October begins a new wellness year! We will continue to offer the Healthy Habits Rewards Program (HHRP) in an effort to help keep you and your family healthy. This program also offers the opportunity for you to be accountable to keep the cost of health care lower. Through achieving the program requirements, you can waive the $15 weekly wellness surcharge - that’s a $780 savings; for the 2017 benefit year! Working together, we can create a healthier work place and you reap the benefit of not paying the weekly wellness surcharge. To ensure American Trim adheres to all government rules (HIPPA and ACA), Medical Mutual will once again help administer our wellness program. The Healthy Habits Rewards Program (HHRP) will begin October 1, 2015 and end August 31, 2016.

This year, you and your enrolled spouse (if applicable) will be required to complete a Health Screening. In addition, the enrolled employee will need to attain additional required Rewards points. The Healthy Habits Rewards Program is divided into two levels of participation:

1. Complete Health Screening = 25 Rewards points 2. Complete Additional Activities = 75 Rewards points

Total Points = 100 Rewards points

Program Change: Completion of the Health Assessment is not a requirement this year. However, you will receive Rewards points for completing the Health Assessment if you chose to complete the assessment. Like last year, only enrolled employees need to earn an additional 75 Rewards points.

Requirement: Both employee and enrolled spouse, if applicable, are required to complete a health screening. This activity must be complete by January 31, 2016 and is worth 25 points.

Program Goals: To qualify for the health care premium reduction in 2017, only enrolled health plan

employees must achieve a minimum of 100 Rewards points. The 2015 -2016 HHRP offers a large selection of preventive screenings, education, and physical activity or health and wellness programs. For a complete list of available activities, review the HHRP Rewards Points document available at YESS/Healthy Habits, or on the SuperWell for Life website. Keep track of your completed activities throughout the program year by visiting the SuperWell for Life website. Many activities require completion or continuous logging on the SuperWell for Life website.

Eligibility: The Health Habits Rewards Program is voluntary and offered to all American Trim health plan enrolled employees and enrolled spouses, if applicable.

Step One – Required Activity: Employee and Enrolled Spouse sign up and participate in an onsite health screening – 25 points. The 2015 onsite Health Screenings will be held during the month of October. Those who participate in the screening will earn 25 Rewards points. (Please allow four weeks for points to be added and only employees will see point accumulations on the SuperWell for Life website.) All enrolled employees and enrolled spouses will be screened for cholesterol, glucose, body mass index (BMI) and blood pressure onsite at no cost to you. If you and your enrolled spouse do not complete the health screening, you will be ineligible to receive additional Rewards points and you will receive the weekly wellness surcharge for the 2017 benefit plan year. Please review the schedule to select a date, time and location that is most convenient for you. Note: You and your enrolled spouse do not have to attend the same session, and you can complete the screening at any of the locations regardless of your work location. The on-site health screenings are offered at no cost to you so take advantage.

Remember, you are required to fast for at least 10 hours prior to your screening in order to receive accurate glucose and cholesterol readings. Please arrive on time of your schedule appointment to help ensure a smooth screening process for everyone.

What to expect at an on-site health screening: Your appointment will only take about 15-20 minutes. Here is how it works:

1. Check-In Station: Check in and complete your registration. 2. Fingerstick: A small blood sample is taken from your finger to measure your cholesterol panel and

glucose levels. 3. Blood Pressure and BMI Stations: Receive readings for your height, weight, Body Mass Index (BMI),

blood pressure and resting heart-rate. 4. Results: Receive your results immediately after the screenings. (You are encouraged to use this

information to complete the online Health Assessment.) 5. Counseling: One-on-one counseling sessions are available with a clinical professional to discuss your

results. These screenings are important gauges of your overall health and we encourage you to take advantage of them. For many years, American Trim has offered health screenings for employees and spouses to “know your numbers.” This year we encourage you to not only know your numbers, but to strive to meet the HHRP goals (shown in green below). The HHRP Goals, while more generous than those of the National Institute of Health Standards, are guidelines to improve your overall health. The weekly wellness surcharge is not impacted by your achievement of the HHRP Goals for the 2015-2016 HHRP.

Option #2: Physician Health Screening If you and/or enrolled spouse are unable to attend an on-site health screening, and have seen or will see your physician between September 1, 2015 and January 31, 2016, you may have your provider complete the Physician Health Screening Form. The Physician Health Screening Form (available through your Benefits Department), must be submitted to Medical Mutual by January 31, 2016 to earn Rewards points and be eligible to participate in the HHRP. You will be responsible for any additional costs from a physician’s visit, so we encourage you to take advantage of the onsite health screening program. Late or incomplete forms will not be accepted.

