Subject: Troubled Housing Recovery Team Onsite Review
Dear Mr. Smith:
This is to notify you that an Onsite Review of the Housing Authority of Anniston is being
conducted. The review is scheduled for Wednesday, July 20 through Friday, July 22, 2011. The
entrance meeting with you, key staff, and the Chairperson of the Board of Commissioners will
commence on Wednesday, July 20, 2011 at 2:00 p.m. The Exit Conference will be held on
Friday, July 22 at 2:00 p.m. The review team will consist of:
José Cintrón, Miami Public Housing Director and Team Lead
Stephanie Stone, Program Analyst, Washington, DC Office of Field Operations
Charlie Barnett, Nashville Public Housing Director and Team Member
Carol Spencer, Louisville Public Housing Division Director and Team Member
Sandra Chesnutt, Financial Analyst, Departmental Enforcement Center and Team
Member
As Executive Director, your participation is required at both the Entrance and Exit
Conferences. We request that the Board Chairperson be present for the Entrance and Exit
Conferences, and that the entire Board of Commissioners be present for the Exit Conference.
Notice of this Exit Conference should be publicized accordingly if state regulations require it.
The purpose of the Onsite Review is to formalize the Department of Housing and Urban
Development efforts in assessing the causes of the PHA’s troubled status and begin crafting a
plan to effectively improve Agency performance to enable its removal from the troubled list.
To facilitate this review, please have the following documents available for review and/or
photocopying on Wednesday, July 20. This will enable effective use of the Team’s onsite time
and minimize the disruption of the PHA’s day-to-day activities.
1. Most recent 12 months of bank statements
2. Most recent 12 months of balance sheets
3. Internal controls policies and procedures
4. Most recent 3 years of budgets
5. Financial interests in subsidiaries
6. Last 12 months of Board agendas and minutes
7. Recent executive contracts, including modernization, personnel, supplies, etc.
8. Most recent 12 months of payroll
9. Most recent 12 months listing of all benefits for all employees
10. Last 24 months of travel by SHA staff and Board members
11. Procurement Policy and Procedures
12. Maintenance Policy and Procedures
13. Personnel Policy
The Team may request additional information as necessary during and after the review.
