1. INTRODUCTION
This user guide introduces the online Quality Oncology Practice Initiative (QOPI®) system, a
Quality Practice initiative from the American Society of Clinical Oncology (ASCO).
QOPI® is an oncologist-led, practice-based quality assessment program, designed to promote
excellence in cancer care by helping practices create a culture of self-examination and
improvement.
QOPI® provides a standard methodology, robust library of quality metrics for oncology, and a
collection tool to reliably and routinely assess care, inform quality improvement activities, and
demonstrate quality to patients and external stakeholders.
This user guide is intended for the practice users and serves as a technical guide which can be
used as an information booklet with a technical perspective.

2. QOPI SYSTEM REGISTRATION
The registration page in the QOPI® system enables users to Register in the system, which is a
prerequisite to accessing the QOPI dashboard.
Upon entering the login URL (https://myqopi.asco.org/registration/login.aspx) the registration
login page is displayed.
This web page is accessible through all web browsers (Internet Explorer, Google Chrome, Mozilla
Firefox etc.), but the most up to date versions of Chrome or Firefox are recommended for best
results.

2.1. REGISTRATION SCREEN
If you are registering for the first time in the QOPI®
system, you need to apply for New Registration (Refer
Figure 1).
You will need an ASCO member ID. If you are unsure
about what to enter as the ASCO member ID, please
contact the QOPI® Help Desk.
For new registration: (Refer Figure 2)
• The ASCO Member ID and ASCO member last name
must match the information in ASCO’s Membership
database.
• The CAPTCHA code is for security purposes and
needs to be entered as displayed in the box.
• If you have trouble reading the
CAPTCHA, generate a new code by
clicking on the green captcha button
provided next to the CAPTCHA entry field.
• The Terms of Use are the agreement and regulatory
set of rules for legal use of the service. It is advised
that you read the Terms of Use carefully before
proceeding.
• To initiate the registration process, it is mandatory to
check the I agree to the Terms of Use box.
Clicking Proceed navigates you to the QOPI System Registration page.

2.2. QOPI SYSTEM REGISTRATION
The QOPI® system registration module displays 3 tabs:
• Basic Demographics
• Primary Contacts
• Terms and Conditions
2.2.1 BASIC DEMOGRAPHICS
To initiate the system registration process, you must provide information for the Basic
Demographics tab. (Refer Figure 3)
• A red asterisk defines MANDATORY fields.
• If the country selected is the UNITED STATES, the state and city fields are auto-
populated upon entering a valid postal code.
• For a country other than the United States, the postal code and zip must be manually
entered.
• The Contact number should be only 10 digits and no special characters like dashes or
plus sign should be entered. An error message will be displayed if special characters are
used.
• Extension and FAX number are optional.
Clicking Proceed to Primary Contacts, saves the entered information and you are directed to
the Primary Contacts tab.

2.2.2 PRIMARY CONTACTS
The Primary Contacts tab captures the QOPI® registrant’s demographics and contact
information. It has two sub tabs - Physician Corresponding (PHC) and QOPI Program
Administrator (PRA).
1. Physician - Corresponding (PHC) PHC is the contact information of an ASCO member who is the registrant.
It captures the information for: (Refer Figure 4)
• Name
• Address
• Contact details
• Email address
After the PHC information is filled, you can select one of the following:
• Back to Basic Demographics: This saves the entered information and takes you back
to Basic Demographics.
• Proceed to Program Administrator (PRA): This saves the entered information and
takes you to the sub-tab PRA.
2. QOPI Program Administrator (PRA) PRA is the contact information of a practice administrator who looks after the QOPI
workflow within the practice.

In the QOPI® program administrator or PRA tab (Refer Figure 5), if the PRA is the same as
the PHC, then you can check the box Same as PHC.
This auto-fills the PHC information for the PRA. If positions are filled by separate
individuals, manually enter the details for the PRA into the appropriate fields.
Clicking on the Proceed to Terms and Conditions, saves the entered information an directs you to the Terms and Conditions tab.

2.2.3 TERMS AND CONDITIONS
The Terms and Conditions tab displays a list of all the pre-qualification terms and conditions
(Refer Figure 6). You need to select all the checkboxes before submitting the application.
Upon clicking Submit,
• The system registration details are submitted to ASCO for approval.
• A congratulatory message is displayed on the screen once the information is submitted
(Refer Figure 7).
• You will receive a thank you email for registering with QOPI®. (Refer Figure 8)
• Additionally, an email is sent to the QOPI® Help Desk staff informing them of the
initiation of the registration process.

