Introduction to Summary of BenefitsThank you for your interest in Anthem MediBlue Value (HMO). Our plan is offered by Anthem Health Plans, Inc.which is also called Anthem Blue Cross and Blue Shield, a Medicare Advantage Health Maintenance Organization(HMO) that contracts with the Federal government. This Summary of Benefits tells you some features of our plan. Itdoesn't list every service that we cover or list every limitation or exclusion. To get a complete list of our benefits, pleasecall Anthem MediBlue Value (HMO) and ask for the "Evidence of Coverage."

You have choices in your health careAs a Medicare beneficiary, you can choose from differentMedicare options. One option is the Original(Fee-for-Service) Medicare Plan. Another option is aMedicare health plan, like Anthem MediBlue Value(HMO). You may have other options too. You make thechoice. No matter what you decide, you are still in theMedicare Program.

You may join or leave a plan only at certain times. Pleasecall Anthem MediBlue Value (HMO) at the telephonenumber listed at the end of this introduction or1-800-MEDICARE (1-800-633-4227) for moreinformation. TTY/TDD users should call1-877-486-2048. You can call this number 24 hours aday, 7 days a week.

How can I compare my options?You can compare Anthem MediBlue Value (HMO) andthe Original Medicare Plan using this Summary ofBenefits. The charts in this booklet list some importanthealth benefits. For each benefit, you can see what ourplan covers and what the Original Medicare Plan covers.

Our members receive all of the benefits that the OriginalMedicare Plan offers. We also offer more benefits, whichmay change from year to year.

Where is Anthem MediBlue Value(HMO) available?The service area for this plan includes:

Fairfield, Litchfield, Middlesex, Tolland Counties, CT.

You must live in one of these areas to join the plan.

Who is eligible to join AnthemMediBlue Value (HMO)?You can join Anthem MediBlue Value (HMO) if youare entitled to Medicare Part A and enrolled in MedicarePart B and live in the service area. However, individualswith End-Stage Renal Disease generally are not eligibleto enroll in Anthem MediBlue Value (HMO) unless theyare members of our organization and have been sincetheir dialysis began.

Can I Choose My Doctors?Anthem MediBlue Value (HMO) has formed a networkof doctors, specialists, and hospitals. You can only usedoctors who are part of our network. The health providersin our network can change at any time.

You can ask for a current provider directory. For anupdated list, visit us athttp://www.anthem.com/medicare. Our customerservice number is listed at the end of this introduction.

What happens if I go to a doctor who'snot in your network?If you choose to go to a doctor outside of our network,you must pay for these services yourself. Neither the plannor the Original Medicare Plan will pay for these servicesexcept in limited situations (for example, emergency care).

Where can I get my prescriptions if Ijoin this plan?Anthem MediBlue Value (HMO) has formed a networkof pharmacies. You must use a network pharmacy toreceive plan benefits. We may not pay for yourprescriptions if you use an out-of-network pharmacy,

except in certain cases. The pharmacies in our networkcan change at any time. You can ask for a pharmacydirectory or visit us athttp://www.anthem.com/medicare. Our customerservice number is listed at the end of this introduction.

Anthem MediBlue Value (HMO) has a list of preferredpharmacies. At these pharmacies, you may get your drugsat a lower co-pay or co-insurance. You may go to anon-preferred pharmacy, but you may have to pay morefor your prescription drugs.

What if my doctor prescribes less thana month's supply?In consultation with your doctor or pharmacist, you mayreceive less than a month's supply of certain drugs. Also,if you live in a long-term care facility, you will receiveless than a month's supply of certain brand and genericdrugs. Dispensing fewer drugs at a time can help reducecost and waste in the Medicare Part D program, whenthis is medically appropriate.

The amount you pay in these circumstances will dependon whether you are responsible for paying coinsurance(a percentage of the cost of the drug) or a copay (a flatdollar amount for the drug). If you are responsible forcoinsurance for the drug, you will continue to pay theapplicable percentage of the drug cost. If you areresponsible for a copay for the drug, a "daily cost-sharingrate" will be applied. If your doctor decides to continuethe drug after a trial period, you should not pay more fora month's supply than you otherwise would have paid.Contact your plan if you have questions aboutcost-sharing when less than a one-month supply isdispensed.

Does my plan cover Medicare Part Bor Part D drugs?Anthem MediBlue Value (HMO) does cover bothMedicare Part B prescription drugs and Medicare PartD prescription drugs.

What is a prescription drug formulary?Anthem MediBlue Value (HMO) uses a formulary. Aformulary is a list of drugs covered by your plan to meetpatient needs. We may periodically add, remove, or make

changes to coverage limitations on certain drugs or changehow much you pay for a drug. If we make any formularychange that limits our members' ability to fill theirprescriptions, we will notify the affected members beforethe change is made. We will send a formulary to you andyou can see our complete formulary on our Web site athttp://www.anthem.com/medicare.

