I) NURSING PRACTICUM I – Introduction to Nursing Practice
Aim
The aim of this practicum is to provide opportunities for students to integrate basic nursing
knowledge and skill into clinical practice in non-acute care settings.
Learning outcomes On completion of this component of the practicum, students will be able to:
1. be familiar with the environment (e.g. emergency exit, location of facilities, general ward
routine);
2. understand at a beginning level investigations, diagnostic and therapeutic procedures,
instruments used, and ethical and legal issues that are likely to arise in non-acute care
settings;
3. demonstrate safe practice and competence in performing basic nursing care (e.g. routine
and vital observation (TPR), lifting of patient, bed making, bed bathing and feeding);
4. demonstrate safe practice in performing complex nursing care (e.g. insertion of naso-
gastric tubes, urinary bladder catheterization, drug administration, and oxygen
administration, oro-pharyngeal suctioning);
5. demonstrate understanding of the bio-psycho-social implications of physical illness on the
client, family and community;
6. contribute knowledgably and skilfully to the care of clients and their family in the non-
acute hospital settings;
7. assess the needs of clients for health education in regard to self-care and, assist in
planning and implementation of a teaching programme that demonstrates appropriate
teaching skills;
8. evaluate the outcomes and effectiveness of care and suggest alternatives for
improvements; and
9. identify the Community Health Care services that are available to promote and maintain
the health of clients upon their discharge from non-acute hospital.

Infection control guidelines for MBBS, Chinese Medicine,
Nursing, and Pharmacy undergraduate students
SUMMARY
1 Standard precautions must be observed at all times for all patients regardless of
their infection status.
2 Perform hand hygiene either by hand-rubbing with alcohol-based hand rub or
hand washing with soap and water in accordance with the WHO Five Moments
of Hand Hygiene. This includes:
(a) before touching the patient;
(b) after touching the patient;
(c) before clean or aseptic tasks;
(d) after risk of body fluid exposure; and
(e) after contact with inanimate surfaces and objects (including medical
equipment) in the immediate vicinity of the patient.
3 One must NOT touch his/her mucous membrane (mouth, nose and eyes)
without cleaning his/her hands with alcohol-based hand rub or soap and water.
4 Wear surgical masks in clinical or patient areas in accordance to the current
infection control guidelines of those areas. Dispose of the masks and perform
hand hygiene before you leave the clinical or patient areas.
5 Use other personal protective equipment (such as gloves, respirators, protective
gowns, and eye shields) according to the instructions or guidelines in specific
settings, such as the type of ward visiting, nature of clinical procedures being
performed and the infective risks of the patients.
6 Report all accidents sustained in health care facilities immediately.
7 Do not come to class if you are suffering from a communicable disease or having
symptoms of a viral illness. Seek medical advice if necessary.
8 Make sure that you have received all the necessary vaccines including the
seasonal influenza vaccine.
9 Consult your supervisor if there is any uncertainty.
PRINCIPLES OF STANDARD PRECAUTIONS IN INFECTION CONTROL
1 Standard precautions must be observed at all times for all patients regardless of
their infection status.
2 Wear gloves whenever you come into contact with mucus membranes, blood and
body fluids including secretions and excretions. Cover wounds on your hands
with appropriate dressings.
3 Wearing gloves is not a substitute for hand hygiene. Perform hand hygiene after
removal of sterile or non-sterile gloves.
4 Remove gloves after caring for a patient. Do not wear the same pair of gloves for
the care of more than one patient.
5 When wearing gloves, change or remove gloves during patient care if you are
moving from a contaminated body site to another body site within the same
patient or the environment.
6 Use of personal protective equipment (PPE) depends on the overall infection
precaution level required at that time, and the specific setting of contact. There
may be additional requirements from specific units or institutions, so please
consult your teacher, supervisor, or staff in charge of the specific unit if
necessary.
7 Needles must never be recapped. Sharp objects must be disposed into sharps box
properly.
8 Do not come to class if you are suffering from a communicable disease. Seek
medical attention as soon as possible.
