OURMISSION

The Saban Free Clinic, in collaboration with strategic partners, serves as a medical home for the underserved and those who are most vulnerable by providing comprehensive, dependable and affordable quality health care in a caring environment.

“Without The Saban Free Clinic, I would not be alive”

Juan Jimenez survived a heart attack, but as he listened to a doctor at a local hospital talk about the next steps in his recovery process, he worried about how he would be able to afford to prevent another one. Mr. Jimenez had worked as a finance inspector at the same company for eighteen years. When the worst of the recession hit in 2009, his company laid off a large number of employees, including Mr. Jimenez, and his health insurance soon expired. At age 60 he found that no one wanted to hire him because they knew he would probably retire in a few years, and he still had to wait five years to become eligible for Medicare.

With only his son’s small income as financial support for the both of them, Mr. Jimenez was concerned about the high cost of health care. Fortunately, the doctor at the hospital told him that he might be eligible to be a patient at The Saban Free Clinic, where he would be able to receive affordable health care.

A week and a half after his heart attack, Mr. Jimenez made his first visit to the Clinic’s Wallis Annenberg Children and Family Health Center for a check-up with Dr. Shahram Javdanfar. Dr. Javdanfar began the appointment by listening to Mr. Jimenez’s heart and immediately knew that something was very wrong. “The doctor ordered, ‘Take him to the hospital right away,’” reported Mr. Jimenez, “and they called the ambulance and rushed me to the hospital

because I was having another heart attack.” When he regained consciousness in the hospital, the cardiologist asked him: “Who sent you here?” and Mr. Jimenez replied, “Dr. Javdanfar from The Saban Free Clinic.” “Well,” the cardiologist replied, “he just saved your life.”

Now a regular patient of the Clinic, Mr. Jimenez says that he is extremely grateful for the lifesaving care he received: “I don’t know how to say thank you to the doctor. He saved my life. He acted so fast—he was so smart! If it weren’t for him I would have died.” Mr. Jimenez met with Dr. Javdanfar and expressed his gratitude: “Thanks to God and thanks to you—I am alive!” To which the doctor replied, “That’s my job.”

Mr. Jimenez often tells people that he would not be alive without the Clinic, and that he stays healthy through the continuing care he now receives at the Clinic. Mr. Jimenez said that the Clinic is now his medical home: “They know me; they recognize me and always welcome me. Everyone is so friendly.” He knows that he is one of a lucky few that have survived multiple heart attacks, and is appreciative that the Clinic exists so he can receive the care he needs. And of course, he feels especially thankful for one person: Dr. Javdanfar, who is now his regular physician. “When I see him, he’s like my brother, my savior. He saved my life.”

Fiscal Year 2011 ushered in tremendous change for The Saban Free Clinic and all health care clinics throughout the country. The national Affordable Care Act, designed to overhaul the nation’s health care system and guarantee access to medical insurance for tens of millions of Americans, presents new opportunities for improving the Clinic’s infrastructure and the way we provide services to patients.

During the year, the Clinic successfully applied for and was designated a Federally Qualified Health Center (FQHC) Look-Alike. This will ensure that the Clinic continues to thrive in the new era of health care reform by opening the door to additional funding opportunities and allowing the Clinic to implement a sliding-fee scale for patients who do not qualify for coverage by government health care programs, such as Healthy Way L.A., Medi-Cal or Healthy Families. With this designation the Clinic strengthens its commitment to providing quality health care to low-income, uninsured people in our community.

This new designation also required the Clinic to reconfigure its Board of Directors to include a specific percentage of Clinic patients, ensuring their involvement in Clinic oversight. Additionally, the Clinic created a Board of Trustees to provide consultation, expertise and assistance to the Board of Directors.

In other recent developments, the School-based Health Center at Hollywood High School has become an important focus for the Clinic. The Los Angeles Unified School District (LAUSD) informed us that Hollywood High School has been identified as one of fourteen “hot-spot” schools in the county. To ensure better health outcome for the students and the neighboring community, LAUSD is renovating an area on campus that will allow the Clinic to expand services on site to more students, as well as eventually offering the surrounding community greater access to health care services.

Looking ahead to fiscal year 2012, we are excited to embrace new opportunities and are prepared to meet any challenges. In advance of full implementation of health care reform in 2014, this coming year will be heavily focused on enrolling all of our eligible patients in the Healthy Way L.A. program, adopting electronic health records for improved data collection, and working to further integrate behavioral health services into primary medical care.

