When to remove the tefillin on Mondays andThursdays On days that the Torah is read, it is customary not toremove the tefillin before the Torah is returned to the AronKodesh. These days include Mondays, Thursdays,Taniesim and Chanuka. In congregations where the Torahis returned only after the kadish following Uva Letzion, thetefillin should not be removed until the Torah is returnedafter kadish. There is an allusion in a pasuk to the idea ofwearing tefillin when returning the Torah to the AronKodesh, as is written: "Their k ing [represented by theTorah] went before them and Hashem [represented by thetefillin] was at their head."(סעיף יג וס"ק נז)

Why the tefillin are removed before Musaf onRosh ChodeshOn Rosh Chodesh the tefillin are removed before Musaf. One reason for this is because thereare communities that recite the kedusha of Keser on Rosh Chodesh and it is not appropriateto talk about Hashem's crown when we are wearing our own crown [the tefillin]. Removing tefillin hurriedly on RoshChodeshThe tefillin are removed between kadish and the MusafShemoneh Esrei. It is customary to remove the tefillinhurriedly in order to minimize the interruption betweenkadish/Uva Letzion and Shemoneh Esrei. Some posk imwrite (in reference to the custom of returning the Torahimmediately after it is read) that there should not be abreak between kadish and Musaf, and the tefillin shouldbe removed during Uva Letzion after the words "yehiratzon...shenishmor chukeicha". It is appropriate toremove the tefillin after these words since in earlier timesa beracha ­lishmor chukav ­ was recited when removingtefillin. For this reason, many people start removing thetefillin (unwrapping the straps from the fingers) afterreciting these words on regular days.(ס"ק נט; ביאורים ומוספים דרשו, 74­76)

If the shel yad loosened before theshel rosh was put on, it may betightened without a new beracha. Ifthe shel yad or shel rosh loosened ata later point, they should be tightenedwith a new beracha.

According to the Mechaber and Gra, ifsomeone removed his tefillin to adjustthe knot, a new beracha is required.According to the Rama, a newberacha is not necessary.
