Confined Space Entry

A confined space is defined as a place which is substantially (though not always
entirely) enclosed and where serious injury can occur from hazardous substances or
processes within the space or nearby. (HSE: Safe Work in Confined Space L101 2014).

1. Serious injury due to fire or explosion.
2. Loss of consciousness arising from increased body temp.
3. Loss of consciousness or asphyxiation arising from gas,
fumes, vapour or lack of oxygen.
4. Drowning from an increase in the level of liquid.
5. Asphyxiation arising from a free flowing solid or being
unable to reach a respirable environment due to being
trapped by such a free flowing solid.

Confined Space: Interpretation in a Shipyard
A confined space is often
confused in shipbuilding and
repair with a small, enclosed
space, or one with only one
entrance.
As the picture on the left
shows; confined spaces in
shipbuilding can be vast and
contain multiple access points.
If there is a specified risk
present (as discussed on the
flow chart) it is a confined
space under the regulations
and the risks must be
assessed with appropriate and
proportionate controls put in
place.

There are a multitude of differing spaces within a shipyard that can be classified as a
confined space under the regulations; they can be on vessels, within caissons, within
dock control rooms, pump rooms and electrical service areas.

• The unit is now
substantially enclosed.
There is more than one
access point and
mechanical ventilation is in
place, however the hot
work tasks taking place
within this space will now
introduce a reasonably
foreseeable specified risk
thereby making this space
now a “confined Space”.
• The unit is in construction,
walls are now in place
dividing and partially
enclosing spaces, but not
substantially. Natural and
mechanical ventilation
ensures that the air flow is
sufficient despite the
welding fumes and the unit
is not at this point a
confined space.

All Confined Space Assessments need to identify all hazards and risks associated with the task to be carried out within the space.
All controls must be listed to either minimise or eliminate the risks. Additional task based risk assessments can be produced from this.
All tools, materials and equipment to be used are listed in the Confined Space Assessment.
When conducting a Confined Space Assessment the emergency rescue arrangements / plan must be identified.

Assessments Must Include:
All confined spaces require a Competent Person to carry out an atmospheric test of the space before anyone enters, they will test for oxygen levels, flammable and explosive gases / vapors. Depending on the space it may be tested daily or weekly. The frequency of testing will be determined by a prior risk assessment of the space.

This is the gas free certificate
for entry and cold work that is
issued once the Competent
Analyst has passed the
confined space, the HSE
Advisor will only issue a PTW
if this certificate is valid.
Hot work is not permitted
under this certificate as the
Competent Analyst has only
gas tested for oxygen levels.

is issued once the
Competent Analyst has
passed the confined space
free of all flammable and
explosive gases and or
vapours, the HSE Advisor
will then confirm this prior to
any permit being issued for
hot work.

For all confined space work the person in
charge of the job must raise a PTW at the
relevant Permit Control Centre.
A task based risk assessment is required
that will determine the type of permit
required: hot, cold, electrical or painting.
The Safety Advisor will ensure a space
assessment is carried out by both the
Supervisor (and his tradesmen) for the
physical hazards, and a Competent Person
for atmospheric hazards and that these are
annotated to the permit.
Supervisor will agree upon all safety
controls listed on the permit, communicate
these and the findings of the space
assessment to all entrants and ensure that
all controls are in place before anyone
enters the confined space.
The name(s) of all persons involved in the
task will go onto the permit.

• All Confined Space work is
located by a numbered pin
on the displayed general
arrangement drawing of the
vessel. The pins
correspond to the work
listed on the Confined
Space Log which identifies
the trade, location and the
name of the top man.
• The Standby Man takes a two way radio to remain in
contact with the Health & Safety and Security Departments
and also a tank entry register (right) that is completed at the
space and kept updated throughout the task. A designated
channel is kept for these communications, with the option
to utilise another channel for 2way communications
between entrants and Standby Man where required.

An entry tag that is displaying the
green tag shows that the confined
space has been tested and passed, it
displays what type of work can be
carried out within the confined space
i.e. entry, hot or cold work. If the tag is
removed from the holder, on the back,
it will show who gas tested the space
and on what date.
An entry tag that is displaying
the words “DO NOT ENTER”
dictates the space has not yet
been gas tested therefore
cannot be entered.

• Where required, Cammell Laird uses both Personal Air Monitors (PAMS) and Safe
Air Monitors (SAMS) to monitor the atmosphere during entry into confined spaces.
The PAMS remain on the person of the entrants, whilst the SAMS are fixed into the
space and continuously monitor the atmosphere.

Standby / Sentry
The Standby / Sentry MUST be trained and understand the roles and responsibilities of his/her job and should carry a 2 way radio at all times.
The Standby / Sentry is responsible for checking all PPE, safety equipment and rescue equipment is worn / in place before allowing anyone to enter the confined space.
The Standby remains outside the confined space and monitors the personnel working in the space. Regular communication.
The Standby monitors the surrounding area for hazards that may become present during the task.
The Standby should have a record of all personnel inside the space. He should have a completed Tank Entry Card on his person.

Confined Space Rescue
If a personal gas monitor (or safe air monitor) alarms whilst in the
confined space then ALL personnel are to exit the space immediately,
the top man raises the alarm via radio. If emergency escape sets have
been deemed necessary by the risk assessment then they are worn
whilst exiting the confined space. If all entrants cannot exit the space
the Emergency Response Team will be called via radio to instigate first
response whilst the emergency services are on route. The ERT have
advanced first aid and will attempt to stabilise any casualty and provide
a supply of air until a rescue can be performed.
