ADDING AN EMPLOYEE
•Will appear in drop menu on the Green Form
PROCESSING A HAZARDOUS WASTE
Conventionally done with a “Green Form” Required pre-printed forms with Code #’s Must be keyed in by hand
Can now be entered digitally Prints as a conventional “Green Form” Generates container labels Standardizes data
GENERAL IDEA
We are focused on each time a material or Item is used. Referred to as a History
Each Item is stored with its properties and attributes, (Safety precautions, Waste Class etc.)
Materials never used before will be added to a growing list of Items
A single Item can be paired with many Histories, (not the other way around)
Items occurring more than 150 times/ year are designated as Frequently Used Items (FUI)

•Making an entry on this form will add a new item to the database.•Once picked up, the new item is logged into receiving and converted to inventory
•“New” does not mean it has never been used before. It refers to a new collection.

Many departments have Repeat Waste Steams HazStat saves these records in a drop menu You can populate the Item’s Profile by selecting
the FUI from that menu Don’t have a frequently used item?
Use the previous Green Form to simply process a new item occurrence
Yes, there are 2 separate Green Forms: One for adding New Entries or non FUI materials One for adding New FUI usage.
Note, each usage of a material or item should be entered into HazStat!
PROCESS A FREQUENTLY USED ITEM (FUI)
STEP 1: SELECT A FREQUENTLY USED ITEM
•Click the “Add a New Item” link if your item is not there, next slide…
•Helpful Hint: Jot down the number next to the Frequently Used Item
ADD AN ITEM TO THE FREQUENTLY USED MENU
Enter the item properties, this becomes the Item Profile!
If a frequently used item, check the last box to indicate it.
You must re-open the green entry form to refresh the drop menu!
STEP 2: ENTERING THE HISTORY
Enter that item’s new history by completing the remaining blank fields
1. Unique Code Number2. Amount (Refers to the container size, not what’s in
it)3. Units4. Research or Clean-up if applicable5. Safety Precautions if not already entered
STEP 3: ENTER ACCUMULATION DATES
Click the Calendar Icon to select a date, or enter your own manually
FAQ: When can I enter the same date for both Initial and Final Accumulation dates?Answer: If there was a spill/ cleanup or if the item was found in inventory and is being removed the same day.
Keeping good records is essential for Safety… Label, Label, Label!
STEP 4: SELECT EMPLOYEE PROFILE
Problem: Is your name not listed above?Solution: Go to the Users Menu and complete your employee profile. This only needs to be done once. Note: If you just added your User Profile,
close the green form and re-open to refresh the drop menu, then your name will appear!
You will lose your previous data so it is best to always make the employee profile first!
STEP 5: SCHEDULE AN EH&S PICK-UP
Select the date of your pick-up and a contact person who has custody of the item(s)Note: This may or may not be the same individual who is completing the form.
Important:•Be sure to include the item’s location in the comments section. •If you are including any other comments, make item location the first entry!
STEP 6: ADD THE ITEM DETAILS
Room/ Course stores:• Room Locations, • Course Numbers • Experiment Numbers
REPORTS
ENTER YOUR CODE RANGE
Step 1: Start Code(First Code in Sequence)
Step 2: End Code(Last Code in Sequence)
Note: The overall Code Range is 90 entries long.This tells the software to generate a report for those 90 entries.
The Code Range will pertain to your assigned block of numbers
REPORTS: READY TO PRINT
Note: It is best to perform a “Print Preview” to verify that the page layout is correct!
You can regenerate your reports to see if they have been picked up or received to inventory
NOW PRINT YOUR CONTAINER LABELS
PRINT 2” X 4” AVERY LABELS
Again, specify the range, same as before.
This generates the same labels as green forms that you already printed
ALTERNATE REPORT METHOD
Instead of filtering by a block of assigned Code Numbers, look up by: Department 3-digit code: e.g. “art” And a range of Initial Accumulation Dates
Note: We assume that any waste being accumulated is labeled as: “Hazardous Waste” Hazards of the material (Waste Class etc.) Initial Accumulation Date

STEP 1: GO TO REPORTS MENU
Click on the filter by Dept and Initial Accumulation Dates button.
STEP 2: ENTER THE DEPT CODE
Note: This must be the official department 3-digit classification.
Check the Departments List under Directory Menu to see a list of official department codes.
STEP 3: ENTER STARTING DATE
Note: This date refers to the Initial Accumulation Dates, not the Final
STEP 4: ENTER FINAL DATE
Note: This date refers to the Initial Accumulation Dates, not the Final
STEP 5: REVIEW AND PRINT
