefoot on green grass. ªAvoid sleeping in day time. ªDo not indulge in extreme physical exertion. ªDo not get exposed to sun frequently. ªIf you get drenched by the heavy rain, dry yourself as ªsoon as possible or you can take a shower first and then dry yourself before taking a sip of that coffee.Always keep the surroundings dry and clean, neat and ªtidy. Keep your body warm as viruses attack immediately ªDry your feet with soft dry cloth whenever they are wet. ªDrink plenty of water and keep your body hydrated. ª

— especially premature babies and those with low birth weights. Black infants appear to have a slightly increased risk of umbilical hernias. The condition affects boys and girls equally.

For adults, being overweight or having multiple pregnancies may increase the risk of developing an umbilical hernia. This type of hernia tends to be more common in women in their 50s or 60s.

Complications ¾For children, complications of an umbilical

hernia are rare. Complications can occur when the protruding abdominal tissue becomes trapped (incarcerated) and can no longer be pushed back into the abdominal cavity. This reduces the blood supply to the section of trapped intestine and can lead to umbilical pain and tissue damage. If the trapped portion of intestine is completely cut off from the blood supply (strangulated hernia), tissue death (gangrene) may occur. Infection may spread throughout the abdominal cavity, causing a life-threatening situation.

Adults with umbilical hernia are somewhat more likely to experience incarceration or obstruction of the intestines. Emergency surgery is typically required to treat these complications.

Preparing for your appointment ¾If you or your child has signs or symptoms

common to an umbilical hernia, make an appointment with your family doctor or your child’s pediatrician.

Your doctor is likely to ask you a number of questions, such as:

What signs and symptoms have you noticed? ªWhen did you first notice these signs and ªsymptoms?Have these signs and symptoms gotten worse ªover time?Are you or your baby in pain? ªHave you or your baby vomited? ªIf you are the one affected, do your hobbies or ªyour work involve heavy lifting or straining, or have you recently gained a significant amount of weight?Have you or your child recently been treated for ªany other medical conditions?Tests and diagnosis ¾An umbilical hernia is diagnosed during a ªphysical exam. Sometimes imaging studies — such as an abdominal ultrasound or X-ray — are used to screen for complications. Treatments and drugs ¾

Most umbilical hernias in babies close on their own by 18 months. Your doctor may even be able to push the bulge back into the abdomen during a physical exam. Don’t try this on your own, however. Although some people claim a hernia can be fixed by taping a coin down over the bulge, this “fix” doesn’t help and germs may accumulate under the tape, causing infection.

For children, surgery is typically reserved for umbilical hernias that:

Are painful ªAre bigger than 1.5 centimeters in diameter ª(slightly larger than a 1/2 inch)Don’t decrease in size after six to 12 months ªDon’t disappear by age 3 ªBecome trapped or block the intestines ª

For adults, surgery is typically recommended to avoid possible complications — especially if the umbilical hernia gets bigger or becomes painful.

During surgery, a small incision is made at the base of the bellybutton. The herniated tissue is returned to the abdominal cavity, and the opening in the abdominal wall is stitched closed. In adults, surgeons often use mesh to help strengthen the abdominal wall. Recurrences are unlikely.

Colon cancer is cancer of the large intestine (colon), the lower part of your digestive

system. Rectal cancer is cancer of the last 6 inches of the colon. Together, they’re often referred to as colorectal cancers. About 112,000 people are diagnosed with colon cancer annually, and about 41,000 new cases of rectal cancer are diagnosed each year, according to the American Cancer Society.

Most cases of colon cancer begin as small, noncancerous (benign) clumps of cells called adenomatous polyps. Over time some of these polyps become colon cancers.

Polyps may be small and produce few, if any, symptoms. Regular screening tests can help prevent colon cancer by identifying polyps before they become cancerous. If signs and symptoms of colon cancer do appear, they may include changes in bowel habits, blood in your stool, persistent cramping, gas or abdominal pain.

Signs and symptoms of colon cancer �include:

A change in your bowel habits, including ªdiarrhea or constipation or a change in the consistency of your stool for more than a couple of weeks Rectal bleeding or blood in your stool ªPersistent abdominal discomfort, such as cramps, ªgas or pain Abdominal pain with a bowel movement ªA feeling that your bowel doesn’t empty ªcompletely Weakness or fatigue ª

Unexplained weight loss ªBlood in your stool may be a sign of cancer, but it

can also indicate other conditions. Bright red blood you notice on bathroom tissue more commonly comes from hemorrhoids or minor tears (fissures) in your anus, for example. In addition, certain foods, such as beets or red licorice, can turn your stools red. Iron supplements and some anti-diarrheal medications may make stools black. Still, it’s best to have any sign of blood or change in your stools checked promptly by your doctor because it can be a sign of something more serious. may become cancerous. In later stages of the disease, colon cancer may penetrate the colon walls and spread (metastasize) to nearby lymph nodes or other organs. As with most cancers, the exact cause for colon cancer is unknown.

Causes ¾In general, cancer occurs when healthy cells

become altered. Healthy cells grow and divide in an orderly way to keep your body functioning normally. But sometimes this growth gets out of control — cells continue dividing even when new cells aren’t needed. In the colon and rectum, this exaggerated growth may cause precancerous cells to form in the lining of your intestine. Over a long period of time — spanning up to several years — some of these areas of abnormal cells may become cancerous. In later stages of the disease, colon cancer may penetrate the colon walls and spread (metastasize) to nearby lymph nodes or other organs. As with most cancers, the exact cause for colon cancer is unknown.

¾ Precancerous growths in the colonPrecancerous cells can occur anywhere

in your large intestine, the muscular tube that forms the last part of your gastrointestinal tract. The colon comprises the upper 4 to 5 feet of your large intestine, and the rectum makes up the lower 6 inches.

Precancerous growths most commonly occur as clumps of cells (polyps) that extend from the wall of the colon. Polyps can appear mushroom-shaped. Precancerous growths can also be flat or recessed into the wall of the colon (nonpolypoid lesions). Nonpolypoid lesions are more difficult to detect, but are less common.

Several types of colon polyps exist. ¾Among the most common are:

Adenomas. ª These polyps have the potential to become cancerous and are usually removed during screening tests such as flexible sigmoidoscopy or colonoscopy.

Hyperplastic polyps. ª These polyps

are rarely, if ever, a risk factor for colorectal cancer. Inflammatory polyps. ª These polyps may follow a bout of ulcerative colitis. Some inflammatory polyps may become cancerous, so having ulcerative colitis increases your overall risk of colon cancerRisk factors ¾

Factors that may increase your risk of colon cancer include:

