INTENDED USE Remel PYR Disk w/ Reagent is a rapid, colorimetric test recommended for use in qualitative procedures for the detection of pyrrolidonyl arylamidase activity for presumptive identification of enterococci, group A streptococci, and Escherichia coli. SUMMARY AND EXPLANATION
In 1981, Godsey et al. described a test to differentiate group A streptococci and enterococci from other streptococci based on the ability to cleave L-pyrrolidonyl-β-naphthylamide (PYR).

al. incorporated PYR substrate into agar and tested for enzyme activity after overnight incubation. Bosely et al. incorporated PYR substrate into a broth and tested for enzyme hydrolysis after 4 hours incubation.

In 1985, Ellner et al. described a colorimetric method using filter paper strips containing the PYR substrate and found it easy to perform and cost-effective for presumptive identification of group A streptococci and enterococci.

the PYR test can be used for differentiating E. coli from other enteric gram-negative bacilli.

PRINCIPLE Filter paper disks are impregnated with the substrate, L-pyrrolidonyl-β-naphthylamide. In the presence of the enzyme, pyrrolidonyl arylamidase (PYRase), the substrate is hydrolyzed resulting in the formation of β-naphthylamine. This compound produces a red color upon the addition of ρ-dimethylaminocinnamaldehyde (Color Developer). The test is routinely performed in less than four minutes. REAGENTS AND MATERIALS SUPPLIED 1. STORAGE This product is ready for use and no further preparation is necessary. Store product in the dark in its original container at 2-8°C until used. Do not freeze or overheat. Allow product to equilibrate to room temperature before use. Do not incubate prior to use.
PRODUCT DETERIORATION This product should not be used if (1) the color of the disks has changed from white, (2) the expiration date has passed, or (3) there are other signs of deterioration. Protect disks from moisture by removing from the vial only those disks necessary for testing. Promptly replace the cap and return the vial to 2-8°C.
Note: Precipitate formation or color variation from straw to pink in the Color Developer is common and does not affect test performance.

SPECIMEN COLLECTION, STORAGE, AND TRANSPORT Specimens should be collected and handled following recommended guidelines.

MATERIALS REQUIRED BUT NOT SUPPLIED (1) Loop sterilization device, (2) Inoculating loop, swabs, collection containers, (3) Incubators, alternative environmental systems, (4) Supplemental media, (5) Quality control organisms, (6) Forceps, micropipette, calibrated loop, (7) Demineralized water.
Optional Materials: (1) Microscope slide, (2) Micropipette, 5-10 μl, (3) Applicator sticks.
PROCEDURE Test isolates should be 18-24 hours old and growing on nonselective culture media. Ensure that the culture is pure or that selected colonies are well-isolated from surrounding organisms.
Streptococci: Verify that the test isolate is catalase-negative, gram-positive cocci with microscopic and colonial morphology consistent with enterococci or group A streptococci.
Enteric gram-negative bacilli: When screening for E. coli, test only lactose-fermenting, gram-negative bacilli which are indole-positive, oxidase-negative, and nonhemolytic.

1. Using forceps, place a disk on a clean microscope slide or in the lid of an agar plate, free from excess moisture.
2. Moisten the disk slightly with 5-10 μl of demineralized water using an adjustable micropipette or a 10 μl inoculating loop. Do not over-saturate the disk. Alternatively, the disk may be placed on the surface of the agar medium for rehydration.
3. With an applicator stick or a sterile inoculating loop, remove a heavy visible paste of the test isolate and rub it into the disk.
4. Incubate at room temperature (RT) for 2 minutes. 5. Add one (1) drop of Color Developer to the disk. 6. Allow up to one minute for a pink to red color to develop. INTERPRETATION Positive Test - Pink to red color development within one minute of
adding Color Developer
Negative Test - Cream, yellow, or no color change within one minute of adding Color Developer
Notes:
If the PYR disk has been pre-moistened on a blood agar plate, the background of the disk may be pale to straw yellow. This will not alter the appearance of a positive reaction.
Weak results may occur if insufficient cells are tested or if the test isolate is poor in the expression of PYRase activity. Confirm a weak result by repeating the procedure with a larger inoculum and incubating the disk for 5 minutes, rather than 2 minutes, prior to the addition of the Color Developer.

