Waiver and Release Agreement has to be signed and dated prior toadmittance to our dog daycare, boarding, obedience classes private orgroup, agility classes and or a visiting guest on the property.

WHEREAS, The undersigned is participating in Atlanta Dog Trainer LLC obedience

Classes, Agility Class, enrolling their dog in doggie daycare or a guest visiting AtlantaDog Trainers classes or facility as indicated by my signature, I understand and agree to

all the following statements contained within this waiver and release agreement.

The undersigned recognizes and understands that their animal will be in close proximity

to other animals. Also, there will be activities their animal will be required to participate

in including running, jumping and engaging in activities requiring use of AgilityEquipment. Such animals will be climbing, jumping on and off equipment, such

activities can cause injury. I release any and all liability and financial obligation of

Atlanta Dog Trainer LLC, their staff, agents, administrators, his or her heirs, contractors,

maintenance staff and trainers of foreseen and unforeseen, known and unknown injuries

to undersigned and their pet hereby now and forever.

WHEREAS, the undersigned recognizes that participating in activities with their doginvolves risks for owner and their animal, included and not limited to being scratched,

bitten, falling, tripping, injury on or due to any agility equipment, dog fights,

transmission of diseases and any and all incidents that might occur on the property12900 Freemanville Road Alpharetta, GA 30004 with Atlanta Dog Trainer LLC and

5660 Howard Road Cumming, GA Atlanta Dog Trainer Pet Resort LLC

WHEREAS, in order to join in group classes, private lessons, agility class or in doggie

daycare the undersigned does hereby release Atlanta Dog Trainer LLC at 12900

Freemanville Rd Alpharetta, Ga , their agents, administrators, his or her heirs,contractors, maintenance staff, officers, directors, staff and trainers of any and all liabilitywhatsoever associated with the undersigns participation at the facility in doggie daycare,

group classes, private lesson and agility classes/lessons and the undersigned does furtherindemnify and hold such entities and persons harmless from loss, injury or damage which

might result to him/her or their pets.

The undersigned executes and delivers this release and covenant not to sue and assumes

all risks and liabilities in order to join any and all classes and or doggie daycare held at

12900 Freemanville Rd Alpharetta, Ga, with Atlanta Dog Trainer LLC.

Undersigned has read and understands this waiver and understands all liabilities, risksand any financial obligation is assumed by the undersigned and any and all risk, liabilitiesor financial obligation is released from
