Module Transportation - 3 mods delivered to site during day, for installation after 3 previous mods brought on Friday and stored on site

over weekend. One mod stored at corner of Dean and 6th overnight because of need to fix gasket, according to Skanska who felt it was OK

to store there because there was no event going on at the arena where the loadout would be inhibited by the mod on a flatbed. Was stored

there to be out of the way of the other mods brought overnight.

Module Installation - 5 mods installed on 6th Floor bringing total installed on 6th floor to 23 out of 36. Mod completion inches up to

15.1% from 14.5%; 140 of 930 installed in 132 work days = an avg. of 1.1 mods per day.

Module Fabrication - Ongoing at Navy Yard, presumably, working on 9th floor mods with a workforce in the high 140's.

MEP -

Additional New Observations: Joined by STV's Phil Taylor and Phil Amicone for tour of roof, 4th and 5th floors of building for

preliminary safety inspection.

less than 1+1/8" from gasket plate to adjoining gasket plate or more than 2+1/8" between plates then warranty void. Joel cau ks the 'crucifix',

where 4 mods meet, to seal any openings.

On 6/6/14 sent pictures of installed/discolored dry wall cross sections to Skanska to ask whether mold may be suspected. No response to

date.

Paper documenting DOB's approval (signed 2/2/14) of no sidewalk shed on Dean received on 3/21/14.

FCR stated on 6/3/14 that mods installed starting (the week of) 6/9 will be floors 6-9.

Information to be requested: More information on quality issues noticed with the mods - exterior: torn gaskets, dents and scratches tothe façade; interior: lack of completeness of mods; utility lines not lining up; utility lines not installed in mods either on purpose or for another

reason.

potential of it in insulation which would then have to be removed, FCR will send copy to STV once they see the report - they are still waiting.

4/8: Jury is still out on gaskets which may be under increasing stress under weight of additional floors.

Previous observations: Front façade is 33-35' from the ped walkway. Flatbush façade is 12' from the edge of the sidewalk shed. Height

exceeds 2 X the distance between the building and the ped crosswalk at Flatbush across Dean.

 At Navy Yard Wed., 10/16, FCR stated that first mods would arrive on site in late December/early January and that there would be a backlog

of 30-40 (1-2 floors worth) to 80 (3 floors worth) mods completed at that time which it was felt would be sufficient for production to keep

ahead of erection so there would be no downtime during mod erection at the B2 site.

Additional New Observations: Mods going in smoothly, as viewed from rooftop. Asked Skanska whether MG McGrath had any

problems with gaskets as installed on any of the 6th floor mods. Skanska responded there was one mod of concern. Took measurements to

send to the designers/plant to see if something is off. Something has been off on the line of mods (western section).

New Information received: Brace frame will be for 2 days after completion of 7th floor which will be either a week from today or a week

from Friday - so that brace frame work would start either Fri., 6/27 or Mon., 6/30.

New Information requested: In email response to FCR's SS, who sent the 2nd report by Skanska's environmental consultant, asked

whether any type of air monitoring can take place to measure potentially high mold spore counts in a unit where any mold is not visible, i.e in

upper or inner layer of wal board in ceiling/wall. Cited leak day on 6/13, after e.c's most recent site visit, when subsequent leak damage is

not that evident.

Previous observations: Front façade is now less than 33-35' from the ped wakway due to the 6th floor Dean St. transfer beam facade

protruding 1-2' closer to the street than the 1st-5th floor facade. Flatbush façade is 12' from the edge of the sidewalk shed. Height exceeds2 X the distance between the building and the ped crosswa k at Flatbush across Dean.

 At Navy Yard Wed., 10/16, FCR stated that first mods would arrive on site in late December/early January and that there would be a backlog

of 30-40 (1-2 floors worth) to 80 (3 floors worth) mods completed at that time which it was felt would be sufficient for production to keep

ahead of erection so there would be no downtime during mod erection at the B2 site.