LIMITATIONS 1. PYR Disk w/ Reagent provides a high probability for presumptive
separation of enterococci and S. pyogenes (group A) from other streptococci; however, it is only part of the overall scheme for identification. Further biochemical and/or serological testing may be required for definitive identification. Consult appropriate references for further instructions.

2. Aerococcus viridans, Lactococcus garvieae, certain staphylococci, and most Corynebacterium haemolyticum strains are PYR-positive, as are some Enterobacteriaceae and other gram-negative bacilli. Consult appropriate references for differential tests when necessary.

In a study by York et al. the PYR Disk w/ Reagent was used to validate the accuracy of rapid spot tests for identification of E. coli.

Participating laboratories obtained 1,064 clinically significant strains of Enterobacteriaceae with core criteria of indole-positive, oxidase-negative, nonswarming colonies. Organisms were identified using commercial kit systems and an algorithm which included lactose fermentation, beta-hemolysis, PYR, and 4-methyl-umbelliferyl-β-D-glucuronide (MUG) results. The study demonstrated that nonhemolytic lactose-fermenters which are indole-positive, oxidase-negative, and PYR-negative, can be reliably identified as E. coli with greater than 99% accuracy, significantly reducing reagent costs, technologist time, and time to reporting.

ZUSAMMENFASSENDE ERKLÄRUNG
1981 beschrieben Godsey et al. einen Test, um Gruppe-A-Streptokokken und Enterokokken von anderen Streptokokken zu unterscheiden. TESTPRINZIP Filterpapierblättchen werden mit dem Substrat, L-pyrrolidonyl-β-naphthylamid, imprägniert. Bei Vorhandensein des Enzyms Pyrrolidonylarylamidase (PYRase) wird das Substrat zu β-Naphthylamin hydrolisiert. Diese Verbindung erzeugt bei Zugabe von ρ-Dimethylaminocinnamaldehyd (Farbentwickler) eine rote Färbung. Routinemäßig wird dieser Test in weniger als vier Minuten durchgeführt.

PROBENENTNAHME, LAGERUNG UND TRANSPORT Die Probenentnahme und -handhabung sollte unter Berücksichtigung der folgenden empfohlenen Richtlinien erfolgen.8,9
BENÖTIGTE MATERIALIEN (NICHT IM LIEFERUMFANG ENTHALTEN) (1) Gerät zur Sterilisation der Inokulationsschlinge, (2) Inokulations-schlinge, Abstrichbesteck, Probenbehälter, (3) Inkubatoren, andere Klimasysteme, (4) zusätzliche Medien, (5) Qualitätskontrollorganismen, (6) Pinzette, Mikroliterpipette, kalibrierte Öse, (7) entmineralisiertes Wasser. Optionales Material: (1) Objektträger für Mikroskop, (2) Mikroliterpipette 5-10 μl, (3) Applikatorstäbchen.
VERFAHREN Test-Isolate sollten 18-24 Stunden alt sein und auf nicht-selektiven Kulturmedien gewachsen sein. Stellen Sie sicher, dass die Kultur als Reinkultur vorliegt oder wählen Sie Kolonien aus, die gut isoliert von umgebenden Organismen wachsen.

1. Blättchen mit der Pinzette auf einen sauberen Objektträger oder den leeren Deckel einer Agarplatte legen. Darauf achten, dass keine überschüssige Feuchtigkeit vorliegt.
2. Das Blättchen leicht mit 5-10 µl entmineralisiertem Wasser benetzen. Benutzen Sie dazu eine einstellbare Mikroliterpipette oder eine 10 µl Inokulieröse. Das Blättchen nicht zu stark benetzen. Alternativ dazu kann die Blättchen zur Rehydrierung auf einem Agarmedium platziert werden
3. Mit einem Abstrichstäbchen oder einer sterilen Inokulieröse einen großen, deutlich sichtbaren Teil der Masse des Test-Isolats entnehmen und ihn auf dem Blättchen abstreifen.
4. 2 Minuten bei Raumtemperatur (RT) inkubieren. 5. Einen (1) Tropfen Farbentwickler auf das Blättchen geben. 6. Eine Minute abwarten, bis die rosa bis rote Färbung erscheint.
INTERPRETATION Positiver Nachweis: Rosa bis rote Färbung innerhalb einer Minute

