Application for a ‘Regular Provisional Residence Permit -
persons making a journey in connection with family
reunification with an asylum seeker’ (647)
Read the explanation before you start to fill out the form.
For whom is this form intended? With this form you can submit an application for a Regular Provisional Residence Permit (mvv) under the scope of a
journey in connection with family reunification for your partner, spouse of (foster) child, who are staying abroad and
who want to join you in the Netherlands. It is a condition that you have a Dutch asylum residence permit. Are you a
unaccompanied minor holder of an asylum residence permit? Then you can use this form to submit an application for
an mvv for your parent(s).
In this form you are referred to as ‘sponsor’. Your family member is referred to as ‘foreign national’. The mvv
application is only granted if the partner, parent, spouse or (foster) child for whom the mvv is applied for:
• actually belongs to the family of the sponsor in the Netherlands; an
• the application has been submitted within 3 months after the asylum residence permit has been granted to the
sponsor (this is the period within which you can apply for family reunification).
Do you and/or the foreign national for whom you are applying for the mvv not comply with these conditions? Then
your family member cannot be granted an mvv for the purpose of family reunification and cannot be granted a
derivative asylum residence permit. In this case you must submit an application for a regular residence permit within
the context of ‘family members and relatives’. For this purpose you must use the application form ‘Application for the
purpose of residence of ‘family members and relatives’ (sponsor)’.
You do not have to pay any fees for an application for an mvv under the scope of a journey in connection with family
reunification. However, you do have to pay fees for an application for a regular residence permit.
How do you fill out this form? This form comprises different appendices. Which appendices you need to fill out depends on your own situation. Only submit your application once you have completed filling out this form, signed and you have gathered together all the requested documents and evidence. If your application is incomplete, the IND will be unable to assess your application properly.
Processing of personal data The Immigration and Naturalisation Service (IND) processes personal data when it processes your application, notification, or request. This means that if needed the IND will request data from you yourself and other organisations or persons. The IND also uses and stores data and shares them with other organisations. When doing so, the IND strictly adheres to the stipulations of privacy legislation. For instance, the IND must treat data safely and with due care. The law also gives rights. 3 Means of evidence
You must collect means of evidence from abroad for your application. All this means of evidence must have been
translated into Dutch, English, French or German by a translator certified at a Dutch court. The means of
evidence does not have to be legalised. Which means of evidence you have to submit depends on which family
member wants to come to the Netherlands. Below you can read which means of evidence is requested for each
family member. Only submit an application when you have collected all the means of evidence.
Identity document
For each family member you must send a copy of the front and back of an identity document. An identity
document is an official document issued by the authorities of the country of origin, which shows the
personal data of the family member and a passport photo. For example, a passport, identity card or any other
document that meets the above requirements.
Document examination
It is possible that the IND wants to investigate certain documents. It is therefore important that you have the
original documents, issued by the authorities of the country of origin. The documents which have already been
examined during your asylum procedure do not have to be examined once again.
Is there a biological relationship?
Then the IND can start a DNA test. This only applies when the IND has established that you are unable to meet
the standard of proof. This is the case when you cannot have the required means of evidence at your
disposal through no fault of your own. This is why you have to complete, send and enclose the appendix
Application for DNA test in case of a family reunification. Will a biological parent remain behind? Then also
complete the details of this parent.

