Trouble-shooting

Students having trouble with login to HS Academic Evaluations.

Call Office of Student Financial Assistance (OFSA) 1-888-827-2004 (Press “1” for FACTS H.S. Eval.)WHY? Login created & maintained by Statewide Student Financial Aid Database (SSFAD). All student demographic info must match.

Incorrect info on student’s evaluation. Transcript must be corrected at district/school level

and resubmitted Student can’t log back in to an admissions application.

“Check” login (pin/password). The admissions applications & the HS Evaluations do not use the same login info. Admissions apps use the student-created FACTS login (pin/password)

District Transcript Transmissions

Batch upload - 3 times a year; 5 times if on Block Scheduling1. First submission must be submitted at the beginning of the school year as

soon as student schedules are finalized in conjunction with Survey 2. Transcripts must include the new classes on the students’ schedules as courses-in-progress. (Aug/Sept)

 2. Second submission must be submitted at the beginning of the second

semester after first semester grades have been posted and second semester schedules are finalized. This submission is in conjunction with the Bright Futures Early Evaluation Submission and the Talented 20 deadlines. Submit second semester courses as courses-in-progress. Specific information regarding Talented 20 and the Jose Marti and Byrd Scholarships will be provided at appropriate time. (Jan/Feb)

 3. Third submission must be submitted at the end of the school year as soon as second semester grades have been posted. This submission is in conjunction with the Summer Evaluation sent for Bright Futures. If scheduling information for the next school year is available at this time, you may send them as courses-in-progress. For schools operating under the trimester or quarter systems, send transcripts at the beginning of the school year and at the end of each term. (May/June)