Hinweise:
Wenn das PYR-Blättchen auf einer Blutagarplatte angefeuchtet wurde, kann sie blass- bis strohgelb erscheinen. Das Erscheinen einer positiven Reaktion wird dadurch nicht beeinflusst.
Schwache Ergebnisse können auftreten, wenn nicht genügend Zellen getestet wurden oder wenn das Test-Isolat eine schwache Expression der PYRase-Aktivität aufweist. Ein schwaches Ergebnis durch die Wiederholung des Vorgangs mit einem größeren Inokulum und der Inkubation der Blättchen für 5 anstelle von 2 Minuten vor Zugabe des Farbentwicklers bestätigen.

Die im Rahmen der Qualitätssicherung durchgeführten Tests mit Kontrollorganismen sollten die Anforderungen anerkannter Qualitätssicherungsverfahren für Labore erfüllen. Treten im Rahmen der Qualitätskontrolle abweichende Ergebnisse auf, dürfen die Patientenergebnisse nicht verwendet werden.

EINSCHRÄNKUNGEN 1. PYR-Blättchen mit Reagenz bieten eine hohe Wahrscheinlichkeit für
die mutmaßliche Separierung von Enterokokken und S. pyogenes (Gruppe A) von anderen Streptokokken, jedoch sind sie nur ein Teil des Gesamtschemas zur Identifikation. Zur endgültigen Identifikation bedarf es weiterer biochemischer und/oder serologischer Tests. Weitere Informationen bitte den entsprechenden Referenzen entnehmen.7-9
2. Aerococcus viridans, Lactococcus garvieae, gewisse Staphylokokken und die meisten Corynebacterium haemolyticum-Stämme sind PYR-positiv, wie auch einige Enterobacteriaceae und andere gram-negative Bakterien. Weitere Informationen können Sie der entsprechenden Dokumentation für Differenzialtests entnehmen.7-9
3. Einige Enterokokken-Stämme sind beta-hämolytisch, deswegen muss die Morphologie der Kolonien sorgfältig kontrolliert werden, um zwischen Enterokokken und S. pyogenes zu unterscheiden.7-9
4. Falsch-negative Reaktionen können auftreten, wenn ein ungeeignetes Inokulum verwendet wird oder wenn das Inokulum von einem selektiven Medium zurückgezogen wurde (beispielsweise Columbia CNA, PEA, MacConkey-Agar usw.).
LEISTUNGSMERKMALE
PYR-Blättchen mit Reagenz wurden in drei verschiedenen Studien mit 284 Stämmen von Streptococcus und Enterococcus spp getestet.11 Die Ergebnisse werden unten gezeigt.

In einer Studie von York et al. wurden PYR-Blättchen mit Reagenz dazu verwendet, um die Genauigkeit von Punktschnelltests für die Identifikation von E. coli zu validieren.6 Die teilnehmenden Labore erhielten 1064 klinisch

signifikante Stämme von Enterobacteriaceae mit den Kernkriterien indol-positive, oxidase-negative, nicht schwärmende Kolonien. Die Organismen wurden mit kommerziellen Kitsystemen und einem Algorithmus, der Lactosefermentation, Beta-Hämolyse, PYR und 4-Methyl-umbelliferyl-β-D-glucuronide (MUG)-Ergebnisse enthielt, identifiziert. Die Studie zeigte, dass nicht-hämolytische Lactosefermentierer, die indol-positiv, oxidasenegativ und PYR-negativ sind, verlässlich mit einer Genauigkeit von über 99% als E. coli identifiziert werden können, damit werden Reagenzkosten, Technikerzeit und die Zeit bis zum Bericht signifikant reduziert.