Age. ª About 90 percent of people diagnosed with colon cancer are older than 50. Colon cancer can occur in younger people, but it occurs much less frequently. A personal history of colorectal cancer or ªpolyps. If you’ve already had colon cancer or adenomatous polyps, you have a greater risk of colon cancer in the future. Inflammatory intestinal conditions. ª Long-standing inflammatory diseases of the colon, such as ulcerative colitis and Crohn’s disease, can increase your risk of colon cancer. Inherited disorders that affect the colon. ª Genetic syndromes passed through generations of your family can increase your risk of colon cancer. These syndromes cause only about 5 percent of all colon cancers. One genetic syndrome called familial adenomatous

polyposis (FAP) is a rare disorder that causes you to develop thousands of polyps in the lining of your colon and rectum. People with untreated FAP have a greater than 90 percent chance of developing colon cancer by age 45. Another genetic syndrome, hereditary nonpolyposis colorectal cancer (HNPCC), also called Lynch syndrome, is more common than FAP. People with HNPCC have an increased risk of colon cancer and tend to develop colon cancer at an early age. Both FAP and HNPCC can be detected through genetic testing. Talk to your doctor about whether your family history suggests you have a risk of these conditions. Family history of colon cancer and colon ªpolyps. You’re more likely to develop colon cancer if you have a parent, sibling or child with the disease. If more than one family member has colon cancer or rectal cancer, your risk is even greater. In some cases, this connection may not be hereditary or genetic. Instead, cancers within the same family may result from shared exposure to an environmental carcinogen or from diet or lifestyle factors. Diet. ª Colon cancer and rectal cancer may be associated with a diet low in fiber and high in fat and calories. Research in this area has had mixed results. Some studies have found an increased risk of colon cancer in people who eat diets high in red meat and processed meats. People who eat a diet similar to that of Western ªcountries, such as the United States and Europe, have a higher risk of developing colon cancer than do people who eat diets typically seen in developing countries. When people move from a developing country to a Western country and adapt to the Western diet, their risk of colon cancer increases. Although many studies have tried to identify what part of the Western diet may cause colon cancer, the answer remains unknown. A sedentary lifestyle. ª If you’re inactive, you’re more likely to develop colon cancer. This may be because when you’re inactive, waste stays in your colon longer. Getting regular physical activity may reduce your risk. Diabetes. ª People with diabetes and insulin resistance may have an increased risk of colon cancer. Obesity. ª People who are obese have an increased risk of colon cancer and an increased risk of dying of colon cancer when compared with people considered normal weight. Smoking. ª People who smoke cigarettes may have an increased risk of colon cancer. They may also have an increased chance of dying of colon cancer. Alcohol. ª Heavy use of alcohol may increase your odds of colon cancer. Growth hormone disorder. ª Acromegaly, an uncommon disorder that causes an excess of growth hormone in your body, may increase your risk of colon polyps and colon cancer. Radiation therapy for cancer. ª Radiation therapy directed at the abdomen to treat previous

cancers may increase the risk of colon cancer. When to seek medical advice ¾

If you notice any symptoms of colon cancer, such as blood in your stool or a persistent change in bowel habits, see your doctor as soon as possible. Keep in mind that colorectal cancer can occur in younger as well as older people. If you’re at high risk, don’t wait until symptoms appear. See your doctor for regular screenings.

Talk to your doctor about when you should begin screening for colon cancer. Guidelines generally recommend colon cancer screenings begin at age 50. Your doctor may recommend more frequent or earlier screening if you have other risk factors, such as a family history of the disease.

Tests and diagnosis ¾Screening

Most colon cancers develop from adenomatous polyps. Screening can detect polyps before they become cancerous. Screening may also detect colon cancer in its early stages when there is a good chance for cure.

You may be embarrassed by the screening procedures, worried about discomfort or afraid of the results. Discuss your screening options and your concerns with your doctor. Most procedures are only moderately uncomfortable, and working with a doctor you like and trust can help ease your embarrassment.

Common screening and diagnostic procedures include the following:

Stool blood test. ª The fecal occult (hidden) blood test checks a sample of your stool for blood. It can be performed in your doctor’s office, but you’re usually given a kit that explains how to take the sample at home. You return the sample to a lab or your doctor’s office to be checked. Not all cancers can be detected with a stool blood test, since not all cancers bleed and those that do often bleed intermittently. This can result in a negative test result, even though you may have cancer. If blood is detected in your stool, your doctor may order other screening tests to determine the source of the blood. Blood in your stool can be the result of hemorrhoids or an intestinal condition other than cancer. Certain foods also may alter test results, suggesting blood in the colon when there is none. Stool DNA test. ª This test involves analyzing several DNA markers, which come from cells that are shed by colon cancers or precancerous polyps into stool. To complete the stool DNA test, you typically receive a collection kit from your doctor, collect one bowel movement at home and then send the kit to a laboratory for analysis. Stool DNA testing is thought to be more accurate for detecting colon cancer than polyps, and it’s important to remember that this test can’t detect all the possible DNA mutations that may indicate a tumor. This test isn’t widely available yet, and it may not be covered by your insurance. For these reasons it’s not commonly used. Flexible sigmoidoscopy. ª In this test, your

doctor uses a flexible, slender and lighted tube to examine your rectum and sigmoid — approximately the last 2 feet of your colon. The test usually takes just a few minutes. It can sometimes be uncomfortable, and there’s a slight risk of perforating the colon wall. If a polyp or colon cancer is found during this exam, your doctor will recommend colonoscopy to look at the entire colon and remove any polyps that are present for examination under a microscope. Because sigmoidoscopy only examines the lower third of the colon, polyps or cancer in the rest of the colon won’t be detected. Barium enema. ª This diagnostic test allows your doctor to evaluate your entire large intestine with an X-ray. Barium, a contrast dye, is placed into your bowel in an enema form. During a double-contrast barium enema, air is also added. The barium fills and coats the lining of the bowel, creating a clear silhouette of your rectum, colon and sometimes a small portion of your small intestine. There’s also a slight risk of perforating the colon wall, and the test has a significantly high rate of missing important lesions. A flexible sigmoidoscopy is often done in addition to the barium enema to aid in detecting small polyps that a barium enema X-ray may miss, especially in the lower bowel and rectum. Barium enema may miss small polyps or small cancers. If an abnormality is detected using barium enema, your doctor may recommend a colonoscopy. Colonoscopy. ª Colonoscopy is similar to flexible sigmoidoscopy, but the instrument used — a colonoscope, which is a long, flexible and slender

tube attached to a video camera and monitor — allows your doctor to view your entire colon and rectum. If any polyps are found during the exam, your doctor may remove them immediately or take tissue samples (biopsies) for analysis. This is done through the colonoscope and is painless. You may receive a mild sedative to make you ªmore comfortable. Preparation for the procedure involves drinking a large amount of fluid containing a laxative to clean out your colon — enemas are no longer necessary. Major risks of diagnostic colonoscopy include bleeding and perforation of the colon wall, but these are rare. Virtual colonoscopy (CT colonography). ª Though not available at all medical centers, virtual colonoscopy may be one screening option. Virtual colonoscopy uses a computerized tomography (CT) machine to take images of your colon, rather than using a scope inserted in your rectum to see inside your colon. Virtual colonoscopy still requires that you undergo a bowel preparation to remove any stool. If any abnormalities are detected on virtual colonoscopy, you’ll still need to undergo conventional colonoscopy. Research into virtual colonoscopy is still ongoing. Diagnosis ¾

If your doctor suspects you may have colon cancer based on your signs and symptoms, he or she may recommend colonoscopy to look for colon cancer. Colonoscopy allows your doctor to look for polyps or unusual areas in your colon. Your doctor can also remove a sample of tissue from your colon to look for cancer cells. In some cases, barium enema or flexible sigmoidoscopy may be used to diagnose colon cancer.

Staging your cancer ¾Once you’ve been diagnosed with colon cancer,

your doctor will then order tests to determine the extent, or stage, of your cancer. Staging helps determine what treatments are most appropriate for you. Staging tests may include imaging procedures such as abdominal CT scan or chest X-ray. The stages of colon cancer are:

Stage 0. ª Your cancer is in the earliest stage. It hasn’t grown beyond the inner layer (mucosa) of your colon or rectum. This stage of cancer may also be called carcinoma in situ. Stage I. ª Your cancer has grown through the mucosa but hasn’t spread beyond the colon wall or rectum. Stage II. ª Your cancer has grown into or through the wall of the colon or rectum but hasn’t spread to nearby lymph nodes. Stage III. ª Your cancer has invaded nearby lymph nodes but isn’t affecting other parts of your body yet. Stage IV. ª Your cancer has spread to distant sites, such as other organs — for instance to your liver or lung, to the membrane lining the abdominal cavity, or to an ovary. Recurrent. ª This means your cancer has come back after treatment. It may recur in your colon, rectum or other part of your body. Treatments and drugs ¾

The type of treatment your doctor recommends

will depend largely on the stage of your cancer. The three primary treatment options are: surgery, chemotherapy and radiation.

