Mix specimen well.
Let refrigerated specimens warm to room temperature before testing.
Dip the strip completely, but briefly, into specimen.
Remove excess urine by withdrawing the strip against the rim of the
container and by blotting the edge of the strip.
Compare reaction colors with the manufacturer’s chart under a good light
source at the specified time.
Be alert for the presence of interfering substances.
Understand the principles and significance of the test, read package inserts.
Relate chemical findings to each other and to the physical and microscopic
urinalysis results.

Nitrite: Clinical significance
Rapid screening test for the presence of UTI (Pyelonephritis, cystitis)
detecting initial cystitis, because patients are often asymptomatic or have vague symptoms that would not lead the physician to request a urine culture
Evaluation of antibiotic therapy
Monitoring of patients at high risk for UTI (diabetes, pregnant women)
Screening of urine culture specimens
combination with the leukocyte esterase
Nitrite : Reagent strip
Principle : Nitrite is detected by the Greiss reaction
o Gram negative bact. ปล่อย nitrate reductase
o nitrite (acidic pH) reacts with an aromatic amine to form a diazonium compound
o then reacts with tetrahydrobenzoquinolin compounds to produce a pinkcolored azodye
Confirm positive : Culture & Microscopic exam.
Diazotization reaction
Azo-coupling reaction

o High specific gravity, pH < 6 -> reduce sensitivity
Leukocyte esterase
Detects the presence of esterase in the azurophilic granules of granulocytic WBCs and monocytes
Neutrophils are the leukocytes most frequently associated with bacterial infections,
LE จะมีประโยชน์มาก เมื่อ pH, Nitrite +, WBCs and Bacteria in urine sediments
LE also are present in Trichomonas, Chlamydia and histiocytes
Clinical significance
o Bacterial and nonbacterial urinary tract infection (Trichomonas, Chlamydia, yeast)
o Inflammation of the urinary tract
o Screening of urine culture specimens (ดูร่วมกับ nitrite)
Principle : Ester hydrolysis + azo coupling reaction
requires the longest time of all the reagent strip reactions (2 minutes)