PYR DISK w/ REAGENT
INDICATION Le disque PYR avec réactif de Remel est un test colorimétrique rapide, dont l’utilisation est recommandée dans les procédures qualitatives de détection de l’activité de la pyrrolidonyl arylamidase à des fins d’identification présomptive d’entérocoques, de streptocoques du groupe A et d’Escherichia coli.
RÉSUMÉ ET EXPLICATION
En 1981, Godsey et al. ont présenté un test permettant de différencier les streptocoques du groupe A et les entérocoques d’autres streptocoques selon leur capacité à séparer la L-pyrrolidonyl-β-naphthylamide (PYR).1 En 1982, des chercheurs ont utilisé le test PYR conjointement au CAMP et aux tests de bile esculine pour identifier les streptocoques de manière présumée.2 Facklam et al. ont intégré le substrat PYR dans de la gélose et ont testé l’activité enzymatique après une nuit d’incubation. Bosely et al. ont intégré un substrat PYR dans un bouillon et ont testé l’hydrolyse enzymatique après 4 heures d’incubation.3 En 1985, Ellner et al. ont présenté une méthode colorimétrique faisant appel à des bandes de papier filtrant contenant le substrat PYR et l’ont trouvé simple à réaliser et rentable pour l’identification présomptive des streptocoques du groupe A et des entérocoques.4,5 Plus récemment, York et al. ont indiqué que le test PYR pouvait servir à différencier l’E. coli d’autres bacilles entériques à gram négatif.6 Cette méthode est recommandée par le CLSI (Clinical and Laboratory Standards Institute) à des fins d’identification rapide et abrégée de l’E. coli.7
PRINCIPE Les disques de papier filtrant sont imprégnés du substrat, le L-pyrrolidonyl-β-naphthylamide. En présence de l’enzyme, la pyrrolidonyl arylamidase (PYRase), le substrat est hydrolysé, d’où la formation de β-naphthylamine. Ce composé produit une coloration rouge après ajout de ρ-diméthylaminocinnamaldéhyde (développeur de coloration). Le test est pratiqué généralement en moins de quatre minutes.

CONSERVATION Ce produit est prêt à l’emploi ; aucune préparation supplémentaire n’est nécessaire. Stocker le produit à l’abri de la lumière, dans son flacon d’origine et le conserver à une température comprise entre 2 et 8°C jusqu’à son utilisation. Ne pas congeler ni surchauffer. Attendre que le produit atteigne la température ambiante avant de l’utiliser. Ne pas incuber avant utilisation. DÉTÉRIORATION DU PRODUIT Ce produit ne doit pas être utilisé si (1) la couleur des disques n’est plus blanche, (2) la date de péremption est dépassée ou (3) d’autres signes de détérioration sont présents. Protéger les disques de l’humidité en sortant du flacon uniquement ceux qui sont nécessaires au test. Reboucher immédiatement le flacon avant de le remettre en place à une température comprise entre 2 et 8°C.
Remarque : La formation d’un précipité ou un virement de la coloration jaune paille au rose dans le développeur de coloration est fréquent et n’a aucune incidence sur les performances du test. PRÉLÈVEMENT, CONSERVATION ET TRANSPORT DES ÉCHANTILLONS Les échantillons doivent être prélevés et manipulés conformément aux recommandations en vigueur.8,9
MATÉRIEL REQUIS MAIS NON FOURNI (1) Dispositif de stérilisation en boucle, (2) boucle à inoculation, écouvillons, récipients de prélèvement, (3) incubateurs, systèmes environnementaux alternatifs, (4) milieux supplémentaires, (5) organismes de contrôle de qualité, (6) pince, micropipette, boucle étalonnée, (7) eau déminéralisée. Matériel en option : (1) lame de microscope, (2) micropipette, 5-10 µl, (3) bâtonnets applicateurs. PROCÉDURE Les isolats de test doivent avoir entre 18 et 24 heures et croître sur un milieu de culture non sélectif. Vérifier que la culture est pure ou que les colonies sélectionnées sont bien isolées des organismes environnants.
Streptocoques : Vérifier que l’isolat de test est un cocci à catalase négative et à gram positif ayant une morphologie microscopique et une colonie cohérente par rapport aux entérocoques ou aux streptocoques du groupe A.
Bacille entérique à gram négatif : Lors du dépistage de l’E. coli, tester uniquement les bacilles à fermentation de lactose et gram négatif positifs à l’indole, négatifs à l’oxydase et non hémolytiques.7
1. À l’aide de pinces, placer un disque sur une lame de microscope propre ou dans le couvercle d’une membrane de gélose exempt d’humidité excessive.
2. Humidifier le disque légèrement avec 5-10 µl d’eau déminéralisée et une micropipette réglable ou une boucle d’inoculation de 10 µl. Ne pas saturer le disque excessivement. Il est également possible de placer le disque sur la surface du milieu de gélose à des fins de réhydratation.
3. À l’aide d’un bâtonnet applicateur ou d’une boucle d’inoculation stérile, retirer une pâte lourde et visible de l’isolat de test et la déposer dans le disque.
4. Incuber à température ambiante (RT) pendant 2 minutes.
5. Ajouter une (1) goutte de développeur de coloration sur le disque.
6. Patienter jusqu’à une minute avant qu’une coloration rose à rouge se développe.