Surgery (colectomy) is the main treatment for colorectal cancer. How much of your colon is removed and whether other therapies, such as radiation or chemotherapy, are an option for you depend on the location of your cancer, how far cancer has penetrated into the wall of your bowel, and whether it has spread to your lymph nodes or other parts of your body.

Surgical procedures ¾Your surgeon removes the part of your colon that

contains the cancer, along with a margin of normal tissue on either side of the cancer to help ensure that no cancer is left behind. Nearby lymph nodes are usually also removed and tested for cancer. Your surgeon is often able to reconnect the healthy portions of your colon or rectum. But when that’s not possible, for instance if the cancer is at the outlet of your rectum, you may need to have a permanent or temporary colostomy. This involves creating an opening in the wall of your abdomen from a portion of the remaining bowel for the elimination of body waste into a special bag. Sometimes the colostomy is only temporary, allowing your colon or rectum time to heal after surgery. In some cases, however, the colostomy may be permanent.

Side effects of colon cancer surgery may include short-term pain and tenderness, and temporary constipation or diarrhea. If you have a colostomy, you may develop an irritation on the skin around the opening (stoma).

Surgery to prevent cancer ¾In cases of rare, inherited syndromes such as

familial adenomatous polyposis, or inflammatory bowel disease such as ulcerative colitis, your doctor may recommend removal your entire colon and rectum in order to prevent cancer from occurring in the future. Then, in a procedure known as ileal pouch-anal anastomosis, your surgeon will likely construct a pouch from the end of your small intestine that attaches directly to your anus. This allows you to expel waste normally, although you may have several watery bowel movements a day.

Surgery for early-stage cancer ¾If your cancer is small, localized in a polyp and

in a very early stage, your surgeon may be able to remove it completely during a colonoscopy. If the pathologist determines that the cancer in the polyp doesn’t involve the base — where the polyp is attached to the bowel wall — then there’s a good chance that the cancer has been completely eliminated.

Some larger polyps may be removed using laparoscopic surgery. In this procedure, your surgeon performs the operation through several small incisions in your abdominal wall, inserting instruments with attached cameras that display your colon on a video monitor. He or she may also take samples from the lymph nodes that drain the area where the cancer is located. Studies have found that people undergoing this procedure need less pain medication and leave the hospital a day earlier on average. Also, people who have this procedure don’t have higher rates of recurrence than those who choose the open surgery.

Surgery for advanced cancer ¾If your cancer is very advanced or your overall

health very poor, an operation to relieve a blockage of your colon or other conditions in order to improve your symptoms may be the best option. This type of surgery is referred to as palliative surgery. The goal of palliative surgery isn’t to cure your cancer, but to relieve signs and symptoms, such as bleeding and pain.

In specific cases where the cancer has spread only to the liver and if your overall health is otherwise good, your doctor may recommend surgery to remove the cancerous lesion from your liver. Chemotherapy may be used before or after this type of surgery. This treatment may improve your prognosis.

Chemotherapy ¾Chemotherapy uses drugs to destroy cancer

cells. Chemotherapy can be used to destroy cancer cells after surgery, to control tumor growth or to relieve symptoms of colon cancer. Your doctor may recommend chemotherapy if your cancer has spread beyond the wall of the colon. In some cases, chemotherapy is used along with radiation therapy.

Possible side effects of chemotherapy include nausea and vomiting, mouth sores, fatigue, hair loss and diarrhea. Discuss the side effects and risks as well as the potential benefits with your doctor.

Radiation therapy ¾Radiation therapy uses powerful energy sources,

such as X-rays, to kill any cancer cells that might

remain after surgery, to shrink large tumors before an operation so that they can be removed more easily, or to relieve symptoms of colon cancer and rectal cancer.

Radiation therapy is rarely used in early stage colon cancer, but is a routine part of treating early stage rectal cancer, especially if the cancer has penetrated through the wall of the rectum or traveled to nearby lymph nodes. Radiation therapy, usually combined with chemotherapy, may be used after surgery to reduce the risk that the cancer may recur in the area of the rectum where it began.

Side effects of radiation therapy may include diarrhea, rectal bleeding, fatigue, loss of appetite and nausea.

Targeted drug therapy ¾Three drugs that target specific defects that allow

cancer cells to proliferate are available to people with advanced colon cancer. The drugs bevacizumab (Avastin), cetuximab (Erbitux) and panitumumab (Vectibix) can be given along with chemotherapy drugs or alone. Bevacizumab works to prevent tumors from developing new blood vessels, which can deliver the oxygen and nutrients cancers need to survive. Cetuximab and panitumumab target a chemical signal that tells cells to divide and reproduce. Cetuximab was approved by the Food and Drug Administration in 2007 as a single-agent treatment for advanced colon cancer for which other treatments have failed. Panitumumab remains experimental.

Premature labour and delivery is a serious risk if a woman is pregnant with multiples. The average length of pregnancy for a singleton (one child) is 39-40 weeks. A multiple gestation pregnancy is usually much shorter: 36 weeks for twins; 33 weeks for triplets; and 31 weeks for quadruplets. Babies born prematurely may have following complications:

Low birth weight; ªUnderdeveloped lungs; ªBrain damage resulting in cerebral palsy. ªWhen born too early, premature infants usually ªrequire care in Neonatal Intensive Care UnitTreatment overview ¾It is always important to take extra good care of ªyourself when pregnant. When carrying twins or more (multiple pregnancy), be sure to eat a balanced and nutritious diet, calcium, iron, and folic acid to nourish you and your babies. A multiple pregnancy can make morning ªsickness worse during the first few months of pregnancy. You can treat your symptoms at home, unless you have become dehydrated or are not getting enough to eat because of vomiting.Avoid alcohol, smoking, caffeine, most ªmedicines, chemicals, x-rays, some cosmetic products and eating contaminated food. All of these precautions are advised for any pregnancy. Medicines should be taken only with advice from doctor.Get plenty of rest, especially after the 24th week ªof pregnancy. See your health professional often. Beginning in the 20th week of pregnancy, you may be checked every other week. Beginning at the 30th week, you may be checked more often and get frequent scans done.Mode of delivery whether normal or caesarean ªdepends on the condition of the mother and the position of babies in the uterus.Uterus is unable to bear the weight of the twin ªbabies and may open up earlier leading to abortions and preterm delivery so a cervical stitch is applied at 12 to!4 weeks of gestation and is removed at 36 weeks to facilitate delivery.

What is uveitis? �Uvea is the middle part of the three coats of the eye. This further

consists of the iris, ciliary body and the choroid. Inflammation of any of these parts is termed uveitis.

What are the types of uveitis? �Based on the part of the uvea involved, uveitis may be Anterior

(involving the iris), Intermediate (involving the ciliary body), Posterior (involving the choroid) or Panuveitis (involving all the parts).

What are the symptoms of uveitis? �Symptoms of uveitis are highly variable and may include any of the

following: redness, pain, watering, inability to see bright light, floaters, and / or decreased vision.

What causes uveitis? �Uveitis occurs as a result of an immune reaction by our body to

antigens (substances our body considers foreign). This reaction may occur against infectious agents such as bacteria, fungi, viruses and even parasites. It may also occur in patients with existing autoimmune diseases such as Rheumatoid arthritis, Systemic lupus erythematosus etc. In a small subset of patients, uveitis can occur due to undeterminable causes.