GOVERNMENT RESPONSE LEVELS IN HOSPITAL AUTHORITY HOSPITALS
1 The Hospital Authority (HA) is currently using a three-level alert response
system depending on the potential impact of an infectious disease outbreak
to the hospital system and the community. These include the Alert (level one),
Serious 1 and 2 (level two), and Emergency 1 and 2 (level three) responses.
2 Clinical attachment and teachings may be restricted or suspended when the
serious 2 level is in effect or under extraordinary circumstances. Please refer to
the announcements made by the Faculty or individual departments under such
situations.
INFECTION CONTROL GUIDELINES IN HOSPITAL SETTINGS
General wards and other patient areas
1 Clean white coat or uniform should be worn in patient areas.
2 Hand hygiene either by hand rubbing with alcohol-based hand rub or hand
washing with soap and water must be performed in the following circumstances:
(a) before touching the patient,
(b) after touching the patient,
(c) before clean or aseptic tasks,
(d) after risk of body fluid exposure, and
(e) after contact with inanimate surfaces and objects (including medical
equipment) in the immediate vicinity of the patient.
3 Alcohol-based hand rubs should be used for hand disinfection. They should be
used only if the hands are not visibly soiled.
4 Wear surgical masks in clinical or patient areas in accordance to the current
infection control guidelines of those areas. Dispose the masks and perform hand
hygiene before you leave the clinical or patient areas.
5 Additional PPE such as gloves, masks, or respirators may be used when coming
into contact with patient’s secretions, excreta, wounds, or during aerosol-
generating procedures and resuscitation. (See ‘High-risk areas’ below.)
High-risk areas
1 These include the Accident and Emergency Department triage stations, isolation
wards, isolation rooms, and cohort areas.
2 Surgical masks must be worn before entering these areas.
3 Protective gowns (disposable or reusable) must be worn before entering these
areas. Gowns must be put on and taken off in dedicated areas of the wards.
4 Additional PPE is used under special circumstances:
a N95 respirators must be worn when:
i Aerosol-generating procedures are being performed. Examples include
resuscitation, bronchoscopy, sputum suction, and high-flow oxygen
therapy;
ii You need to have close contacts with patients infected with diseases that
are transmissible via the airborne route.
b Gloves should be worn for contacts with patients with suspected or
confirmed infectious conditions of the skin, or when contacting the mucous
membranes, open wounds, blood, or body fluids. Wearing gloves is not a
substitute for hand hygiene. Perform hand hygiene after removing the gloves.
c Face shields or goggles should be worn during aerosol-generating
procedures.
5 Perform hand hygiene in accordance to the WHO Five Moments of Hand

6 Different wards may have different requirements and protocols. You should
consult the teacher, supervisor, or the staff in charge of the wards if you are not
certain.
Private practitioner clinics and other outpatient clinics, private community centres, and
nursing homes
1 The principles of infection control and the use of PPE are similar to those listed
above.
2 Standard precautions must be observed at all times if there is patient contact.
3 Masks and other PPE should be used when there is a substantial risk for
communicable diseases in the community setting. Consult your supervisor or the
staff in charge centre if you are not certain.
Autopsy rooms
1 Surgical masks, gloves, disposable protective gowns, shoe covers, and clip cap
will be provided prior to the autopsy sessions. These should be worn during the
sessions.
2 Goggles should be worn by students who do not wear glasses.
3 Consult the pathologists in charge of the session if there is any doubt.
PERSONAL HEALTH AND PRECAUTIONS
Vaccination
1 All students should receive the hepatitis B and varicella vaccines and have the
post-vaccination hepatitis B surface antibody level checked unless there are
specific contraindications. The vaccination schedule will be distributed by the
Faculty Office.
2 All students should take the seasonal influenza vaccination unless there are
specific contraindications.
3 Students should ensure that they have completed the childhood immunization
schedule and received the required booster doses (e.g. tetanus, diphtheria) unless
there are specific contraindications.
Precautions
1 Students should wear a surgical mask if they are suffering from respiratory
illnesses.
2 If the student is suffering from an infection and the condition is clinically
deteriorating or not responding to standard treatments, he or she should not
attend classes or enter the health care facilities and should seek medical advice as
soon as possible.
