Kindergarten Orientation

Registration

• March 3, 4, 5, 6, 7

• 9:30-11:30 & 12:45-2:15

• To pick up a packet you will need proof of residency (DL, utility bill –with name on it)

• Packets need to be returned on or before March 7

What is needed to register your child?

• Original birth certificate

• Immunizations – all up to date

• Physical Exam - after 7/1/13, due to me by 6/20/14

• Proof of residency – 4 documents

• Emergency contact information

Growth & Development Health History

• Fill out front and back and sign.

• Include any health information relevant to your child.

• Any medical information you provide me is strictly confidential unless other staff members need to know (food allergies, seizures, asthma)

Medication Policy• Physician’s Note• Parent’s permission• Original container• Children are not allowed to deliver

medications to school.• You will be notified at the end of the year

for medication pick up.• Any questions, please call me.

Communicable Diseases

• A Doctor’s note is required when your child has been absent with such diseases as Strep Throat, Scarlet Fever, Conjunctivitis, etc. If you have any questions, please keep in touch with me.

• Absences are medically excused with a signed doctor’s note for attendance purposes (not a written parent note).

Illnesses

• Common cold – OK for school, exceptions• Fever – child should be fever free for 24 hours,

fevers over 100 in school will be sent home• Diarrhea – do not send to school, if occurs at

school you will be called to take your child home• Vomiting – any vomiting will be sent home• Conjunctivitis – clears quickly when treated• Lice – excluded from class, home until nit free

Food Allergies• We take this matter very seriously.

• If your child has life threatening food

allergies, see me for additional paperwork.

• Your child may be in a classroom with a

student with severe food allergies. You will

be notified at the beginning of the year

regarding food restrictions in the classroom.

Nutrition

• Childhood obesity is rising.

• Send healthy snacks: fruit, vegetables, yogurt, crackers, 100% fruit juices or milk.

• Avoid candy, sugary cookies, chips.

• For parties try sending a healthy alternative – fruit kabobs or a non-edible surprise.

Orthopedic Injuries

• If your child requires an ace bandage, splint, or arm sling come in to the Health Office with a Doctor’s note.

• A crutch order is necessary from your Doctor.

• A Doctor’s note is needed to be excused from Physical Education Class.

Screenings

• Height and weights - BMI calculations for 1st and 4th grade

• Blood pressure

• Vision

• Hearing

• Scoliosis (Grade 5 or over 10 years old)

Special Activities• Jump Rope for Heart

• Holiday Food and Clothing Drives

• Field Day

• Bayview Helps its Own

• Wellness Activities – walking club, animal fitness challenge, health ambassadors, nutrition/fitness assemblies, garden events
