Declaration of Agreement *

I agree, that my address data will be recorded to the mentee file of the Big Sister project and that my E-Mail address

will be made accessible to the project participants. The private information and wishes concerning the mentoring

programme and the mentor will be treated confidentially and will be exclusively used by the Big Sister project

coordinator.

I herewith consent to meet my mentor on a regular basis over a period of nine months, to participate in the Big

Sister framework programme and in the project evaluation.