Remarques:
Si le disque PYR a été pré-humidifié sur une plaque de gélose de sang, l’arrière-plan du disque peut être jaune pâle à jaune paille. Cela ne modifie pas l’apparence d’une réaction positive.
Les résultats risquent d’être faibles si le nombre de cellules testées est insuffisant ou si l’isolat de test est faible lors de l’expression de l’activité de la PYRase. Vérifier un résultat faible en recommençant la procédure à l’aide d’un inoculum plus grand et en incubant le
disque pendant 5 minutes, plutôt que 2 minutes, avant d’ajouter le développeur de coloration.

CONTRÔLE DE QUALITÉ Tous les numéros de lots de disques PYR avec réactif ont été testés avec les organismes de contrôle de qualité suivants et reconnus acceptables. Les tests des organismes de contrôle doivent respecter les critères établis pour les procédures de contrôle qualité en laboratoire. En cas de résultats de contrôle de qualité aberrants, ne pas rapporter les résultats des patients.

LIMITES 1. Le disque PYR avec réactif offre une forte probabilité de séparation
présomptive d’entérocoques et S. pyogenes (groupe A) d’autres streptocoques. Cependant, il fait seulement partie du modèle global d’identification. D’autres analyses biochimiques et/ou sérologiques peuvent être nécessaires pour obtenir une identification définitive. Consulter les références correspondantes pour obtenir de plus amples instructions.7-9
2. Aerococcus viridans, Lactococcus garvieae, certains staphylocoques et la plupart des souches de Corynebacterium haemolyticum sont positifs à la PYR, de même que certains Enterobacteriaceae et d’autres bacilles à gram négatif. Consulter les références correspondant aux tests différentiels, si nécessaire.7,9
3. Certaines souches d’entérocoques sont bêta-hémolytiques. Par conséquent, la morphologie de la colonie doit être évaluée soigneusement pour différencier les entérocoques des S. pyogenes.7-9
4. Les réactions faussement négatives peuvent se produire si un inoculum inadapté est utilisé ou s’il est retiré du milieu sélectif (par ex. Columbia CNA, PEA, géloses de MacConkey, etc.).