What investigations are uveitic patients required to �undergo?

Uveitic patients often require a whole battery of investigations in order that the underlying cause of uveitis be determined and hence appropriately treated. These usually include blood and urine tests and/ or X rays. At times, a sample of the fluid from the patients1 eye may have to be subjected to lab tests.

How is uveitis treated? �Steroids are the mainstay of treatment in uveitis. Depending on the

location and the severity of the inflammation, they are used in the form of eye drops, eye ointment, injections around/ in the eye or injectable / oral medications. Anterior (and intermediate) uveitis is treated with topical steroids along with dilating eye drops which help in reducing the pain associated with inflammation. These drops are to be used until the inflammation has completely subsided. The dose, strength and duration of the drops are determined by your doctor who decides the treatment in accordance to the amount of inflammation.

Injection of the steroid around the eye (particular steroids) is used in certain cases of intermediate uveitis (or in macular edema as a consequence of uveitis). This results in slow release of the drug over a period of three to four weeks.

in the treatment of uveitis is immunosuppressives. These are especially reserved for patients intolerant to steroids, inflammation not resolving with only steroids and patients with certain systemic conditions like rheumatoid arthritis. The commonly used immunosuppressives include Methotrexate, Azathioprine, Cyclosporine , Mycophenolate mofetil, Cyclophosphamide and Biological agents.

What are the side effects of the drugs �used in the treatment of uveitis?

Both steroids and immunosuppressives have side effects that are often not serious and reversible following the discontinuation of the drugs. These drugs should always be taken as per your doctors’ instructions. Never start or stop these drugs of you own accord.

Topical steroids may cause cataract or an

increase in the intraocular pressure (glaucoma). Oral steroids may cause acidity, increase in weight and rarely diabetes, hypertension, osteoporosis and nervousness/ depression.

Immunosuppressives may cause bone marrow depression that is reflected as a decrease in your blood counts. Some of them also interfere with the normal functions of the liver, cause mouth ulcers, rarely sterility and secondary malignancies. Thus, periodic blood counts/liver function tests may be required and will be advised by your doctor when on these drugs.

Women in the reproductive age group are advised not to become pregnant when on treatment with immunosuppressives steroids. If you develop any infection while on treatment you need to take appropriate antibiotics immediately after consulting your doctor. In case you require to undergo any surgical procedure while on these drugs, please inform your treating doctor about the same.

Will uveitis recur after treatment? �It is important to remember that uveitis is a

recurrent condition and hence requires a prolonged and regular follow up with your doctor. Consult your ophthalmologist at the earliest signs of a recurrence. This will make treatment easier and resolution speedier.

What are the complications of �uveitis?

Uveitic patients may develop cataract (opacification of the lens), glaucoma (raised intraocular pressure) and macular edema (swelling of the central part of the retina) in addition to the inflammation itself. These complications may require additional medical or surgical management.

Bhuamlika is a wonder herb found growing as a weed of rainy season in India. It is commonly known as Hazardana or Bhooyi aamla in Hindi and Nelanelligida in Kannada. The plant is native of America and is chiefly distributed in tropical and sub-tropical parts of the world as a folk-medicine to treat liver disorders particularly due to hepatitis-B and jaundice, intestinal infection, diabetes etc. In traditional system of medicine, it is one of the essential ingredients of many formulations used in the treatment of bronchitis, leprosy, asthma and hiccup, while in unani system the fruit is used for wounds, scabies and ring worm. An infusion of root is a good tonic. The main compounds are lignins like phyllanthin and hypophyllanthin. The plant is found to grow extensively in many parts of Punjab, Uttar Pradesh, Tamilnadu, Maharashtra and Sikkim.

MEDICINAL USES ¾This is known as the best medicine for jaundice. ªThe Siddha system has been using this as medicine for decades. Almost all the medicines available in the market for jaundice contain Nelanelli. In Unani system, for skin disease and anthelminthic.Juice of the whole plant is given to jaundice ª

patients early in the morning. The dosage - five spoons. If the fresh drug is not available dry drug can be used. The leaf can be used in daily foodstuffs.Tender root juice is antidiarrhoetic and can be ªused every hour.Paste of the root and whole plant is good for ªexternal application for ulcers.The leaf made into paste with salt, if applied ªreduces itching in skin disease.In diabetes, this has most effective result. ªDose- 2-3 spoons of juice to be taken early in the morning or fresh leaves can be chewed.This is also a good medicine for skin disease, ªasthma, hiccough.Decoction of the leaf mixed with asafoetida fried ªin ghee controls abdominal pain.It controls metrorrhagia and can be used as food ªor by taking decoction thrice daily.Fresh juice of the leaf filtered can be used as ªeye-drops in redness of eye.Fruit juice or decoction improves immunity. ªIt acts as preventive drug to the healthy people during the spread of Cholera, Chikungunya and Dengue.

The length , color texture the curl the bounce and its style are all very significant magical tools to mesmerize .With a constant change in weather conditions there is a persistent challenge round the clock to our skin and the hair. Hair growth is cyclical and includes the period of growth, rest and shedding down. The hair on the scalp usually grows about 0.3mm/day. The center of the hair called as medulla supplies oxygen to the hair, Cortex around the medulla has an outer covering called the cuticle which contains the melanocytes which produce melamine the dark pigment, if melanin is not secreted the hair turns white. The outer covering of the root of a hair is called a follicle. The root of a hair is fixed in the dermis by the follicle. The cells of the papilla present at the root of the hair manufacture the hair. Ayurveda says that these activities are controlled by Bhrajaka Pitta a type of Bioenergy which takes care of the metabolism in the skin, enables the (paka) transformation or conversion of the substances in our nutrition and also externally helps in absorption of medications used in the form of (abhyanga) massage, (Parisheka) –sprinkling, application of pastes etc.

The color, luster of the hair depends on the constitution, environmental temperature, diet, activity and psychological state of the individual. Natural Sun and heat damage your hair vaporizing all the moisture needed for your hair to look healthy and shinny. Sunlight can make the hair brittle, decrease the luster and roughen the texture. The chemicals in the swimming pool and the salt present in sea water can damage the hair.

The hair of an individual with vata constitution is dry, rough, and scanty and has an ash like color

and these individuals sweat scantily. The individuals with pita constitution have scanty hair and they suffer from premature graying of hair and have baldness earlier than others. Kapha constitution individuals have thick black and ample hair and their sweat is sticky.

Oiling your ¾hair:

Appling oil and massaging the scalp increases the oxygen supply to the brain along with the circulation of the

cerebrospinal fluid. It also increases the level of life energy. , cures dryness, loss of hair and premature baldness .Vata constitution people should use sesame oil, Nilibringadi or Ksirabala taila . Pitta constitution ones coconut oil , Nili bringadi or Triphaladi and Kapha constitution ones can opt for sesame oil or nilibringadi taila.

Combing your ¾hair:

Combing your hair over the head regularly is beneficial for the hygiene of the hair by removing the dust, dirt, dead tissue and other wastes from the hair. Bend forward allowing hair to fall .Using only a wooden base natural bristle brush or a wide mouthed comb brings hair from the nape of the neck over the head and down to the ends. Then stand straight and brush from underside of the hairline down the strands to the ends now brush the top layers into place with long even strokes .After each stroke smooth hair with the hands to reduce static. Increase the number of strokes weekly starting with five and increasing by one a week until you find a comfortable routine. If oiliness develops reduce strokes.