2.3. APPROVAL PHASE OF RE GISTRATION
2.3.1 APPROVAL FROM ASCO
The QOPI® Help Desk staff needs to approve the request received via email in order for you
to proceed with the remaining tasks. You may be contacted if there are discrepancies in the
registration that need to be clarified before QOPI® staff can approve the registration.
2.3.2 WELCOME EMAIL TO QOPI PARTICIPANT
• Once ASCO approves the QOPI® system registration request, an email is sent to the
Corresponding Physician (PHC) (Refer Figure 9) and the QOPI® Program Administrator
(PRA). (Refer Figure 10)
• If the Physician Corresponding (PHC) also works as QOPI® Program Administrator (PRA),
only one email is sent to the registered account.
• Access the link provided in the email to complete the registration process.
• Please disregard the login credentials starting with “admin” and only use the login
credentials that have your name.

2.3.2.1 ONE TIME PASSWORD RESET
The reset your password link in the email redirects you to the reset password window
(Refer Figure 11) to reset the password for your account.
• The requirements for password are defined to the right and it’s mandatory to follow
them.
• You must select a password challenge question from the drop-down. The questions are
predefined and cannot be changed.
• Provide an answer to the question.
• Clicking Reset saves your password and security questions details and redirects you to
a new window presenting two options (Refer Figure 12). Registration allows you to
complete QOPI® Round Registration. Dashboard will take you to the QOPI® Dashboard.
This completes the QOPI® system registration process.
3. ROUND REGISTRATION
To complete the QOPI® Round registration process you will select Registration (Refer Figure 12).
You will be taken to the login page where you will use the username you have received and
password you have set up to login to the Registration Portal.

3.1. QOPI® ROUND REGISTRATION SCRE EN
If you are an existing QOPI® user and have already
received QOPI® system credentials, enter them in
the appropriate fields. (Refer Figure 13)
If you have signed up through the QOPI Reporting
Registry (QRR), you can use the same login
credentials to log into the QOPI® Registration Portal
and QOPI® Dashboard.
If you are an existing user and have forgotten your
password or are asked to reset your password,
enter your username in the UserName field, then
click on I forgot my password.
3.2. FORGOT PASSWORD
I forgot my password: If you are unable to log into the portal due to the incorrect or forgotten
password, click on I forgot my password link. (Refer
Figure 13)
You will be taken to the Forgot Password window.

• In order to recover the password through email,
you need to enter the pre-registered email
address
OR
• Respond to the challenge question and answer
from the predefined response given while
registering for the first time.
Note: You must enter the correct username to be
redirected to this window. If under any circumstance
the username is not available, kindly contact the
QOPI® Help Desk.

4. REGISTRATION TILES
The following tiles are displayed on the registration screen: (Refer Figure 15)
• Practice Tile
• Site Tile
• Legal Agreement Tile
• QOPI® Round Registration/Participation Tile
It is compulsory to fill the information in the registration tiles from left to right.
All the tabs within the tile should have green check marks next to them before proceeding to the
next tile.

4.1. PRACTICE TILE
The Practice tile collects information pertaining to the practice (Refer Figure 16). There are 9
tabs that capture the following practice specific information:
• Basic Demographics
• Primary Contacts
• Affiliations
• Patient Characteristics
• Clinical Trials
• Staffing and Patient Services
• Practice Out-Patient and Chemotherapy Facilities
• EMR System
• Health Plan Program
4.1.1 BASIC DEMOGRAPHICS
This tab is pre-populated with the information captured during the system registration
process, but can be updated if necessary.
4.1.2 PRIMARY CONTACTS
The Primary Contacts tab enables you to enter the Corresponding Physician (PHC)
Information and the QOPI Program Administrator (PRA) Information.
1. Corresponding Physician (PHC)
The PHC is the contact information of an ASCO member who may also be the registrant.
It captures the QOPI® registrant’s demographics and contact information such as
registrant name and contact details, communication preferences, and the corresponding
physician ASCO member information. This information was entered during the QOPI®
System registration process therefore most of the information is pre-filled.
Communication Preferences: You are required to select either Yes or No.
If the answer to the question “Would you like to receive QOPI information via email?”
• Is Yes, it generates a series of questions related to available information topics. You
are required to select Yes or No for each option and at least one answer must be
Yes.
• Is No, no QOPI® updates are communicated to the registered email address.
You can change these selections at any time.