Step Two – Required Points: Employee only will need to complete additional wellness activities by August 31, 2016 – Total 100 points. During the Health Habits Rewards Program (October 1, 2015 – August 31, 2016) you may complete the following activities of your choice to accumulate the remaining 75 Rewards Points:

There are many offered wellness programs and activities throughout the HHRP. The following is a description of what the activities are, how to complete them, where they are located and how many points they are worth. It is your responsibility to review the activities and maintain completion throughout the HHRP to ensure you achieve the required points total. Online Health Assessment: The Health Assessment (HA) is a confidential online health resource tool that helps you understand your overall health and explains your risks for certain medical conditions. It’s recommended to use your health screening results to complete the HA for a complete wellness profile report. Please follow the instructions for how to complete the HA (located on the SuperWell for Life Website, Health Promotion Tab, then Health Assessment page). If you complete during the program timeframe, 10/1/2015 - 8/31/2016, you will receive 10 Rewards points. Rewards points will not display on the SuperWell for Life website until 4 weeks after completing the HA. Preventive Care Visit(s): Establishing a relationship with your physician and having routine blood work can help determine health risks and prevention of chronic conditions. Additionally there are several preventive screenings and/or exams that are equally important to your overall health and well-being such as a routine mammography and pap screening, PSA screening, colonoscopy or an eye exam. You are eligible for Rewards points for up to six preventive care visits and/or screenings during the program timeframe, 10/1/2015 - 8/31/2016. Each visit is worth 10 Rewards points with a maximum of six screenings/exams during the program timeframe. To receive up to 60 points simply attend your appointment; visit the SuperWell for Life website, click on the Health Promotion tab, select the healthcare visits page, check the box next to the preventive care you completed, enter the date of your appointment and click Submit to receive points. Dental Visit(s): Dental health is so important to your overall health and the recommendation is to have two routine dental exams every year. You are eligible for reward points for each routine dental exam you have during the program timeframe, 10/1/2015 - 8/31/2016. Each visit is worth 10 Rewards points with a maximum of two exams during the program timeframe. To receive up to 20 points simply attend your dental appointment(s), visit the SuperWell for Life website, click on the Health Promotion tab, select the healthcare visits page and enter the date of your dental exam and click Submit to receive points. Flu Vaccination: Receiving the flu vaccination can protect you, your coworkers and family members from contracting the flu virus. Participate in an American Trim worksite flu clinic and no further action is needed to receive 10 Rewards points. If you receive the vaccination from your local pharmacy or healthcare provider, you will need to submit documentation to your Benefits Department by 12/31/15. The Flu Vaccination Verification Form can be obtained from the Benefits Department or on the SuperWell for Life website, Health Promotions tab, and then Flu Immunization page. Educational Modules: are designed to teach you about a specific health topic. Each module consists of a short, informative lesson followed by a 10 question quiz (passing of 80% is required to receive up to 18 Rewards points.) Each module is worth 3 Rewards points with a maximum of six modules during the program timeframe. To access the modules, go to Medical Mutual’s SuperWell for Life website, click on the education tab and then select the educational module page. Points are awarded automatically after you successfully pass the quiz.