Washing Hair ¾Do not wash your hair every day especially if it is

long. Hair that has not been washed for a day or two is easier to style .Infact washing your hair once in 3 to 4 days will make your hair more healthy. Washing your hair every day robs hair of essential oils and dries up your scalp and hair. It is not recommended to wash hair twice in a succession. First will wash your hair and second will strip your hair of its natural nutrients. By repeating you can irritate your scalp and make your hair more dull and sensitive. Washing your hair with hot water leads to hair loss, premature graying and is bad for eyes. Cold water or Luke warm water will make your hair shinier .Coupled with this if your scalp is greasy clod water prevents dilatation of sebaceous glands and moderates sebum production.

How to dry your hair naturally? ¾Let your hair dry naturally. ----a good absorbent ªtowel will suck up excess moisture and speed up dry time .Do not rub your hair. Rubbing will damage the strands and cause frizzy hair and split ends. Try to keep the hair shafts as smooth as possible .If your hair tends to become flat or too frizzy you can fluff it by finger combing.Moisture makes the hair supple so make sure ªyou get plenty of fluids .Do not waits till you are thirsty. Water hydrates your body and helps keep your hair healthy and shiny. Shiny healthy hair is a reflection of healthy body.Take a balanced nutritious diet -- A recipe for ªhealthy hair, and you will be really amazed at the difference it makes.

Hair care

The main cause of baldness is hereditary. Male baldness has certain typical patterns. Some men

have baldness only in the front or only at the vertex, whereas others are more extensively bald. But, almost all men have hair at the back and on the sides. The hair in this areas are permanent and do not fall. In hair transplant surgery, it is from this area (back and the sides) that hair is taken and transplanted to the front and vertex by the follicular unit hair transplant (PUT) technique .The entire operation is done under local anesthesia and patient goes home immediately after the operation.

FOLLICULAR UNIT HAIR TRANSPLANT (FUT) ¾From the back or sides of the head, a narrow strip of scalp 6 to 15 mm in width

is harvested. The donor defect is closed primarily, leaving no residual bald area. From this strip, individual hair follicles are separated into groups of 1 to 4 hairs. These plugs are properly cleaned and trimmed.

area where transplant is to be done. In these defects, the cleaned plugs are placed. Usually, no dressing is applied after the procedure.

There may be some temporary swelling of the forehead for 1 to 5 days. It takes 5 to 15 days for the healing of the donor site area. In the transplanted grafts, the hair may fall along with the scabs in 3 to 4 weeks after the surgery. New hairs starts to grow app. 10 to 12 weeks after the surgery, behave like normal hair, never fall and subsequently require haircuts. They can be combed to the style of your choice, oil and shampoo can be applied to them too. Normal hair growth is about 2 to 3 cms per month.

Graft method may require more than one sitting (operation) depending upon the extent of baldness. The time interval between each sitting is 6 weeks or longer. This procedure is done as an outpatient without any hospital stay. The patient may be allowed to travel 36 hours after the surgery.

Patient with small amount of hair at the back and sides are not good candidates for this surgery. Though hair growth is noted in everyone who undergoes this surgery, the density of the growth may be insufficient in about 10% of patients.

PRE OPERATION INSTRUCTIONS ¾Wash hair and face with LIQUID BFTADINE ªSCRUB (Not Betadine Solution) on the night before surgery and on the morning of the surgery. Do not use any hair oil, cosmetics or make up ªafter the hair wash. Please take the medicines according to ªinstructions given in your prescription. Please have a full meal 2 hours before the ªsurgery.

POST ¾ OPERATION INSTRUCTIONS

Normal diet. ªSee the doctor in 1 to 5 days ª

after the operation or as instructed by the doctor. You will be advised to wear a headband 3 to 5 days after the procedure.

You may use a loose cap for ªa few days, if you like.

Please take the medicines ªaccording to instructions given in your prescription.

Keep head elevated at all times (use 1 to 2 pillow ªwhile lying down) for 3 weeks from the day of surgery, Do not get the head wet on the day of surgery. ªUse BABY SHAMPOO from the 5th day to wash ªthe hair daily for first 15 days. You may use any shampoo after the 18th day.One may travel 36 hours after the operation. ªDon’t undertake any strenuous activities or ªexercises for 3 weeks after the surgery.Hair oil, Hair spray, etc.. should not be used for ª3 weeks after the surgery. The blood flowing in our veins and arteries is really living tissue. Nearly half of it

consists of red blood cells which carry oxygen to the tissues. Approximately one trillion (10,000 million ) new blood cells are formed in the bone marrow daily. The raw materials required in the production of these cells are iron, proteins, and vitamins, especially folic acid and B12. The red colouring matter, called haemoglobin is a protein which is composed of an organic iron-compound called “heme”. The globin is a sulphur -bearing protein which makes up 96 percent of the molecule. The formation of haemoglobin thus depends on adequate dietary supplies of iron and protein. Red cells have a lifespan of approximately 120 days and are destroyed and replaced daily. Each person should have 100 percent haemoglobin or about 15gms to 100 cc of blood, and a blood count of five million red cells per millimetre. A drop in the haemoglobin content results in anaemia and a consequent decreased ability of the blood to carry oxygen to the tissues.

Symptoms ¾A haggard look, with lines of strain, premature

wrinkles, greyish skin, and dull and tired looking eyes are the main symptoms of anaemia. Other symptoms include poor memory, weakness, dizziness, fatigue, lack of energy, shortness of breath on exertion, slow healing of wounds, headaches, mental depression, pale fingers, lips and ear lobes. The patient usually complaints of weakness, easy fatigue, lack of energy and dizziness.

Causes ¾There are two principal causes of anaemia. It

can result from reduced or low formation of red blood cells either due to defects in the bone marrow or an inadequate intake of iron vitamins, and protein. Heavy loss of blood due to injury, bleeding piles and heavy menstruation may also cause anaemia. A lack of digestive acid of hydrochloric acid needed for digestion of iron and proteins may also result in anaemia. Emotional strain, anxiety and worry usually interfere with

AnaemiaAnaemia, which means “ lacking in blood “, is among the most common diseases

affecting human beings. It denotes a shortage of rich red blood cells and colouring matter and usually results from consumption of refined foods

the manufacture of hydrochloric acid in the body. Anaemia can also be caused by a variety of

drugs which destroy vitamin E or by others which inactivate the nutrients needed in building blood cells. Chronic diseases such as tuberculosis, when accompanied by haemorrhage, may also result in anaemia. Other little known causes of anaemia are intestinal parasites or worms. Hookworm, pinworms, roundworms and tapeworms feed on the blood supply as well as on the vitamins. Twenty five hookworms can consume fifteen grams of blood every 24hours; a tapeworm can cause acute shortage of vitamin B12.

Symptoms of intestinal worms are itching at the rectum, restlessness at night with bad dreams, diarrhoea, foul breath, dark circles under the eyes and a constant desire for food. Garlic can help get rid of some types of intestinal parasites. Fresh papaya and grated raw carrot are also effective. After successful treatment for intestinal worms, perfect cleanliness should be observed to prevent recurrence.

The Cure ¾Anaemia is much more easily prevented

than corrected. A liberal intake of iron in the formative years can go a long way in preventing iron-deficiency anaemia. Diet is of the utmost importance in the treatment of anaemia. Almost every nutrient is needed for the production of red blood cells, haemoglobin and the enzymes, required for their synthesis. Refined food like white bread, polished rice, sugar, and desserts rope the body of the much needed iron. Iron should always be taken in its natural organic form as the use of inorganic can prove hazardous, destroying the protective vitamins and unsaturated fatty acids, causing serious liver damage and even miscarriage and delayed or premature births.