After the PHC information is filled you can select one of the following:
• Save & Back to Basic Demographics: This saves the entered information and takes
you back to Basic Demographics.
• Save & Proceed to Program Administrator (PRA): This saves the entered
information and takes you to the PRA tab.
2. QOPI Program Administrator (PRA)
QOPI program administrator or PRA tab displays the contact information of a practice
administrator who manages the ASCO workflow within the practice.
If the PRA personnel is the same as PHC for a practice, you can check the box, Same as
PHC. This will populate all the PHC information that has been entered in PHC. If they are
different then you can enter the specific PRA details.
Note - If the PHC/PRA details have changed, they should be updated. A user with
administration privileges (PHC or PRA) should enter the new First Name, Last Name and
E-mail address corresponding to the new PHC and/or PRA. Once these changes are
saved, an email with the new login credentials and reset password link is sent to the new
user. The new user can then reset their password.

• Click Save & Proceed to Affiliations.
This saves the entered information and takes you to the next tab - Affiliations.
4.1.3 AFFILIATIONS
The Affiliations tab collects the practice affiliation type, the ownership and agreement
details. You can set the practice affiliations as per the norms. For example, Academic full
time, Employee etc.
Note: Practice Ownership dropdown is enabled only when the Employed option in the
Practice Affiliation tab is selected.
The practice ownership can be of the following types:
• Hospital owned
• Large physician practice
• Large insurance / HMO group
• Veteran’s Health Information
• Other Government Health Care Information
• Other (When selected, the text is enabled to enter details)
4.1.4 PATIENT CHARACTERIST ICS
The Patient Characteristics tab captures the patient details such as:
• New patients annually
• Percent covered by private insurance
• Percent covered by Medicare
• Percent covered by Medicaid
• Percent uninsured
• Percent of non-English speaking patients
4.1.5 CLINICAL TRIALS
The Clinical Trials tab captures the details of the trials associated with NCI, pharma,
investigators, etc. If the practice does not support any clinical trials, then you can select the
Not Applicable checkbox.
4.1.6 STAFFING AND PATIENT SERVICES
This tab defines the number of oncologists and the types of services they offer for the
practice. (Refer Figure 18)
The Total Practice NPIs count is updated whenever providers are added to the Provider tab
through the QOPI® dashboard. This field is auto-populated by the system and cannot be
edited.

The sum of (FTE Hematologist-Oncologists Only + FTE Medical Oncologists Only + FTE
Medical/Hematologist-Oncologists, refer Figure 18 for fields highlighted in red) is the default
value of the Total FTE that is displayed on the Round Participation screen.
This is a very important tab. Correct calculation of FTEs is crucial for calculating the target
number required for chart abstraction. If you need assistance calculating FTEs, please
contact the QOPI® Help Desk.
Total FTE count must be an integer.
4.1.7 PRACTICE OUTPATIENT AND CHEMOTHERAPY FACILITIES
This tab describes the outpatient facilities, their count and the details for the chemo infusion
centers.
4.1.8 ELECTRONIC MEDICAL RECORD (EMR) SYSTEM
This tab captures the EMR integration details, and the system used by the practice for
integration. Note, it is not required to capture your patient data in an EMR in order to report
with QOPI®.
4.1.9 HEALTH PLAN PROGRAM
This tab captures the Health Plan details for QOPI®. Selection of Health Plans is optional.
The Health Plan Opt-in functionality enables the Practice to request ASCO to submit
confirmation of the Practice’s participation in QOPI® or of QOPI® Certification status.
To participate, choose the health plans that you would like ASCO to notify of your QOPI®
participation. You may select more than one health plan by selecting multiple items.
• Click Save to save the entered details.
• Please ensure that all the mandatory information is completed to proceed to the site
tile.
• Click Proceed to Site to proceed to the site tile.

