 At  our  discretion,  items  will  be  included  in  the  Live  and  Silent  Auctions,  and  may  be  packaged  with  other  items.  

We  must  have  this  form  AND  the  donation  in  hand  by  the  deadline  for  inclusion.    If  we  do  not  have  the  item  in  hand,  it  will  not  be  included  in  our  auction.