The common foods rich in natural organic iron are wheat and wheat grain cereals, brown rice, green leafy vegetables, cabbage, carrot, celery, beets, tomatoes, spinach; fruits like apples, berries, cherries, grapes, raisins, figs, dates, peaches and eggs. It has been proved that a generous intake of iron alone will not help in the regeneration of haemoglobin. The supplies of protein, too should

be adequate. The diet should, therefore be adequate in proteins of high biological value such as those found in milk, cheese and egg. Copper is also essential for the utilisation of iron in the building of haemoglobin. Vitamin B12 is a must for preventing or curing anaemia. This vitamin is usually found in animal protein and especially in organic meats like kidney and liver. A heavy meat diet is often associated with a high haemoglobin and high red cell count, but it has its disadvantages.

One cause of anaemia is intestinal putrefaction, which is primarily brought on by a high meat diet. Moreover, all meats are becoming increasingly dangerous due to widespread diseases in the animal kingdom. There are, however, other equally good alternative sources of vitamin B12 such as dairy products, like milk, eggs and cheese, peanuts. Wheat germ and soya beans also contain some B12. Vegetarians should include sizeable amounts of milk, milk products and eggs in their diet. For prevention of anaemia, it is essential to take the entire B-complex range which includes B12, as well as the natural foods mentioned above. Brewer’s yeast is a good source of complete protein. A liberal intake of ascorbic acid is necessary to facilitate absorption of iron. At least two helpings of citrus fruits and other ascorbic acid rich foods should be taken daily.

Beets are extremely important in curing anaemia. Beet juice contains potassium, phosphorous, calcium, sulphur, iodine, iron, copper, carbohydrates, protein, fat, vitamins, B1, B2, niacin B6, and C. With its high iron content, beet juice regenerates and reactivates the red blood cells, supplies the body with fresh oxygen and helps the normal function of vesicular breathing.

Water Treatment ¾A cold water bath is among the most valuable

curative measures in anaemia. The patient should be given carefully graduated cold baths twice daily. Cold friction, hot Epsom salt bath for five to 10 minutes once a week and an occasional cabinet steam bath are also recommended. Full sun baths are especially beneficial as sunlight stimulates the production of red cells. There are other important factors which are helpful in curing anaemia.

Deep breathing and light exercise like walking and simple yoga asanas should be undertaken to tone up the system. Sarvangasana, paschomittanasana, uttanpadasana and shavasana are recommended. Massage also helps to keep the blood level high.

In such a state, there is a split in our being -between the actor and the thinker. Commitment creates integrity. In this state, it is easier to bring forth the excellence in our lives.

It does ª not eat a dead prey. - Can we learn not to live on dead information?If there is a storm, it ª can glide on the strong breeze. It picks up a twig, flies high and as the male follows, it flies around to escape and finally drops the twig. Before the twig falls on the ground, the male catches the twig and gives it to the female. It repeats this act. If a male succeeds in catching the twig consistently, then it allows mating to take place - can we test before we trust?

It lay eggs on a cliff or at the edge ªof a high peak by making a nest of grass

entwined with thorns.

When the eggs hatch, the weight of the eaglets makes it uncomfortable as the thorns start pricking. Then the female pushes the eaglets out of the nest. As the eaglets are about to fall, the male picks them and brings them back to the nest. Meanwhile, the female removes the upper layer of

grass, so that the eaglets rest directly on the thorns that prick them. The eaglets are yet to test their wings to fly. Now the male pushes the eaglets out of the nest. This process continues till such time that the eaglets are able to foresee the danger in falling and start using their wings. Slowly they realise their ability to use their wings and start flying. - Difficulties are like divine surgery. Do not resist difficulties. Nature expects us to use our heart and head to discover new and wiser skills to fly in life.

In the overview of the 6 Tastes below, “-” after a doshic initial refers to a balancing (or decreasing) effect on that particular dosha, while “+” refers to an aggravating (or increasing) effect.

Sweet taste results from the combination of Water and Earth and is heavy, moist, and cooling by nature. In the West, sugary foods are most commonly associated with this taste. Sweet taste is also found in milk and milk products (like butter, ghee, and cream), most grains (especially wheat, rice, and barley), many legumes (like beans and lentils), sweet fruits (such as bananas and mangos), and certain cooked vegetables (such as carrots, sweet potatoes, and beets).

Sweet taste naturally increases bulk, moisture, and weight in the body. For this reason, it is excellent for building the body’s seven vital tissues (called dhatus) of plasma, blood, fat, muscles, bones, marrow, and reproductive fluids. Sweet taste also increases saliva, soothes mucous membranes and burning sensations, relieves thirst, and has beneficial effects on the skin, hair, and voice.

Sour Taste is composed of Earth and Fire and is hot, light, and moist by nature. It is commonly found in citrus fruits (such as lemon and limes), sour milk products (like yogurt, cheese, and sour cream), and fermented substances (including wine, vinegar, pickles, sauerkraut, and soy sauce). Used in moderation, Sour taste stimulates digestion, helps circulation and elimination, energizes the body, strengthens the heart, relieves thirst, maintains acidity, sharpens the senses, and helps extract minerals such as iron from food. It also nourishes all the vital tissues (dhatus) except the reproductive tissues (the exception being yogurt, which nourishes all the tissues).

Bitter taste is composed of Air and Ether and is light, cooling, and dry by nature. It is found in green leafy vegetables (such as spinach, kale, and green cabbage), other vegetables (including zucchini and eggplant), herbs and spices (like turmeric, fenugreek, and dandelion root), coffee, tea, and certain fruits (such as grapefruits, olives, and bitter melon). While Bitter taste is often not appealing alone, it stimulates the appetite and helps bring out the flavour of the other tastes. Bitter taste is a powerful detoxifying agent, and has antibiotic, anti-parasitic, and antiseptic qualities. It is also helpful in reducing weight, water retention, skin rashes, fever, burning sensations and nausea.

Salty taste is composed of Fire and Water and is hot, heavy, and moist by nature. It is found in any salt (such as sea salt and rock salt), sea vegetables (like seaweed and kelp), and foods to which large amounts of salt are added (like nuts, chips, and pickles). Due to its drying quality in the mouth, it may seem counterintuitive to think of Salty taste as moistening. The element of Water in its composition, however, relates to its water retaining quality. Salty taste falls somewhere between Sweet and Sour tastes with regard to its heavy and moist qualities. While Sweet taste stimulates the greatest water retention and weight gain in the body, Salty taste will have similar effects when used in excess by any of the doshas.

In moderation, Salty taste improves the flavour of food, improves digestion, lubricates tissues, liquefies mucous, maintains mineral balance, aids in the elimination of wastes, and calms the nerves. Due to its tendency to attract water, it also improves the radiance of the skin and promotes overall growth in the body.

Pungent taste derives from the elements of Fire and Air and is hot, dry, and light. It is the hottest of all the 6 Tastes and is found in certain vegetables (such as chilli peppers, garlic, and onions), and in spices (like black pepper, ginger, and cayenne). In small amounts, Pungent taste stimulates digestion, clears the sinuses, promotes sweating and detoxification, dispels gas, aids circulation, improves metabolism, and relieves muscle pain.

Astringent taste results from the combination of Air and Earth and is dry, cooling, and heavy by nature. It is the least common of all the 6 Tastes and can be found in legumes (such as beans and lentils), fruits (including cranberries, pomegranates, pears, and dried fruit), vegetables (such as, broccoli, cauliflower, artichoke, asparagus and turnip), grains (such as rye, buckwheat, and quinoa), spices and herbs (including turmeric and marjoram), coffee, and tea. Astringent taste is not as cold as Bitter taste but has a greater cooling effect on the body than Sweet taste.

Astringent taste is classified more in relation to its effect on the tongue than its actual taste. It creates a puckering sensation in the mouth (such as cranberries) or a dry, chalky feeling (such as many beans). Foods like broccoli or cauliflower have a mildly Astringent taste that is less detectable. Dry foods such as crackers and chips, most raw vegetables, and the skins of fruits also have Astringent qualities.

