The 2011 Annual Open Meeting is dedicated to the 10th Anniversary of CHART
CHART (Community Housing and Re-enablement Team) is hosted by Crosby Housing Association and based in our offices in Church Road, Waterloo. The team works with Sefton residents with severe and enduring mental illness who experience difficulties with their accommodation.
Four team members are employed by CHA and the Team Manager is employed and seconded from Mersey Care NHS Trust Positive Care Partnerships Clinical Business Unit, who jointly manage the project. The team is funded by NHS Sefton and Sefton MBC’s Social Care and Housing Departments. Since the introduction of CHART in 2001, the team has developed into a well-established and highly-regarded service. Outcomes from its work regularly bear testimony to how safe and secure accommodation can positively impact on a person’s mental health and well-being. The CHART team would like to say a sincere ‘Thank you’ to everyone involved in supporting the team over the past 10 years and pay particular thanks to Mandy, Doug and all the staff at Crosby Housing Association for their continued support, without whom none of the work would have been possible.