Select the family member you want to bring. You can only tick one family member per application form. If you tick more than one family member, then your application will be no longer valid. For each family member, you submit an application form, the means of evidence requested below and an identity document from that family member. If you are in doubt, then contact the IND. If you have any doubts, please contact the IND.
Your statutory spouse or registered partner
When it concerns a marriage, enclose the following documents with your application:
• A copy of the marriage certificate issued by the authorities of the country of origin; and
• A copy of an extract from the register of births, deaths, marriages and registered partnerships, which shows
your family composition; and
• A completed appendix ‘Questionnaire (married) partner for asylum in connection with family reunification’.
When it concerns a registered partnership, enclose the following documents with your application:
• A copy of the certificate of the registered partnership, which was issued by the authorities of your country of
origin.
• A completed appendix ‘Questionnaire (married) partner for asylum in connection with family reunification’.
Your partner with whom you are married in a traditional/religious sense
Enclose the following evidence with your application:
• A copy of the religious/traditional marriage certificate; and
• A copy of the evidence which shows the religious/traditional marriage is registered in the registers of the
country of origin; and
• A copy of an extract from the register of births, deaths, marriages and registered partnerships in your
country of origin, which shows the family composition; and
• A completed appendix ‘Questionnaire (married) partner for asylum in connection with family reunification’.
Your unmarried partner
Enclose the following evidence with your application:
• A completed appendix ‘Questionnaire (married) partner for asylum in connection with family reunification’.
• Evidence to substantiate your relationship.
Your parent(s). You have a residence permit asylum and you were under the age of 18 when
you applied for asylum. You want to submit an application for your parent(s).
Enclose the following evidence with your application:
• A copy of your birth certificate issued by the authorities of your country of origin. You do not have a birth
certificate? Then enclose a family booklet or an extract from the register of births, deaths, marriages and
registered partnerships with your application; and
• A copy of the evidence which shows who your guardian is in the Netherlands. For example, the guardianship
order of the court.
Do you not have these documents? Then enclose the following document with your application:
• A completed and signed appendix ‘Application for DNA test in case of a family reunification’. Will a parent
remain behind? Then also complete the details of this parent.

Your biological child
Enclose with your application a copy of the birth certificate of the child. Or if you do not have a birth certificate a
copy of evidence which shows the biological relationship between the child and the biological parents. For
example, a family booklet or an extract from the register of births, deaths, marriages and registered
partnerships.
Do you not have these documents? Then enclose the following document with yourapplication:
• A completed and signed appendix ‘Application for DNA test in case of a family reunification’.
If your child is 15 years or older, then also send:
• A completed and signed appendix ‘Questionnaire family relationship child’.
• The ‘Declaration of civil status’ appendix, completed and signed by the child.
When the other parent of your child remains abroad, also enclose the following evidence with your application:
• The ‘Declaration of consent of the parent remaining behind’ signed by the parent remaining behind. With
this declaration the parent gives permission for the child to travel to the Netherlands.
• A copy of the identity document (for example, passport) including the signature of the parent remaining
behind;
• A copy of evidence, issued by the authorities of the country of origin, which shows the biological relationship
between the child and the parent remaining behind (for example, a birth certificate or an extract from the
register of births, deaths, marriages and registered partnerships).
• If it is not possible to have the declaration of consent signed by the parent remaining behind, then complete
the ‘Statement regarding the absence of the ‘Declaration of consent of the parent remaining behind’. In this
declaration, you state what you did to come into contact with the parent remaining behind.
Attention! Is there no evidence which shows that the parent remaining behind is the biological parent of the
child? Or does the parent remaining behind not have an identity document with signature? Then the parent
remaining behind must go to a Dutch representation to yield DNA material for a DNA test. In this way it can be
examined whether the remaining parent is the biological parent of the child. The parent must also sign the
‘Declaration of consent of the parent remaining behind’.
If your child is 25 years or older, then also send
• A completed and signed declaration “more than normal emotional ties”.
Your foster child
Enclose the following evidence with your application:
• A copy of the birth certificate of the child. Or if you do not have a birth certificate a copy of evidence which
shows the biological relationship between the child and the biological parents. For example, a family booklet
or an extract from the register of births, deaths, marriages and registered partnerships.
• In case you and the child are related: a copy of evidence which shows the family relationship between you
and the child.
• Evidence to substantiate the relationship between you and your foster child.
If the biological parents are deceased, then also enclose with the application:
• A copy of the death certificates of the biological parents of your foster child. These certificates are issued by
the authorities of the country of origin; and
• A copy of evidence which shows that you are the guardian of your foster child (e.g. the custody papers);
and
• A completed and signed appendix ‘Questionnaire family relationship child’.
If a/the biological parents(s) remains abroad, then also enclose with the application:
• A copy of evidence, issued by the authorities of the country of origin, which shows the biological relationship
between the child and the parent remaining behind (for example, a birth certificate or an extract from the
register of births, deaths, marriages and registered partnerships).
• A completed and signed appendix ‘Application for Questionnaire family relationship child’.
• The ‘Declaration of consent of the parent remaining behind’ signed by the parent remaining behind. With
this declaration the parent gives permission for the child to travel to the Netherlands.