WHAT HAPPENS IF YOU HAVE BEEN LOGGING 6,000 TO 10,000 STEPS PER DAY ON YOUR PEDOMETER AND YOU SUDDENLY CUT YOUR STEPS PER DAY TO LESS THAN 2,000?

A Danish researcher says your risk of diabetes, heart disease, and premature death zooms

up -- and it only takes two weeks of reduced activity to produce the bad effects. The results of the study by Dr. Rikke Krogh-Madsen, from the Centre of Inflammation and Metabolism in Copenhagen were published as a letter in the March 19, 2008 issue of the Journal of the American Medical Association. The study used healthy young men in their mid-20s who were not overweight, were non-smokers, and who had no family history of diabetes, medication use or other physical abnormalities. One group normally walked 6,000 steps per day, and a second group normally walked 10,000 steps per day. All were asked to reduce their walking by simple methods such as taking a car for short trips instead of walking or biking, and taking elevators instead of stairs. After cutting their steps to under 2,000 steps per day, in both groups the amount of insulin circulating in their blood

climbed 60%. The researchers say this is a sign that the body is no longer efficiently processing energy from food and is increasing insulin to try to process the food energy. This effect was seen with only two weeks of reduced activity. In the group that normally walked 10,000 steps per day, cutting their steps to under 2,000 produced additional increases in heart disease risk factors. Trigylceride and C-peptide levels increased and they had a 7% gain in abdominal fat without overall weight gain. The researchers were amazed that these healthy young men had impaired metabolisms in only two weeks, without the usual warning sign of weight gain.

The 2008 Physical Activity Guidelines for Americans state that “Adults should strongly consider walking as one good way to get aerobic physical activity. Many studies show that walking has health benefits and low risk of injury and the most widely accepted mode of

physical activity is walking Physical Activity refers to physical

exercise. Inadequate physical activity is said to be a major contributor to health problems. Therefore health experts recommend at least 30 minutes of moderate exercise a day, at least 5 days a week, in intervals of ten-minutes or more (Surgeon General 1999). An increasing portion of the population, including many children, lack regular physical activity. Although there are many ways to be physically active, walking is one of the most practical ways to increase physical activity among a broad population. Walking tends to be particularly important for elderly, disabled and lower-income people who have few opportunities to participate in sports or formal exercise programs. Walking is a major component of daily physical activity, and is the most common form of exercise. Walking is self regulated in intensity, duration and frequency, and

can be an important indicator of a person’s health and fitness status. Among healthy individuals, 10,000 steps daily have been estimated to be of value in maintaining desired health benefits.

Homo sapiens are walking animals. Walking is a fundamental activity for physical and mental health, providing physical exercise and relaxation. It is a social and recreational activity. Environments that are conducive to walking are conducive to people. Walking is also a critical component of the transportation system, providing connections between homes and transit, parking lots and destinations, and within airports. Often, the best way to improve another form of transportation is to improve walkability.

WHAT IS WALKING? ¾Walking is a complex activity which requires

the cooperation and control of the whole body. Patients readily accept the necessity for practicing preparatory activities provided they can see and understand that they lead towards walking itself.

PRINCIPLES OF WALKING ¾Some principles are to be observed by the

individual while walking such asThe person must learn the correct pattern of ªwalking from the start, because movement patterns learnt and repeated accurately become habitual very quickly.Sufficient and /or aids must be provided to allow ªa correct pattern of walking. In many cases, the correct or optimum pattern of movement is only possible with the assistance of suitable support, e.g. parallel bars, crutches or sticks, to help balance or to reduce weight-bearing on one or both legs to an acceptable magnitude. METHOD & MECHANISM OF �WALKING

The activity of the legs alternates during walking i.e one leg supports the body weight while the contra-lateral limb swings forwards to replace the foot in a new position ready to receive the body weight when it is transferred

Phase I- Stance ¾From walk, standing body weight is moved

forwards until the body is supported in vertical alignment over the forward foot; the line of gravity will reach the floor through the transverse tarsal joints. The toes of the rear foot remain on the floor to help maintain balance. This position of stance must be well learnt as progression is achieved by moving from one position of stance to another.

Phase II- Swing and Heel strike ¾The supporting function of the forward leg is

maintained while the rear leg is lifted and flexed to clear the floor as it swings forwards; it is then extended at the knee and the foot is dorsi-flexed to allow the heel to be brought to the ground i.e. heel strike. Thus this foot has been replaced in a new position ahead of the supporting leg and it must now be termed the forward foot. The balanced support of the other leg must not be disturbed during the practice of the swing and heel strike.

Phase III- Weight transference and return to ¾stance

Then there is a return to stance position but with the other foot forward. The body weight is transferred from what is now the rear leg to be supported over the forward leg. Using the heel as a pivot, the hip extensors of the forward leg are mainly responsible for initiating the forward shift of the pelvis in combination with lumbar and hip extensors of the rear leg during normal walking on level ground.

Once phase 3 is completed, phase 2 follows and then these two phases are repeated alternately so that the patient moves from one stance to the other.

BENEFITS OF WALKING �Many scientific studies has shown that walking

has been significantly associated with a lower risk of cardiovascular disease, stroke,coronary heart disease, type 2 diabetes and hypertension. Consequently, walking also reduces expenditure on medication needed for these conditions.

According to the magazine, ScienceDaily (May 14, 2012) — A walk in the park may have psychological benefits for people suffering from depression. In one of the first studies to examine the effect of nature walks on cognition and mood in people with major depression, researchers in Canada and the U.S. have found promising evidence that a walk in the park may provide some cognitive benefits. The researchers found eight relevant studies featuring a total of 341 people. Overall, the combined results of these trials suggested that walking reduced the symptoms of depression.

Walkability Index rating (based on residents’ evaluations) affect walking and biking activity, and health outcomes in 16 diverse neighborhoods. The analysis reveals a statistically significant association between improved walkability and more walking and cycling activity, lower body mass index (BMI), and lower hypertension. Regression analysis indicates that people living in more walkable neighbourhoods are more likely to walk for at least 10 daily minutes and are less likely to be obese than those living in less walkable areas, regardless of age, income or gender.

Heart disease is a large health problem affecting India today. Heart disease happens when your arteries become narrowed through the process, called Arteriosclerosis. When this happens the blood and oxygen supply to the heart muscle is restricted. The lack of blood and oxygen is caused by fatty plaques, which a porridge-like sludge is made up of fatty deposits of cholesterol, cell waste and other substances that accumulate along the walls of the coronary arteries. These form raised patches narrow the arteries, reducing the space through which blood can flow, and at the same time the blood becomes more prone to clotting. Over time, the heart muscles are starved of oxygen and nutrients carried by the blood and it begins to weaken and parts of it may even die. This process begins in most people during childhood, and progresses as we get older.

MICROLARYNGEAL SURGERY

Surgery to remove a tapeworm Columbia Asia Hospital, Hebbal conducts a rare surgery to remove a tapeworm from a

patient’s voice box:

Thirty four-year-old, Anil Kumar (name changed) was suffering from intermittent hoarseness of voice since two

years. While he had no history of throat pain or voice abuse the situation turned worse when he started having excruciating pain and could barely manage to talk. The doctors he consulted prescribed him medications but were not able to detect any problem or give satisfactory response.

With little hope of regaining his voice, Kumar and his family decided to consult Dr Santosh.S Consultant ENT, Head and Neck Surgeon, Columbia Asia Hospital, Hebbal, Bangalore. On further examination and investigation it was discovered that there was a grayish semi transparent mass inside the voice box. “The mass was a parasitic infestation caused by a tapeworm found in the intestines of pigs and a leading cause of multi-organ infections,” says Dr Santosh.

