U.S. vessel

ACP vessels included

Immediately after addressing safety concerns, notify the nearest Coast Guard Sector.

Written Notification Must be filed within five days

CG-2692 Report of Marine Injury Accident or Death

1. Grounding/Allision

• Intended• Unintended

2. Loss of: Main propulsion Primary steering Maneuverability

3. Adverse affects to service or route: Fire Flooding Failure /damage to:

o fire systemso lifesavingo power generating equipmento bilge

4. Loss of life

5. Injury:• Professional medical

treatment • Unfit to perform

duties.

7. Damage over $25,000

MSO Hampton Roads

Scheduling Inspections COI 5 year renewal

– Written Application w/in 30 days (CG-3752)

– Local CG Sector– Schedule w/in 90

days– ACP vessels =

Complete ABS prior to CG

MSO Hampton Roads Annual & Periodic Exam

– 3 months before/after anniversary

– 1 or 2 weeks notice– No written notice– ACP vessels = Complete

ABS before CG Dry-dock

– Time frame same as COI Renewal

– Package review

MSO Hampton Roads Scope of Inspection

– Conventional• Full Tests• Documents• ABS & CG coordination

allowed– ACP

• Spot Check• Documents• ABS & CG coordination not

allowed (oversight) • Expand as needed