The biological child of your (registered) partner
Choose this option if the child is a biological child of your spouse, registered partner, or traditionally/religiously
related partner, but not from yourself. Add to your application the following evidence:
• A copy of the child’s birth certificate, issued by the authorities of the country of origin; and
• A completed and signed appendix ‘Application for Questionnaire family relationship child’.
The other parent of the child (of your legal spouse), registered partner or traditional/religious partner) must give
permission for the arrival of the child in the Netherlands. Therefore, please add the following evidence for the
application:
• The ‘Declaration of consent of the parent remaining behind’ signed by the parent remaining behind and;
• A copy of the identity document (for example, passport) including the signature of the parent remaining
behind;
• A copy of evidence, issued by the authorities of the country of origin, which shows the biological relationship
between the child and the parent remaining behind (for example, a birth certificate or an extract from the
register of births).
• If it is not possible to have the declaration of consent signed by the parent remaining behind, then complete

7 Place of collection of regular provisional residence permit (mvv) (if the regular provisional residence permit is granted)
At the Dutch embassy or consulate in the country of origin (or country of continuous residence). Continues residence is assumed, among others, when your family member resides in a country on the basis of a residence permit with a period of validity of 3 months or more. The continuous residence in that country must be substantiated with evidence. When there is no Dutch embassy or consulate, the mvv must be collected in the nearest country that has a Dutch representation. If your family member has to travel to the Dutch embassy because additional information is needed, the embassy has to be able to contact your family member to make further arrangements. This is why you must give a telephone number so that your family member can be contacted.

• A copy of the front and back of the residence permit asylum for a specific period.
Documents of the foreign national:
• A recent passport photo that reflects the current appearance of the foreign national. If the foreign national
does not have a recent passport photo, you can also submit a another recent and sharp photo in which the
face of the foreign national is clearly visible;
• A copy of the pages with identity details and period of validity of the passport. Also copy the pages with
travel stamps. Do not copy any empty pages; or
• A copy of the front and back of a different identity document issued by the authorities of the country of
origin. Enclose a Dutch translation of the front and back of the identity document.
If the foreign national does not have an (own) passport or other identity document, tick below what applies:
The foreign national does not have an own valid passport or other identity document. In that case you
enclose as much other evidence as possible, which shows the identity and nationality.
The foreign national is under the age of 18 and registered in the valid passport of his parent. You enclose
the copies of that passport with the application, including the pages with the identity details, period of
validity of the passport and the travel stamps. Do not copy empty pages.
For most Dutch embassies abroad an identity document with a passport photo is required to obtain access to the
embassy. The IND requests to enclose a copy of this identity document with this application.
9 Biometric information (fingerprints, passport photo) and signature,
Antecedents certificate and Declaration of civil status
• The foreign national must have his fingerprints and facial image (passport photo) taken at the Dutch
embassy or consulate to determine his identity. The biometric information is also required to create a
residence permit. For the residence permit the IND also needs a signature of the foreign national.
NB. No fingerprints are taken from children younger than 6 years of age, only a passport photo is required.
• Fill out the Antecedents certificate appendix and submit this appendix together with your application (only if
the foreign national is 12 years of age or older).
• Enclose the ‘Declaration of civil status’ appendix, completed and signed by the foreign national (only if the
foreign national is 15 years of age or older).

10 Declaration of consent to provide special personal data
The IND requests your consent to provide the municipality with special personal data. Family reunification
migrants need a citizen service number (BSN) before they can use public services in the Netherlands.
Your family members will have a citizen service number (BSN) once they have been registered in the Personal
records database (BRP). Before registering your family members, the municipality checks if their names are
written correctly and if a marriage, for instance, is legally valid. For this purpose, the municipality needs special
personal data, such as data regarding a person’s religion, race, nationality and ethnicity. This information is in
the asylum interview report and the (digital) registration form.
The IND is not allowed to provide the municipality with special personal data without the consent of you and
your family members. You give your consent by completing the declaration of consent. Your family members will
give their consent upon arrival in the Netherlands.
The municipality and the IND do not record special personal data in their systems. The municipality only uses
the special personal data to establish a person’s surname and to check the legal validity of a marriage or
acknowledgement of a child, for instance.

