If you have requested a 2018 Non-filer Letter from the IRS but they are unable to provide it to you, you may sign this affidavit and attach it to the Non-Filer Statement form (Parent or Student/Spouse). Or, if you do not have a Social Security Number or Taxpayer Identification Number, you may also sign this affidavit and attach it to the Non-Filer Statement form (Parent or Student/Spouse).
