As Chair of the Board of Directors, I am inspired by how deeply LCH impacts our community, and by how deeply our supporters care about our mission. We continue to evolve and expand to serve southern Chester County. Dear Friends,

What a pleasure it is to greet you on behalf of LCH. As Chair of the Board of Directors, I am inspired by how deeply LCH impacts our community, and by how deeply our supporters care about our mission.

We continue to evolve and expand to serve southern Chester County. Our primary care locations in Kennett Square and Oxford provide key health care, mental health, and social services to thousands of people every year. Our West Grove specialty care site now houses our women’s health services, our expanded dental center, and soon our growing pediatric practice.

LCH works to connect with all in our community who need our services, ensuring that they feel safe, supported, respected, and valued here—whether they are an immigrant family struggling with legal matters, or a working family with inadequate health care coverage. Those we serve need the compassionate, integrated health and community services we offer.

Our members can and do thrive. We are extremely proud of the positive impact we have had on so many who have entrusted us with their care. You are an integral part of this impact. accomplishments

West Grove specialty care siteLCH’s Women’s Health team anchored our West Grove facility in the summer of 2018, and they were joined in January 2019 by an expanded Dental Center. This coming winter, our full-time pediatrics team, including a pediatric mental health specialist, will round out the specialty services LCH offers in West Grove.

While the number of our patients without insurance has decreased thanks to the

Affordable Care Act and our enrollment support services with LCH Social Assistance

staff, we have seen an increase in the number of underinsured patients who still

need a place to access affordable care.

Through our expanded services and new locations, we have more than doubled the number of members

we have cared for over the past six years.

88.2% of LCH patients who test positive for symptoms of depression receive follow-up counseling from one of LCH’s Behavioral Health Consultants.

67.2% of our patients with diabetes achieve controlled blood glucose levels under our care; and 70.7% of our patients with high blood pressure achieve controlled blood pressure, compared to 55.2% from the previous year.

Almost 95% of pregnant women who receive prenatal care at our West Grove Women’s Health Center have healthy, full-term babies – a rate higher than that in Chester County as a whole.

Welcome dr. mariana izraelson, ceo of lchDr. Izraelson brings more than ten years of management experience of health care and behavioral health care organizations, most recently serving as Executive Director of Shepherd’s Clinic in Baltimore. She holds a B.A. in Psychology from the University of Maryland, and a M.A. and Ph.D. in Clinical Psychology from Argosy University. Dr. Izraelson is originally from Argentina and is bilingual in Spanish and English. She is excited to begin leading LCH and meeting all of our friends and supporters when she joins us at the beginning of December.

enhanced inteGrated services throuGh team-based careIn October 2018, LCH began to implement the Team Based Care model for our services. This model uses coordinated care teams to serve a common panel of patients. A typical care team at LCH includes a provider (physician or nurse practitioner), a nurse, a medical assistant, a member relations specialist, a social assistance caseworker, and a behavioral health consultant. The clinical team sees each patient on every visit, so each patient gets to know their entire care team over time. When behavioral health or social assistance needs are identified, each clinical team has a specific behavioral health or social assistance team member they work with to ensure that their patients can access the care they need. In this proven model of care, patients feel better connected to and supported by their care team, providers experience a better work environment and less burnout, and clinical outcomes are improved over time.
