REGISTRATION PACKETS:

Registration Steps:

• Complete all the information in the registration packet found below.

(Please print one-sided only)

• Gather all required documents- (student’s birth certificate and passport if born

outside of the United States, proof of address (two required)

• Immunization and health records are only required for new students to M-DCPS -

(students entering 7th grade need the Tdap Vaccine)

Once you have gathered the documents and completed the forms required for

registration, email all of your documents to Ms. Susan Suarez, school registrar,

at ssuarez@dadeschools.net. She will contact you if anything is missing.

Should you want to submit your paperwork in person, please email Ms.

Michelle Coto, principal at MLCoto@dadeschools.net. She will schedule a time

for you to bring the documents to the school.