4.2. SITE TILE
This tile collects information related to practice site. A site is defined as the physical location of
a hospital or a clinic separate from the practice.
There are 8 tabs to capture the required site related data
(Refer Figure 19).
• Basic Demographics
• Affiliations
• Patient Characteristics
• Clinical Trials
• Staffing and Patient Services
• Site Out-Patient and Chemotherapy Facilities
• EMR System
• Health Plan Program
All the tabs are the same as in the practice tile. Only the
Primary Contacts tab is not available as this is captured
only at the practice level.

Clicking the Site tile opens the site details page (Refer Figure 20).
• The default site is the main practice with information populated from the Basic
Demographics tab on the practice tile. Any information added at the practice level reflects
on main site level (for site Id 1).
Site Id = 1 is always Primary Site
• The site details page enables you to update an existing site or add a new site. Selecting an
existing site opens the site details which can then be edited using the tabs in the left
navigation panel. To add a new site, click Add New Site. This opens a window where the
new site information can be completed.
Click Proceed to Legal Agreement to proceed to the Legal Agreement tile.

4.3. LEGAL AGREEMENT TILE
The Legal Agreement tile is a gateway to access the multiple
agreements. (Refer Figure 21)
4.3.1 TYPES OF AGREEMENTS
There are multiple types of legal agreements that need to be signed between the registry
and practice such as:
• Business Associate Agreement (BAA)
• QOPI® Participation Agreement
• Veteran's Health Administration
• EU Participation and General Data Protection Regulation Agreement Data Protection
Addendum (GDPR DPA)
• Country-specific Participation Agreement
4.3.1.1 BUSINESS ASSOCIATE AGREEMENT (BAA)
The BAA agreement is a standard HIPAA compliant agreement between ASCO and
participating practices. It lays out the safeguards and rights of access, amendment,
accounting, and audit that ASCO provides to the practice for participation in QOPI®.
The BAA was written for broad application to all participating practices, so customizations
cannot be accommodated. This agreement can be accessed by clicking on the Business
Associate Agreement (BAA) link. The agreement should be read in its entirety and signed
by someone at the practice with legal authority to sign agreements.

4.3.1.2 QOPI® PARTICIPATION AGREEMENT (QPA)
The QPA is signed between the registry and practice and was written to apply to all
participating practices, so customizations cannot be accommodated. This agreement can
be accessed by clicking on the QOPI® Participation Agreement (QPA) link. The agreement
should be read in its entirety and signed by someone at the practice with legal authority to
sign agreements.
4.3.1.3 VETERAN’S HEALTH ADMINISTRATION (VHA) AGREEMENT
It is an additional option to identify your practice as a Veteran’s Affairs Center (VA)
providing oncology care.
It must be added through the
Affiliations tab of the
Practice tile.
(Refer Figure 22)
The Veteran’s Health Administration (VHA) has entered into a National BAA with QOPI®
which governs the data sent by the practice. Designating your practice as an VA practice
will change the Legal Agreements that populate on your Legal Agreement tile.
VHA practices will not sign the BAA but rather check the box that appears attesting that
they are a VHA practice. (Refer to Figure 24 on the next page under Signing the Legal
Agreements.) VHA practices are still required to sign the QOPI® Participation Agreement.
• A checkbox Confirm that this site is a VHA site is displayed on this page only if VHA was
selected in the Practice tile – Affiliations tab.
o Before you proceed with the signing process, confirm that the practice is a VHA site.
o If the box is unchecked, a pop-up notice is displayed: Please confirm the correct
affiliation was selected under Affiliation tab on Practice Tile, Practice Ownership .
In order to participate, US non-VHA sites must agree to standard BAA and US VHA
sites must confirm VHA site status.

4.3.1.4 EU PARTICIPATION AND GENERAL DATA PROTECTION REGULATION AGREEMENT -
DATA PROTECTION ADDENDUM (GDPR DPA) (IF APPLICABLE)
Practices that are part of European Union must sign the GDPR DPA and EU Participation
agreement.
4.3.1.5 COUNTRY SPECIFIC PARTICIPATION AGREEMENT
Non-EU international practices must sign the Country-specific Participation Agreement that
applies to the country their practice is located in.
4.3.2 SIGNING THE LEGAL AGREEMENTS
• Clicking the Agreement tile opens agreement page (Refer Figure 24) which displays the
instructions for signing the agreement, practice address, and the signing options.
There are two options for signing the agreements - Sign Now or Sign Later.
Sign Now
Select this option if you are an authorized signatory. It opens the agreement PDF and allows
you to sign the agreement. A signed copy of the agreement document along with an email
notification is sent to you and QOPI®.