3 Any accident (e.g. percutaneous injuries or mucosal exposures) sustained in the
health care facilities must be reported to the teacher or supervisor, and the
infection control unit of that facility immediately.
Arrangements for distribution of PPE to students
1 Masks and disposable protective gowns will be provided by the Faculty for
clinical teaching sessions.
2 PPE can be obtained from the following locations.
a Medical students: Faculty Office, 2/F, William MW Mong Block, Faculty of
Medicine Building.
b Nursing students: School of Nursing, 4/F, William MW Mong Block, Faculty
of Medicine Building.
c Chinese Medicine students: School of Chinese Medicine, G/F, Estates
Building.
d Pharmacy students: Department of Pharmacology & Pharmacy, 2/F,
Laboratory Block, Faculty of Medicine Building.
e A small supply of pocket-sized alcohol-based hand rub, surgical masks, and
gowns will be made available in clinical areas in case individual students
forget to bring their own. The exact sites of placement will be arranged with
class/groups representatives. However, students are reminded that they
should obtain and use their individual supply obtained from the Faculty
Office as much as possible.

INTRODUCTION TO NURSING PRACTICE (CLASS 2015)
NOTES TO SUPERVISORS AND STUDENT
On the first day of practicum
Kowloon Hospital:
Students are to meet their supervisors outside the Central Nursing Division, General Office,
Room 2-042, 2/F, Main Building, at 8:45am.
1. All nursing students should report duty at 9:00am to CND office at 2-042, 2/F, Main Building,
Kowloon Hospital.
2. CND clerk will bring the students to the conference room and assist in the arrangement of lockers
and changing facilities with the hospital administrator.
3. Briefing session is to be delivered by the representative from the RM&E Department
(at Rm 4-056).
An orientation programme will be arranged on the first day of duty separately in each lot of
clinical practicum at 9:00am. Students MUST bring their own digital padlock for locker storage.
Tung Wah Hospital:
Students are to meet their supervisors at the Main Entrance of Li Shiu Chung Memorial Building
at 8:45 am. An orientation programme will be arranged on the first day of duty separately in each
lot of clinical practicum at 9:00am in the Lecture Room, 3/Floor, Block L, Li Shiu Chung
Memorial Building, Tung Wah Hospital,.
The content of the orientation will include Introduction to Tung Wah Hospital Authority; Work Safety
and Healthy in hospital and locker arrangement. The cafeteria is located at 1/F of Hawking's Wing
where students could have lunch there. Students MUST bring their own digital lock for locker storage.
Wong Chuk Hang Hospital:
Students meet their supervisors at the lobby of the Main Building at 8:45am. An orientation
programme will be arranged on the first day of duty separately in each lot of clinical practicum in
the A520, 5/Fl., Main Building, WCHH at 09:00am.. Changing rooms and locker facilities will be
arranged afterwards. For your information, there is only a tuck shop available in the Hospital but
students can go to the canteen in Grantham Hospital nearby. Students MUST NOT bring their
lunchboxes as there is no refrigerator provided and they MUST bring their own digital padlock for
locker storage.
Ruttonjee and Tang Shiu Kin Hospitals:
Students meet with their supervisor at the Lobby of the Main Block (next to Admission
Office) at 8:45am. Then, they will meet Ms Chan, APN at CND. Students will have their
Orientation program on the first day of duty separately in each lot of clinical practicum before
commencing duties at the designated wards. An orientation progamme will be held in Rm 815,
NSD, 8/Fl., Admin. Building, RHTSK Hospital at 09:00am. Students MUST bring their
own digital padlock for their locker storage. Cafeteria is available on site.
Fung Yiu King Hospital:
Students meet with their supervisors in 5/F, Conference Room at 8:45 am. Students (1st batch and
2nd
batch) will have their Orientation program on the first day of duty together before practicum
before commencing duties at the designated wards. An orientation programme will be held in
Conference Room, 5/Fl., FYKH at 09:00am. Locker keys are provided to the students after
Orientation Programme and please return the key back at the end of each lot of clinical practicum by
themselves.