If you are currently taking a drug that is not on ourformulary or subject to additional requirements or limits,you may be able to get a temporary supply of the drug.You can contact us to request an exception or switch toan alternative drug listed on our formulary with yourphysician's help. Call us to see if you can get a temporarysupply of the drug or for more details about our drugtransition policy.

How can I get extra help with myprescription drug plan costs or getextra help with other Medicare costs?You may be able to get extra help to pay for yourprescription drug premiums and costs as well as get helpwith other Medicare costs. To see if you qualify forgetting extra help, call:

* Your State Medicaid Office.

What are my protections in this plan?All Medicare Advantage Plans agree to stay in theprogram for a full calendar year at a time. Plan benefitsand cost-sharing may change from calendar year tocalendar year. Each year, plans can decide whether tocontinue to participate with Medicare Advantage. A planmay continue in their entire service area (geographic areawhere the plan accepts members) or choose to continueonly in certain areas. Also, Medicare may decide to enda contract with a plan. Even if your Medicare Advantage

Plan leaves the program, you will not lose Medicarecoverage. If a plan decides not to continue for anadditional calendar year, it must send you a letter at least90 days before your coverage will end. The letter willexplain your options for Medicare coverage in your area.

As a member of Anthem MediBlue Value (HMO), youhave the right to request an organization determination,which includes the right to file an appeal if we denycoverage for an item or service, and the right to file agrievance. You have the right to request an organizationdetermination if you want us to provide or pay for anitem or service that you believe should be covered. If wedeny coverage for your requested item or service, youhave the right to appeal and ask us to review our decision.You may ask us for an expedited (fast) coveragedetermination or appeal if you believe that waiting for adecision could seriously put your life or health at risk, oraffect your ability to regain maximum function. If yourdoctor makes or supports the expedited request, we mustexpedite our decision. Finally, you have the right to filea grievance with us if you have any type of problem withus or one of our network providers that does not involvecoverage for an item or service. If your problem involvesquality of care, you also have the right to file a grievancewith the Quality Improvement Organization (QIO) foryour state. Please refer to the Evidence of Coverage(EOC) for the QIO contact information.

As a member of Anthem MediBlue Value (HMO), youhave the right to request a coverage determination, whichincludes the right to request an exception, the right tofile an appeal if we deny coverage for a prescription drug,and the right to file a grievance. You have the right torequest a coverage determination if you want us to covera Part D drug that you believe should be covered. Anexception is a type of coverage determination. You mayask us for an exception if you believe you need a drugthat is not on our list of covered drugs or believe youshould get a non-preferred drug at a lower out-of-pocketcost. You can also ask for an exception to cost utilizationrules, such as a limit on the quantity of a drug. If youthink you need an exception, you should contact usbefore you try to fill your prescription at a pharmacy.Your doctor must provide a statement to support yourexception request. If we deny coverage for yourprescription drug(s), you have the right to appeal and askus to review our decision. Finally, you have the right to

file a grievance if you have any type of problem with usor one of our network pharmacies that does not involvecoverage for a prescription drug. If your problem involvesquality of care, you also have the right to file a grievancewith the Quality Improvement Organization (QIO) foryour state. Please refer to the Evidence of Coverage(EOC) for the QIO contact information.

What is a Medication TherapyManagement (MTM) Program?A Medication Therapy Management (MTM) Programis a free service we offer. You may be invited to participatein a program designed for your specific health andpharmacy needs. You may decide not to participate butit is recommended that you take full advantage of thiscovered service if you are selected. Contact AnthemMediBlue Value (HMO) for more details.

What types of drugs may be coveredunder Medicare Part B?Some outpatient prescription drugs may be covered underMedicare Part B. These may include, but are not limitedto, the following types of drugs. Contact AnthemMediBlue Value (HMO) for more details.

Some Antigens: If they are prepared by a doctor andadministered by a properly instructed person (whocould be the patient) under doctor supervision.Osteoporosis Drugs: Injectable osteoporosis drugs forsome women.Erythropoietin: By injection if you have end-stagerenal disease (permanent kidney failure requiringeither dialysis or transplantation) and need this drugto treat anemia.Hemophilia Clotting Factors: Self-administeredclotting factors if you have hemophilia.Injectable Drugs: Most injectable drugs administeredincident to a physician's service.Immunosuppressive Drugs: Immunosuppressive drugtherapy for transplant patients if the transplant tookplace in a Medicare-certified facility and was paid forby Medicare or by a private insurance company thatwas the primary payer for Medicare Part A coverage.Some Oral Cancer Drugs: If the same drug is availablein injectable form.

Out-of-NetworkPlan drugs may be covered in specialcircumstances, for instance, illness whiletraveling outside of the plan's service areawhere there is no network pharmacy. You mayhave to pay more than your normalcost-sharing amount if you get your drugs atan out-of-network pharmacy. In addition, youwill likely have to pay the pharmacy's fullcharge for the drug and submitdocumentation to receive reimbursement fromAnthem MediBlue Value (HMO).
