Waiver and Release
1. The undersigned states that he/she is in good health or has medical approval to engage in Martial Arts training, which is rigorous and involves physical contact. The undersigned agrees that the “Agencies” described below shall not be responsible for death or injuries of any kind to participants whether or not directly of indirectly caused by or attributable to any fault or negligence of any of the “Agencies” or of others.
2. The undersigned shall indemnify and save harmless the “Agencies” (meaning Nippon Budo Jinmukan, DojoMedia, Japan Karate-Do Ryobu-Kai, JKR Kansas, their agents, students, instructors, affiliates and employees) against all claims, loss and expenses which any of the “Agencies” may incur as the result of any claim or lawsuit made or instituted by or on behalf of the undersigned (and by or on behalf of any minor for which the undersigned has signed), including, without being limited to, any claim or lawsuit based upon the alleged fault or negligence of any of the “Agencies”.
3. The undersigned acknowledges that it has been explained to him/her and he/she understands that Martial Arts and other physical activity taught by the “Agencies” and engaged in by participants involve a risk of death or bodily injury. The undersigned freely assumes such risk and understands that he/she cannot hold the “Agencies” or others under their supervision or control liable for any death or injury that the undersigned may sustain while learning or practicing Martial Arts and other physical activity taught by the “Agencies” or when using any of their facilities or equipment, whether with or without a supervisor, by himself/herself, or with one or more fellow students.
4. The undersigned further releases all rights to any photos or videos taken during his/her training and makes no claim for compensation in regards to any use of such photos or videos.
