To create a new provider account, providers should click on “Create Provider Account” on the Login Page:

Submit Document:

Please Note: When adding attachments to a submission, please ensure that the file name does not include any special characters. Valid characters are alphanumeric, space, underscore, hyphen, and period ONLY.

Address Update:

The “Address Update” tab should be completed. If the new address is for 1099 purposes, a W-9 form should also be attached.

Submit Dental Information:

The “Dental Submissions” tab should be completed:

Then, click the attachments tab:

Then browse for the file and click "Add":

Once added, a message will appear that the document has been uploaded successfully:

The user should then return to the "Dental Submissions" tab and click the "Submit" button.

Submit Medical Records:

The “Medical Records Submit” tab should be completed:

The user should then return to the "Medical Records Submit" tab and click the "Submit" button.