Dans une étude menée par York et al. le disque PYR avec réactif a servi à valider la précision de tests ponctuels rapides destinés à l’identification d’E. coli.6 Les laboratoires participant ont obtenu 1 064 souches

cliniquement significatives d’Enterobacteriaceae, les critères déterminants étant qu’elles soient positives à l’indole, négatives à l’oxydase et qu’il s’agisse de colonies sans essaimage. Les organismes ont été identifiés à l’aide de systèmes de kits disponibles dans le commerce et d’un algorithme qui comportait les résultats de la fermentation du lactose, de la bêta-hémolyse, de la PYR et du 4-méthyl-umbelliferyl-β-D-glucuronide (MUG). L’étude a démontré que les fermenteurs de lactose non hémolytiques positifs à l’indole, négatifs à l’oxydase et négatifs à la PYR, pouvaient être identifiés de manière fiable comme E. coli, avec une précision supérieure à 99%, d’où une réduction significative des coûts de réactifs, du temps consacré par les techniciens et du temps de rédaction de rapports. Nel 1981, Godsey et al. descrissero un test per differenziare gli streptococchi del gruppo A e gli enterococchi da altri streptococchi basati sulla capacità di scindere L-pirrolidonil-β-naftilamide (PYR).

PRINCIPIO I dischi di carta da filtro sono pregni di substrato L-pirrolidonil-β-naftilamide. In presenza dell’enzima, pirrolidonil-arilamidasi (PYRase), il substrato viene idrolizzato risultando nella formazione di β-naftilamide. Questo composto produce un colore rosso dopo l’aggiunta di ρ-dimetil ammino cinnamaldeide (sviluppatore cromatico). Il test viene eseguito normalmente in meno di quattro minuti. PROCEDIMENTO Gli isolati del test dovrebbero essere di 18-24 ore e coltivati su un terreno di coltura non selettivo. Accertarsi che la coltura sia pura o che le colonie selezionate siano ben isolate dagli organismi circostanti.
Streptococchi: verificare che l’isolato del test sia catalisi negativo, cocci gram positivo con morfologia coloniale e microscopica coerente con enterococchi o streptococchi del gruppo A.
Bacilli enterici gram-negativo: quando si controlla l’E. coli, testare solamente bacilli gram negativo, che fermentano lattosio e che sono indolo positivi, ossidasi negativi e non emolitici.

1. Usando le pinze, porre un disco su un vetrino di microscopio pulito o nel coperchio di una piastra agar eliminando l’umidità in eccesso.
2. Inumidire il disco leggermente con 5-10 μl di acqua demineralizzata usando una micropipetta regolabile o un’ansa per inoculo da 10 μl. Non riempire troppo il disco. In alternativa, il disco può essere posto sulla superficie dei terreni agarizzati per la reidratazione.
3. Con un bastoncino applicatore o ansa per inoculo sterile, rimuovere una porzione visibile dell’isolato del test e spalmarla sul disco.
4. Incubare a temperatura ambiente (RT) per 2 minuti.
5. Aggiungere una (1) goccia di sviluppatore cromatico al disco.
6. Attendere per un minuto affinché si sviluppi il colore da rosa a rosso.

Se il disco PYR è stato inumidito in precedenza su una piastra di agar, il fondo del disco può essere pallido fino a giallo paglia. Questo non cambierà l’aspetto di una reazione positiva.
Possono verificarsi risultati insufficienti se vengono testate poche cellule o se l’isolato del test è esiguo nell’espressione dell’attività PYRasi. Confermare uno scarso risultato ripetendo la procedura con un inoculo e incubando il disco per 5 minuti, anziché 2, prima dell’aggiunta dello sviluppatore cromatico.