What made it challenging was the fact that the worm was lodged right in his voice box. In a two hour long surgery the team led by Or Santosh conducted MLS (Microlaryngeal surgery) which removed the worm from his vocal cord. The patient has recovered extremely well, has recovered his normal voice and has resumed a normal life.

About Cysticercosis:Cysticercosis is a parasitic infestation by pork tapeworm

and is a leading cause of multiorgan infections. Involvement of the central nervous system, subcutaneous tissues, and orbits are very common. Involvement of the vocal cords or voice box by cysticercus is a rare occurrence. Only 2-3 cases have been reported across the globe. Confirmatory diagnosis is usually done by histological examination from affected part.

Cysticercosis is no longer an endemic disease of the developing countries. It has become a ‘global problem’, because of immigration from endemic areas. Tapeworm infestation is commonly seen in developing countries due to a combination of factors like crowding and poor sanitation leading to faecal contamination of food and water, close contact between humans and pigs and thus more opportunities for transmission of disease. Tapeworm infestation is common in rural areas than in urban areas, due to lack of a proper sewerage system and lesser access to health care. Infestation with the adult worm takes place by the ingestion of uncooked or poorly-cooked pork containing encysted larvae of tape worm, Taenia solium.

Known as the “master of the endocrine orchestra”, the pituitary gland is a tiny but

powerful structure situated just below the brain, in a bony recess called the sella turcica. Although it is only about the size of a pea, it exerts a strong influence over all the endocrine organs of the body such as the thyroid, the adrenals and the organs of reproduction (the ovary in the female and the testis in the male). In addition, it exerts important influences on growth and on lactation.

Situated just above this gland is the crossing of the optic nerves (the nerves that carry sight from

the eyes to the brain) called the optic chiasm. The pituitary gland is a common site for tumors, called pituitary adenomas. These can be small (<10 mm) microadenomas or large (>10 mm) macroadenomas. Tumors can be functional (secrete one or many hormones) or nonfunctional (non-secretory). Pituitary tumors are common but difficult to diagnose because of a wide variety of symptoms with which they present. Some present due to

Compression effects from the tumor on vital areas ªof the brainLike a “stroke” with headache, vomiting and ªsudden unconsciousness due to a bleed into an existing tumor. This is known as apoplexy and can be life-threatening unless immediate surgery is performed.

Most patients with pituitary tumors are detected when they present to an eye specialist (ophthalmologist) with worsening of vision. Patients with compression of the optic nerves typically complain of inability to see objects in the outer halves of the visual fields (known as bitemporal hemianopia). Some patients may also present with complete loss of vision in one or both eyes.

Pituitary tumors are best diagnosed with a contrast MRI scan of the brain, which beautifully demonstrates the tumor, its size and extension, and compression on any surrounding structures. Patients diagnosed with these tumors also require a complete hormonal evaluation to detect any derangements in

hormone secretion, which can influence the management of these tumors.

Most tumors need surgical removal which is possible in all but the largest tumors. Approaching these tumors through the head is complex since these tumors are deeply located inside the skull.

Therefore, pituitary tumors are often better approached through the nose, through a convenient air space in the skull known as the sphenoid sinus which lies just below the pituitary. This approach has tremendous advantages since it does not require opening of the skull or retraction of the brain, leaves no scar, often takes less time and requires shorter hospitalization. In the past, this approach was used in conjunction with an operating

microscope and a speculum placed in one nostril. Diagram showing the route of approach to

through both nostrils, with no speculum and using extremely thin endoscopes. The endoscopic approach gives the surgeon a much better visualization, better illumination and causes less tissue damage.

Most patients require hospitalization for 3-4 days.

Many tumors, even fairly large ones can be nearly completely excised with very little morbidity through this approach. Most patients have excellent recovery of their eyesight after the surgery and most have good hormonal response as well, depending on the type of hormone affected.

ACHALASIA CARDIA A rare disease

Patient who had not eaten for years, operated on to remove esophagal obstruction caused due to food pipe motility disease called Achalasia Cardia.

Raghavendra (61) was admitted to Apollo Hospital in February, 2012. Little did he know that he would be living a new life in few months time and happier times awaited him. Raghavendra was a sound engineer for 30 years and then an agriculturist in Shimoga district. Since the tender age of 9 he had a strange disorder. He had difficulty in swallowing for about 10 years and it had gone undiagnosed.

At 20, his visit to a local doctor in Shimoga, threw light on Raghavendra’s ailment. He was informed that his food pipe was blocked and he needed to be operated upon. He got a surgery done in Shimoga to remove the blockage from his esophagus after which he got some relief. He was fine for next 20 years till 1990 when the problem resurfaced and he was unable to eat. Pain in the food pipe was so intense that he could not swallow even a few morsels of food. Following which he became very weak and lost around 20 kgs of weight. About the same time he came to Bangalore and visited many private hospitals, seeking a solution to his miserable existence. Once again he was on endoscopic dilatation just to dilate his food pipe so that he was able to eat. Endoscopic dilatation continued for 20 years and then end of last year his surgeon refused to continue dilatation. A complicated surgery involving removal of food pipe and stomach was the only option left.

Dr. S. T. Gopal, Consultant Medical Gastroenterologist says “Achalasia cardia can

be present at any age. It is a condition where the lower part of the food pipe is very tight due to contractions and as a result the person finds it very difficult to swallow food. Usually this is managed by making the constriction open-up by an endoscopic day procedure (non surgical method). If this first choice fails and especially

like in this situation where the stomach and food both are damaged, surgery is the best choice”.

Raghavendra’s family was ready to do anything to get him back on track. And that is when he came into contact with Dr. Ravishankar Bhat, Senior Consultant GI & Liver Transplant Surgery at Apollo Hospitals.

According to Dr Ravishankar Bhat, “When I first met Raghavendra I was shocked to see that he was emaciated and nutritionally challenged. He weighed 40 kgs. Not surprising since he had not been eating for years and

had been dependent on nasal feeding. Initially, we had put a tube straight to his stomach for a month to improve his nutritional levels. He had a very unique, disease of food pipe where the motility of food pipe is hampered. And the patient is unable to eat. In addition to this Mr.Raghavendra had massively thickened stomach, the cause of which couldn’t be found and concluded that it could be due to gastritits medicine (Protone pump inhibitor) he was consuming for more than two decades”

Raghavendra was operated in mid June. The surgery is called “Transhiatal oesophagectomy with total gastrectomy and colonic transposition”. In this procedure, the blocked and infected part of esophagus i.e., food pipe was taken out along with the excision of the stomach and remaining esophagus was directly connected with the Large intestine. The surgery lasted for seven and half hours as Mr Raghavendra was operated before and had plenty of adhesions (i.e. Intestines, liver are stuck to each other). After which the patient had a 3 week stay at the hospital. It is amazing that within four days of surgery, Raghavendra started walking and within a week he started taking solid food and discharged within 4 weeks.

Dr Ravishankar Bhat said, “He had undergone a surgery to correct the problem 40 years ago but received limited result. Multiple endoscopic procedures were done to correct it with transient benefit. Due to chronic medication his stomach wall had thickened and became non-functional. We then surgically

removed the patient’s food pipe and stomach. To re-establish continuity we used large intestine which is joined to the food pipe in the neck and small intestine in the abdomen, thus helping Raghavendra to resume eating normally”.

His family is very happy on his fast recovery and his wife says, “Raghavendra is a very strong person. He has gone through this phase very patiently and he performs all his daily activities on his own.”