What happens to your application?
Once you have submitted your application to the IND, then the IND will assess your application. If your
application is incomplete, the IND will be unable to assess your application properly. You will be informed in
writing once your application has been handled. If your application is granted, you will also receive a letter
with information about the follow-up procedure.

Explanation
If you apply for a residence permit for a foreign national, you are the sponsor. This comes with responsibilities. As a
sponsor, you must ensure that the foreign national meets the conditions for a residence permit. Not only when applying
for a residence permit, but also afterwards. If the foreign national can no longer lawfully reside in the Netherlands,
you, as sponsor, are responsible for the foreign national leaving the Netherlands.
Obligation to provide information
You are obliged to report changes to the IND within 4 weeks if they could have an effect on the residence permit. Please
use the ‘Notification form for family members and relatives’, which you can download from www.ind.nl. The form lists the
changes you need to report.
Duty to keep records
You are obliged to collect and store any relevant information about the foreign national. You can find the data you are
obliged to store in the Aliens Regulations. You must store this documentary evidence up to 5 years after you are no longer
the sponsor of the foreign national. The IND can request data at any time in order to assess whether you have
complied with your obligations.

Appendix Application for DNA test in case of family
reunification
You applied for family reunification (whether or not within the context of a journey in connection with family reunification).
The person submitting such an application must demonstrate that a family relationship exists. This is usually done by
means of official (legalised) documents. If these documents are missing through no fault of your own, a DNA test could
offer a solution. Here, you can read how a DNA test works.
What is a DNA test? Each person has unique characteristics. These characteristics have been embedded in our DNA. This DNA can be found in
all our body cells. Everyone passes part of these unique characteristics on to his or her children. So father, mother and
children partly have the same DNA. A DNA test compares the DNA of your family members.
Possible results • A DNA test only provides certainty about the biological parent-child relationship. A DNA test has no use if it concerns
foster children or adopted children.
• A DNA test can establish (with certainty) that you are not the father or mother of a child.
• A DNA test can establish (with certainty) that you are the father or mother of a child.
• If the DNA test is performed on both parents, it could become clear that one of them is not the biological parent of the
child.
How does the test proceed? If you have applied for family reunification and it is impossible for you to obtain the necessary documents, the Immigration
and Naturalisation Service (IND) offers you a DNA test. In that case, you must complete this form in order to state that
you want to make use of a DNA test. The test means that a mouth swab is taken from you, your spouse and child(ren) and
examined. By comparing the DNA in these mouth swabs, it can be established whether you are the biological parent(s) of
the child or the children. Your family members must make an appointment to have their DNA taken at the Dutch embassy.
After they have done their DNA test, they must inform you immediately. Starting the next day you can make an
appointment online (for yourself and your biological family members in the Netherlands) for DNA collection at the IND desk
in Utrecht.
DNA test step by step The DNA test is not painful and not injurious to one’s health.
1. The identity is established.
2. A mouth swab is taken from the inside of your cheek. This is done by using a special swab.
3. All the DNA material is investigated by the Netherlands Forensic Institute (NFI).
4. The NFI sends the result of the DNA test to the IND.
What happens with the test results? The IND will involve the results of the DNA test in the decision on your application for family reunification. The DNA taken
and the pertaining data are destroyed after six months. These data will not be used for other purposes.
Countercheck With a negative result from the DNA test you can have a counter-inquiry carried out by a different laboratory. This is called
a countercheck. This countercheck must be carried out by a laboratory which has been recognised by the Dutch
Accreditation Council RVA for carrying out DNA relationship tests. You can check this through the search function on
the website of the Dutch Accreditation Council RVA (www.rva.nl/search). You must pay the costs of a countercheck
yourself.