MacLehose Medical Rehabilitation Centre:
Students meet their supervisor at 3/F, Nursing Department, MMRC at 8:45am on the first
day of duty. Briefing and orientation will be provided afterwards. Two lots of students will
have their Orientation programme on the first day of duty separately in each lot of
clinical practicum in 3/Fl., Nursing Department, MMRC at 09:00am. Locker keys are shared
among students and will be provided to the students after Orientation Programme and please return
the key back at the end of each lot of clinical practicum by themselves.
Tung Wah Eastern Hospital:
Students are to meet with their supervisors at 1/F, Lobby, Main Block, at 8:45am on 21 May 2012
& 1st day duty. ALL two lots of students are compulsory required to attend the Orientation
Programme together on 16 May 2011 (Monday) at 9am to be conducted by Tung Wah Eastern
Hospital in Multi-function Room, 9/F Ophthalmic Centre, TWEH. Students ARE NOT required
to bring digital locks. Keys will be provided and students are advised to return the keys to the hostel
manageress on leaving TWEH. Details of the Orientation Programme are as follows:

On the last day of practicum
Students are required to return ALL keys to related parties on the last day of practicum. There
should be NO excuse for any delay in returning. A group leader in each group may facilitate the
arrangement.
Absenteeism from Practicum
Student who is absent from practicum due to medical or any other reasons is required to inform both
the clinical supervisor and the ward in-charge directly and immediately. A sick leave certificate or
explanatory letter together with a completed “Record of Absence from Nursing Practicum” form
should be submitted to respective supervisor and copies faxed to the Year I Clinical Coordinator, Dr
Athena Hong as soon as the student resumes duty. Absence for more than two days will need to
seek the Dean’s approval via a written document
For special leave, students should inform the responsible clinical teacher and submit a written
application to the Clinical Coordinator one week in advance with valid reasons.

Proposed guidelines:
1. FIRST AID:
(a) First aid is of utmost importance for lowering the risk of infection.
(b) In case of needlestick injury, wash the wound immediately and thoroughly
with soap and water. The wound should be disinfected and dressed.
(c) In case of mucosal contact, such as spillage into the eyes, wash immediately
and liberally with running water.
2. COMPLETE USUAL PROCEDURE FOR ACCIDENT REPORTING
3. REFER TO Accident & Emergency Department of Hospital Authority Hospitals
IMMEDIATELY (There is a special team in Accident & Emergency Department of
all Hospital Authority Hospitals for handling needle-stick injuries. If required, post-
exposure zidovudine prophylaxis reduces the HIV transmission rate by about 80%
and should be started within 1-2 hours after exposure).

PRACTICUM ARRANGEMENTS DURING BAD WEATHER
When Tropical Cyclone Warning Signal No.3 or the Red Rainstorm Warning Signal is in
force, it should be assumed that all practicum will proceed as scheduled.
When Tropical Cyclone Warning Signal No.8 (or a higher number) or the Black Rainstorm
Warning Signal is hoisted, the following practicum arrangements will apply:
For practicum that has not been started
If either of the warnings is hoisted
or in force at or after 04:00 am
All AM practicum will be cancelled.
If either of the warnings is hoisted
or in force at or after 11:00 am.
All PM practicum will be cancelled.
If either of the warnings is hoisted
or in force at or after 06:00 pm
All NIGHT practicum will be cancelled.
For practicum that has already started:
When Tropical Storm Warning
Signal 8 or above is to be hoisted in
less than 2 hours (as announced by
Hong Kong Observatory)
All practicum will be suspended
immediately.
When Black Rainstorm Warning
Signal is hoisted
All practicum, except those held outdoors,
shall continue.
For practicum held outdoors, such as
community nursing, the responsible
mentors on the spot shall suspend the
activities immediately, ensuring all
students are housed in a safe place, and
remain there until it is safe for them to
return home.
No resumption of practicum is needed once it is cancelled.
Remarks:
In situations where the practicum has been cancelled as above but students have
already reached the hospital premises, they should stay in a sheltered environment
until it is safe for them to return home