SuperWell Seminars: are podcasts designed to provide you with the opportunity to learn more about a wellness topic. Each seminar is conducted by a qualified health professional and will provide you with important, reliable information that promotes and encourages healthy living. At the end of each seminar, there will be a short quiz (passing of 80% is required) to receive up to 18 Rewards points. Each seminar is worth 3 Rewards points with a maximum of six seminars during the program timeframe. Points are awarded automatically after you successfully pass the quiz. To access the seminars, go to Medical Mutual’s SuperWell for Life website, click on the education tab and then select the SuperWell Seminar page. Onsite Educational Seminars: American Trim will provide the opportunity for employees to learn more about a wellness topic. Each seminar is conducted by a qualified health professional and will provide you with important, reliable information that promotes and encourages healthy living. Attend the offered seminars throughout the HHRP and receive up to 9 Rewards points. Each seminar is worth 3 Rewards points with a maximum of three onsite seminars during the program timeframe. Program details will be announced closer to the seminar timeframe. Points can take up to 4 weeks to display on the SuperWell for Life website. Disease Management: Medical Mutual offers several disease management and maternity program to help members with a chronic condition or who just found out they are pregnant. If you are actively engaged in a program (asthma, COPD, chronic pain, depression, diabetes, heart failure or maternity management) for at least 6 months during the program timeframe, 10/1/2015 - 8/31/2016, then you will automatically be awarded 10 Rewards points. (Rewards points will not display on the SuperWell for Life website until the end of the Rewards program.) For more information about the programs call Medical Mutual at (800) 258-3175. Weight Watchers: Medical Mutual offers a reimbursement program to help members with their weight loss goals. This program offers up to $150 in reimbursement per calendar year for members who participate in three or four month of Community Meetings. Visit the Healthy Living tab on My Health Plan for more information or call (866) 204-2878 to enroll in a Weight Watchers program. Follow the steps to receive reimbursement from Medical Mutual and if completed during the HHRP timeframe, 10/1/2015 - 8/31/2016; provide a copy of the reimbursement form to the Benefits Department by 8/31/2016. You will then be awarded up to 20 Rewards points. Each completed series is worth 10 Rewards points with a maximum of two series during the program timeframe. (Rewards points will not display on the SuperWell for Life website until two weeks after providing documentation.) Quitline: Medical Mutual offers a program to help tobacco users give up the habit for good. This program offers one-on-one coaching, a personalized quit plan, educational materials and a supply of nicotine replacement therapy. Visit the Healthy Living tab on My Health Plan or call (866) 845-7702 to enroll in the Quitline Program. If you complete the program during the HHRP timeframe, 10/1/2015 - 8/31/2016; provide a copy of the certificate of completion to the Benefits Department by 8/31/2016. You will then be awarded 10 Rewards points. (Rewards points will not display on the SuperWell for Life website until two weeks after providing documentation.) Community Education Programs: Many community organizations, such as local hospitals, offer programs to help with many health and wellness needs. The program includes but is not limited to: Diabetes Management, Weight Management, Nutrition Management, Stress Management or Tobacco Cessation. If you complete a community program during the HHRP timeframe, 10/1/2015 - 8/31/2016, return a completed copy of the Community Education Program Form to the Benefits Department by 8/31/2016. You can earn up to 50 Rewards points for the year! Each completed program is worth 10 Rewards points with a maximum of five programs during the program timeframe. (Rewards points will not display on the SuperWell for Life website until two weeks after providing documentation.)

Health Fair: Visit at least 5 booths at the American Trim Health Fair in October to receive 11 points. The verification card will need completed and return to the Wellness Committee the day of the event. Healthy Habits Wellness Challenge: American Trim holds wellness challenges at each location tailored to help employees reach health and wellness goals. Employees who meet the weekly participation goals, regardless of outcomes, will earn up to 30 Rewards points. Each challenge is worth 10 Rewards points with a maximum of three programs during the program timeframe. Program details and requirements will be announced closer to the wellness challenge timeframe. (Example: Biggest Loser Contest.) Cardio Log: is an online journal to keep track of your daily exercise routines. Recording your workouts can boost your motivation, keep you on track and allow you to see progress over time. To get started, go to Medical Mutual’s SuperWell for Life website, click on the fitness tab and then select the cardio log page. Select a date on the calendar and an activity from the Activity list. Enter your stats and click Add Entry. Note: You can only enter activities within the past 10 days. The Cardio Log will not allow you to enter activities prior to that timeframe. Consistently logging a minimum of 8 days a month can earn up to 55 Rewards points for the year! Points are automatically awarded on the SuperWell for Life website. Community Fitness Event: Participate in any community fitness-related event and receive up to 30 Rewards points. Each event is worth 10 Rewards points with a maximum of three events during the program timeframe. Submit documentation of the event to the Benefits Department by 8/31/2016. The Community Fitness Event Participation Form can be obtained from the Benefits Department or on the SuperWell for Life website, Fitness tab, and then Community Fitness Event page. Points can take up to 4 weeks to display on the SuperWell for Life website. Healthy Habits Fitness Event/Day(s): American Trim holds multiple events per Rewards program to encourage all employees to be active. For each event completed you will earn 10 Rewards points, that’s up to 30 Rewards points. Sign in at each event and your points will automatically be awarded. Points can take up to 4 weeks to display on the SuperWell for Life website. (Activities include: Onsite 5K walks/runs or American Trim softball tournament.)

If you have never been to My Health Plan you will need to register: 1. Click Create an Account under the “Not Registered” heading. 2. Enter the required information in each field on the Registration page (Please note: You will need the

username and password you create to access the website each time, so please write it down and keep it in a safe place.) If you do not have an e-mail address, please enter [email protected] to continue creation of your profile page. You will want to turn off the email message feature as this is not an active email account.

3. If you need further assistance logging in, call the Medical Mutual Help Desk at 800/294-7583.

Step 3: Select the Healthy Living tab and then click on the link “SuperWell for Life”.

Please Note: The Healthy Habits Rewards Program information will not be included in the SuperWell Tutorial due to the customization of your wellness program.