6 I laboratori che hanno partecipato hanno
ottenuto 1.064 ceppi clinicamente importanti di Enterobacteriaceae con i criteri principali di colonie di indolo positivo, ossidasi negativa, nonswarming. Gli organismi sono stati identificati usando sistemi di kit in commercio e un algoritmo che ha incluso risultati sulla fermentazione di lattosio, beta emolisi, PYR e 4-metilumbelliferil-β-D-glucuronide,(MUG). Lo studio ha dimostrato che i fermentatori di lattosio non emolitico, che sono indolo positivi, ossidasi negativi e PYR negativi, possono essere tranquillamente identificati come E. coli con precisione superiore al 99%, riducendo in modo considerevole i costi di reagente, tempo del tecnico e tempo di refertazione. USO PREVISTO El Disco PYR con reactivo Remel es una prueba colorimétrica rápida que se recomienda en procedimientos cualitativos para detectar actividad de la pirrolidonil-arilamidasa, con vistas a la identificación de enterococos, estreptococos del grupo A y Escherichia coli.
RESUMEN Y EXPLICACIÓN
En 1981, Godsey et al. describieron una prueba para diferenciar los estreptococos del Grupo A y los enterococos de otros estreptococos basándose en su capacidad de escindir L-pirrolidonil-β-naftilamida (PYR)1. En 1982, los investigadores utilizaron la prueba PYR junto con las pruebas CAMP y de la bilis-esculina para identificar estreptococos2. Facklam et al. incorporaron sustrato PYR al agar y analizaron la actividad enzimática después de una noche de incubación. Bosely et al. añadieron sustrato PYR al caldo de cultivo y analizaron la hidrólisis enzimática después de 4 horas de incubación3. En 1985, Ellner et al. describieron un método colorimétrico con tiras de papel de filtro que contenían sustrato PYR y descubrieron que podían identificar estreptococos de grupo A y enterococos de un modo sencillo y rentable4,5. Más recientemente, York et al. informaron de que la prueba PYR podía utilizarse para la diferenciación de E. Coli con respecto a otros bacilos gramnegativos6. Este método es recomendado por el Instituto de Normas para Laboratorios Clínicos (CLSI, por sus siglas en inglés) para la identificación rápida de E. Coli. 7
PRINCIPIO DEL ENSAYO Los discos de papel de filtro se impregnan con el sustrato, L-pirrolidonil-β-naftilamida. En presencia de la enzima pirrolidonil arilamidasa (PYRasa), el sustrato se hidroliza formando β-naftilamina. El compuesto produce un color rojo al añadir ρ-dimetilaminocinamaldehído (Revelador de color). La prueba se realiza normalmente en menos de cuatro minutos.

DETERIORO DEL PRODUCTO Este producto no se debe usar si (1) el color de los discos ha cambiado y ya no es blanco, (2) se ha sobrepasado la fecha de caducidad o (3) hay otros signos de deterioro. Se deben proteger los discos de la humedad, extrayendo del vial sólo los que sean necesarios para la prueba. Hay que volver a colocar rápidamente el tapón y volver a conservar el vial a una temperatura de 2-8°C.
Nota: La formación de un precipitado o el cambio de color de pajizo a rosa en el Revelador de color, son normales y no afectan al rendimiento de la prueba.
OBTENCIÓN, ALMACENAMIENTO Y TRANSPORTE DE MUESTRAS Las muestras se deben usar y manipular de acuerdo con las siguientes recomendaciones8,9.
MATERIALES NECESARIOS PERO NO SUMINISTRADOS (1) Asa para esterilización, (2) asa de inoculación, torundas y envases para recogidas, (3) incubadoras, sistemas ambientales alternativos, (4) medio complementario, (5) microorganismos para el control de calidad, (6) pinzas, micropipeta, asa calibrada y (7) agua desmineralizada.
Materiales opcionales: (1) portaobjetos del microscopio, (2) micropipeta de 5-10 μl, (3) aplicadores.
PROCEDIMIENTO Los aislamientos en estudio deben tener 18-24 horas y crecer en un medio de cultivo no selectivo. Debe comprobarse que el cultivo es puro o que las colonias seleccionadas están bien aisladas de los microorganismos circundantes.
Estreptococos: compruebe que el aislamiento en estudio sean cocos catalasa-negativos, grampositivos y que presente la morfología colonial y microscópica propia de los enterococos o los estreptococos del grupo A.
Bacilos entéricos gramnegativos: cuando busque E. Coli, pruebe únicamente los bacilos fermentadores de lactosa y gramnegativos, los cuales son indol-positivos, oxidasa-negativos y no hemolíticos7.
1. Utilizando unas pinzas, coloque un disco en un portaobjetos para microscopio limpio o en la tapa de una placa de agar que no presente exceso de humedad.
2. Humedezca el disco levemente con 5-10 μl de agua desmineralizada con una micropipeta ajustable o un asa de inoculación de 10 μl. No sature el disco en exceso. Otra opción es colocar el disco sobre la superficie del medio de agar para su rehidratación.
3. Con un aplicador o un asa de inoculación estéril, extraiga una pasta visible y gruesa del aislamiento en estudio y aplíquela en el disco.
4. Incube a temperatura ambiente (RT) durante 2 minutos.
5. Añada al disco una (1) gota de Revelador de color.
6. Espere hasta un minuto para que se desarrolle un color entre rosa y rojo.
INTERPRETACIÓN Prueba positiva: Desarrollo de un color entre rosa y rojo en un
minuto a partir de la adición del Revelador de color
Prueba negativa: Sin cambio de color o color crema o amarillo en un minuto a partir de la adición del Revelador de color
Notas:
Si el disco PYR ha sido prehumedecido en una placa de agar sangre, el fondo del disco puede ser de un color entre pálido y amarillo pajizo. Esto no modificará el aspecto de una reacción positiva.
Pueden producirse resultados débiles si no se analiza una cantidad suficiente de células o si el aislamiento en estudio presenta escasa actividad en la expresión de la PYRasa. Debe confirmarse un resultado débil repitiendo el procedimiento con un inóculo más grande e incubando el disco durante 5 minutos, en lugar de 2 minutos, antes de añadir el Revelador de color.

