This new three storey facility is located on the old Hornsey Hospital site and embraces the existing listed memorial building. The clinical accommodation reflects new care models and is arranged around a central foyer space which contains reception areas, waiting spaces and a new cafe. There is also day care unit for older people with dementia which opens onto a private therapy garden at the rear of the site.

At the heart of the building, there is a two storey, glazed, public atrium space that provides a backdrop to the listed war memorial. Co-located on the site is a Community Mental Health Resource centre. This is a base for West Haringey Community Mental Health Team for Older People. Also, co-located on this site is a Dementia Care Centre

Under the planning agreement, the existing war memorial had to be retained and has been fully restored to form a dramatic design feature which dominates the front entrance of the new health centre. In addition, some of the more important artefacts from the former hospital were salvaged and forms part of the new memorial garden to the front of the property. The centre also provides two private garden spaces for both the new Dementia Day Care Centre and the physiotherapy department.