Our patients come first, and the services we provide will continue to reflect their needs. The Clinic will remain an advocate for the thousands who need us, as part of the health care safety net for the uninsured and the underinsured. HIGHLIGHTSWith the support of our community partners and generous donors, The Saban Free Clinic accomplished the following significant achievements that benefitted our patients in fiscal year 2010-2011:

y The Clinic provided a total of 88,351 patient visits to 21,211 unique clients, 99% of whom reported to be living at or below 200% of the Federal Poverty Level, which for a family of four is less than $44,700 annually.

y The Clinic continued its dedication to the Patient Centered Medical Home Model. This Model enables the Clinic to coordinate care around the needs of patients with greater efficiency, using a streamlined process that allows for immediate referral of all patients to other Clinic services.

y The Clinic developed an interdisciplinary planning team to transfer OB/GYN services to our Hollywood Wilshire Health Center, transforming this site into a comprehensive family practice clinic offering a full-range of health care services to children, adults and pregnant women.

y The Clinic established a qualified intermediary audit subcommittee to ensure consistent ongoing quality assessment for all Clinic sites.

y To prepare for implementation of health care reform, the Clinic applied for and received designation as a Federally Qualified Health Center (FQHC) Look-Alike, which will greatly contribute to the Clinic’s sustainability in the years ahead.

y Per FQHC Look-Alike requirements, the Clinic reconfigured its Board of Directors to include Clinic patients, which will ensure their involvement in Clinic oversight. Additionally, the Clinic created a Board of Trustees to provide consultation, expertise and assistance to the Board of Directors.

y The Clinic implemented Healthy Way L.A., Los Angeles County’s new low-income health program. Healthy Way L.A. expands health care coverage to low-income,

uninsured individuals and reimburses providers treating these individuals for each patient visit.

y The Clinic’s Chronic Disease Management Program continues to positively impact the community and is one of the first programs in Los Angeles County to have a mental health component. In the past year, this program served 3,055 patients, accounting for 14% of the Clinic’s total number of patients served.

y During the past year, approximately 400 volunteers dedicated over 10,600 service hours to the Clinic in areas ranging from case management to recordkeeping to special events.

y The Clinic was able to expand the quality and quantity of services offered to patients through support provided by the Weingart Foundation, Cedars Sinai Medical Center, The Eisner Foundation, the California Community Foundation and many other generous funders.

y The Clinic partnered with Reach Out and Read to film a public service announcement at The Wallis Annenberg Children and Family Health Center promoting early literacy, which featured actor Hill Harper of CSI: NY..

y The Clinic gained exposure through coverage in over 100 media outlets, including prominent publications such as The Los Angeles Times, The Hollywood Reporter and Variety.

y The Clinic produced four successful fundraising events, coordinated by our Special Events Department, that raised almost $2 million in support of patient services.

y The Clinic’s Chief Medical Officer, Dr. Jehni Robinson, was featured on KPCC’s Crawford Family Community Forum as part of a panel of “L.A.’s Top Docs.”

Patient Care: Striving to Go Above and Beyond

It is our patients at The Saban Free Clinic that motivate us to work hard each and every day to ensure that health care is a right and not a privilege. We are gratified to be providing the necessary services for them to lead healthier and happier lives. Last year our patients donated more than $94,000 to thank us for being their medical home. We were truly moved by their generosity and very appreciative for these funds, which will allow us to continue to provide the high-quality care that they depend upon.

We highly value our patients, and we are committed to addressing their needs to the fullest. In addition to providing primary health care, the Clinic has been able to offer services such as showers for homeless persons, prenatal gift baskets for expecting mothers, support groups for those with special needs, and we’ve even found a wonderful way to make our patients’ holidays brighter. This year, the Clinic hosted our 21st Annual Patient Holiday Party. This community event was created in 1989 to address the needs of high-risk youth in the Hollywood area by providing them warm clothes. Thirty-five people attended that first holiday event. This year the Clinic was able to provide more than 500 patients and their family members with a warm meal, clothing, toiletries, toys, blankets and other necessities. We are constantly looking for new opportunities to better serve our patients, and we thank our donors and volunteers for their continued support and assistance in helping us create a real medical home.