CONTROL DE CALIDAD Todos los números de lote de los Discos PYR con reactivo se han estudiado usando los siguientes microorganismos de control de calidad, y los resultados son aceptables. El estudio de los microorganismos de control se debe realizar de acuerdo con los procedimientos de control de calidad establecidos en el laboratorio. Si se observan resultados anómalos en el control de calidad, no deben comunicarse los resultados del paciente.

LIMITACIONES 1. El Disco PYR con reactivo proporciona una alta probabilidad de
separación de enterococos y S. pyogenes (grupo A) de otros estreptococos; sin embargo, sólo es una parte del sistema global para la identificación. Pueden necesitarse otras pruebas bioquímicas y/o serológicas para la identificación definitiva. Consulte las referencias correspondientes para acceder a más instrucciones7-9.
2. Los Aerococcus viridans, Lactococcus garvieae, algunos estafilococos y la mayor parte de las cepas de Corynebacterium haemolyticum son PYR-positivos, del mismo modo que algunas enterobacterias y otros bacilos gramnegativos. Consulte las referencias adecuadas de las pruebas diferenciales cuando resulte necesario7-9.
3. Algunas cepas de enterococos son beta-hemolíticas, motivo por el cual debe evaluarse cuidadosamente la morfología colonial para la diferenciación entre enterococos y S. pyogenes7-9.
4. Si se utiliza un inóculo inadecuado o se extrae de los medios selectivos (p. ej., CNA Columbia, PEA, agares MacConkey, etc.), pueden producirse falsos negativos.

El Disco PYR con reactivo ha sido evaluado en tres estudios separados utilizando 284 cepas de Streptococcus y Enterococcus spp11. Los resultados se muestran a continuación.

En un estudio de York et al., se utilizó el Disco PYR con reactivo para verificar la precisión de las pruebas rápidas para la identificación de E. Coli6. Se suministraron a los laboratorios participantes 1.064 cepas clínicamente relevantes de enterobacterias con colonias indol-positivas, oxidasa negativas y sin crecimiento. Los organismos se identificaron utilizando sistemas disponibles en el mercado y un algoritmo que incluía resultados de fermentación de la lactosa, beta-hemólisis, PYR y 4-metil-umbeliferil-β-D-glucurónido (MUG). El estudio demostró que los fermentadores de la lactosa no hemolíticos que sean indol-positivos, oxidasa-negativos y PYR-negativos, pueden identificarse como E. Coli con una precisión superior al 99%, reduciendo significativamente los costes de reactivos, el tiempo del técnico y el tiempo para el informe.
