The test of an organization is whether it has the ability to constantly manage change and the duration over which those changes will be realized. It’s a challenge that we at NorthPointe embrace because we recognize that there is a bigger picture, a longer time horizon, and a broader scope in which we operate. If our history and trajectory have taught us one thing, it’s that we have to think in terms of both today and tomorrow if we are going to be truly successful in serving people with disabilities.

Not-for-Profits do not operate in a vacuum—they operate under a mandate to serve society and fulfil their mission. For NorthPointe that requires that we deliver person-centered services, focused on community inclusion. From day-to-day operations to strategic decision-making, we are guided by these principles. Over the years, we have woven this focus into every aspect of our organization—and in doing so, strengthened the very fabric of NorthPointe.

Our ability to strive for innovation in what we do fuels our growth and allows us to stay ahead of the trends shaping our industry. It motivates our partners, positions our strategic plan and helps to deliver today and well into the future NorthPointe’s award-winning services. We’ll continue to be a leader in health initiatives and training for people with I/DD and expand our innovative services to meet changing needs.

We have achieved much so far. We recognize that there is a great amount of work to be done to meet the aggressive goals we’ve set for ourselves. NorthPointe has the capacity and the will to lead. NorthPointe’s talented staff is vital to our continued success and we recognize their contributions. In an environment of reduced funding, we will continue to do whatever we can to reflect how deeply they are appreciated.

We’re proud of each step we’re taking and the larger journey on which we’ve embarked. We remain steadfastly dedicated to creating opportunities for people with I/DD and mental illness. We will continue to work with our community partners toward common goals of community inclusion and making the world a better place for everyone. • We are one of very few agencies that have a dedicated Quality Assurance Department ensuring continued dedication to quality and innovation.

• We are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF International). Our accreditation reviews consistently result in the highest rating of a three-year accreditation for services.

• We are accredited in the Self-Employment program to help individuals develop their own micro-enterprise business.

• We continue to be long-standing members of IARF (Illinois Association of Rehabilitation Facilities) a highly-regarded nationwide professional resource for information and advocacy initiatives.

• We were one of the first agencies in the country to receive accreditation for the “Self Directed Services” program designed to support individuals choosing, planning and coordinating their own services.

We are committed quality services with superior standards

We believe in providing innovative opportunities through our partnerships• We partnered with The University of Illinois Rehabilitation Research and Training Center on Developmental Disability and Health (RRTCDD) to develop and launch a nation-wide health initiative (HealthMatters) designed to teach and assist agencies in developing wellness initiatives.

• We partner with Partners for Progress to provide the opportunity of horseback riding therapy classes.

• We partner with Rosalind Franklin University to provide psychology doctoral students an opportunity to gain experience working with persons with I/DD so they may better understand and serve the needs of a diverse population.

• We partner with University of Illinois Certified Master Gardeners to provide Horticulture classes so program participants can learn about gardening. Vegetables are grown, harvested and distributed to the CILA homes.

• Awarded the Employment First Grant (2014) modeled on a national movement that believes employment in the general workforce is the first and preferred option when exploring goals and a life path for people I/DD.

• Awarded the Caregiver Connection Grant in 2008 (and continue to be awarded the grant annually) a support service for parents and care-givers of children ages 0-5 experiencing social and emotional adjustment issues (serving Lake County).

We are a recognized leader by national and state agencies for delivering quality services

RESiDEnTiAL DiViSiOn NorthPointe Resources CILA program offers residents the optimum experience of living in the community in a variety of supported housing arrangements. Residents learn daily living and social skills and are encouraged to participate in a variety of community activities.

The residential staff is a professionally trained team involved with each resident to help each set measurable attainable goals and assist them as they strive to realize their personal aspirations and highest potential.

24x7 Residential Support Services offer living arrangements with on-site staff seven days a week and 24 hours a day to provide the level of care required by each resident. The homes are located throughout Lake County, IL and are an integrated part of the community.

Intermittent Living Support Program. Remember your first apartment and the excitement of being on your own? Many of our 24x7 CILA program participants have the same dream and choose to work toward living on their own or with a roommate with customized intermittent support.

DAY SERVicESLocated in the Killian Center, program participants thrive in an enviroment developed to build social, personal and work skills to ensure successful community participation. Programs include:

The LifeSkills Program offers six-week classes on a variety of subject matters; computer science, daily living skills, photography, health and exercise, etc. Program participants are also offered the option of taking Art Therapy classes and joining a variety of groups, i.e. Horse-back riding, theatre, and consumer council, etc.

Employment Resource Services (ER) participants are assisted in acquiring employment in the community and support in retaining their positions.

Caregiver Connections—awarded to NorthPointe by the State of Illinois for nine consecutive years—is a support service for caregivers and parents of children (up to five years of age) experiencing social and emotional adjustment issues.

The Killian Job Training Center partners with businesses in the community to provide paid on-the-job training opportunities for program participants in the Outsource and Warehouse Departments. Individuals learn various business skills in a supported work environment to help them transition to community employment.

Person centered ServicesnorthPointe offers a wide range of services and programs for persons with intellectual and developmental disabilities (i/DD) and mental illness. Emphasize in placed on individual growth, skills training, well-being with a strong focus on community involvement through employment, residency, and leisure activities.

cOMMUniTY BASED SERVicESNorthPointe Resources is a recognized leader in high quality, service facilitation supporting children and adult Home-Based Support Services (HBSS) recipients.

PSYcHO-SOciAL REHABiLiTATiOn S.O.A.R.R. (Support • Outreach • Achievement • Recovery • Resilience) is a Psycho-Social Rehabilitation (PSR) program that focuses on long-term recovery for persons diagnosed with chronic mental illness and/or co-occurring psychiatric disabilities. S.O.A.R.R. promotes recovery, resilience and relapse prevention. Program are personalized to help members successfully manage their symptoms of their mental illness.
