Role of Home and Hospital

Ensures compliance with Code of Maryland Regulations (COMAR) to provide instruction to public school students who are unable to participate in their school of enrollment due to a verified physical or emotional condition.

Physical / Emotional Conditions

•  Broken limb •  Cancer •  Hospitalization •  Surgery •  Pregnancy/Postpartum •  Sickle cell anemia •  Cystic fibrosis •  Substance Abuse •  Cancer

•  Juvenile diabetes •  Crohn’s disease •  Temporary emotional

condition •  Psychiatric

hospitalization

Physician’s Form Recommendations for School Attendance

q Unable to attend school q Able to attend school partial day q Able to attend school intermittently

as health permits.

Consent to contact physician

•  The Initial Contact and Referral (DSS-21) provides permission for PGCPS Health Services to contact the student’s physician.

Documentation of Physical Condition

The Physician’s Verification form (DSS-22/22A) must be completed by a licensed physician. psychiatrist or psychologist.

*COMAR will soon be updated to allow the form to be completed by a certified nurse practitioner.

Role of School Nurse

•  Consult with PGCPS Nurse Manager •  Contact student’s family •  Retain copy of physician’s form for

student’s health folder

Role of the School Nurse (cont)

ü Explain to staff the impact of health condition on student’s school participation (i.e. low energy level, medication effects, etc.)

ü Recommend health-related accommodations

Documentation of Emotional Condition

Certification of Need & Treatment Plan for Student with Emotional Condition (DSS 22A)

•  Must be completed and signed by a licensed psychiatrist or licensed psychologist

•  School Psychologist reviews and signs

Role of the School Psychologist

•  Review documentation from the referring psychiatrist or licensed clinical psychologist

•  Review the treatment plan and confer with the referral source on the school re-entry transition plan

•  Participate in the school team meeting and provide input in the action plan

Role of the SIT Team

•  Develop a plan of action for the student •  Identify instructional objectives on the

IEP •  Review accommodations on the 504

plan •  Collect student work and provide a

duplicate set of books •  Ensure that classroom teachers mark

attendance in SchoolMax

Role of the Classroom Teacher

•  Provide work aligned to the Common Core standards

•  Correct student assignments and enter grades in SchoolMax

•  Ensure that work addresses IEP goals and or reflects appropriate 504 plan accommodations

Home/Hospital Teaching Service Delivery Options

Direct face-to-face instruction by an instructor who teaches the student in the home, hospital, therapeutic center or other designated site.

Virtual-live instruction taught by highly-qualified teachers. This service is available to a student with a computer and internet access.