After the signing process is complete, (Refer Figure 25)
• A record is added to the agreement table with date stamp and the name of the person
who completed signing the agreement.
• If the status of the agreement does not change on its own, click Refresh.
• Signing option column changes to Sign through portal and the status column displays
Signed.
• You are able to view/download the signed copies of the agreement.

Sign Later
Select this option if you are not an authorized signatory or would like to sign the agreement
later. A link to the agreement is emailed to the configured email address for signing at a later
time (Refer Figure 27).
After the signing process is complete-
• A record is added to the agreement table with date stamp with and the name of the
person who completed signing the document.
• If the status of the agreement does not change on its own, click Refresh.
• Signing option column changes to Sign through mail and the status column displays
Confirm Mail. (The status column will change to Sign only after the agreement has been
signed.)
• You are able to view/download the signed copies of the agreement.
Click Proceed to QOPI Round Participation to proceed to QOPI® Round Participation tile.

4.4. QOPI® ROUND PARTICIPATION TILE
QOPI® modules, reporting tracks, and QOPI® round
participation status is displayed in this tile. (Refer Figure
28)
The round participation tile is accessible generally one
month before data abstraction for the round opens.
• Clicking the tile displays the round participation page.
• If the site/practice is affiliated with a fellowship or if
the participation is for the practice level, then only an
individual target is displayed.
• Clicking the QOPI® Round Participation tile takes you
to the QOPI® Round Registration/Participation screen
to select your round information.
There are two types of participation styles (Refer Figure 29)
1. QOPI® Round
There are two QOPI® rounds per year for a duration of five months each. You can
participate in the QOPI® round and perform chart abstraction over the five month
duration. The QOPI® rounds are fixed duration rounds that open and close at predefined
timelines set by ASCO.
• Select QOPI® Round plan to apply for QOPI® Certification/Recertification, Participate
in QOPI® or Maintain Certification.
2. QOPI® Annual Pilot
The QOPI® Annual pilot is currently an invitation-only initiative focused on developing a
method for practices to abstract year-round as well as facilitate easier abstraction for
quality improvement initiatives. If you would like to know more, please contact the QOPI
Help Desk.

• Select QOPI® Round unless you have been invited by QOPI® staff to participate in the
Annual Pilot program and click Let’s Start to proceed.
You are navigated to the landing page of Round Participation.
• Back navigates you to the registration landing page.
4.4.1 PARTICIPATION DETAILS
The round participation page captures the participation type and track/module details.
The participation type is at practice or site level.

• The selected participation round is displayed on the top right corner of the page.
• Click on the practice or site hyperlink to edit the corresponding details.
• Select the box to confirm that the organization’s information is complete, updated, and
accurate.
• Clicking Start opens the Participation Round Settings page.

4.4.2.1 PARTICIPATION
This tab captures the round participation information (Refer Figure 33). All the fields are
mandatory.

The following details are displayed in this tab:
• Data submission method: Default displayed as QOPI® and cannot be changed.
• Future Round Participation: Identifies if you are interested in participating in future
year for QOPI®. If you choose option Yes then the Select Future Round is enabled. The
dropdown allows you to select the year(s) you plan on participating in QOPI® in the
future.
• Medical Oncologist/Hematologist Oncologist/Total FTEs: Indicates the number of
Medical Oncologist and Hematologist Oncologist FTEs who are employed at the
practice/site. The number entered here is used to calculate the target set for the
practice/site.
• Participation for: Confirms if your participation is for QOPI® Certification, QOPI® Re-
Certification, Maintain QOPI® Certification Status or QOPI® Participation.
• Participate using Ad Hoc measure selection: Ad Hoc measure selection is only enabled
for QOPI® Annual Pilot practices. This option is defaulted to No for regular QOPI®
rounds.
• Reset button: Clicking Reset deletes all entered information resetting the form to
blank.
• Save button: Clicking Save saves the information entered navigates to the next tab. If
you do not click on save and switch across by selecting next tabs, the information
entered in the current tab is auto-saved.
4.4.2.2 TRACKS
Tracks are a logical grouping of cross-cutting measures from different modules. Tracks are
displayed based on the Type of Care selected in the Basic Demographics tab of the
Practice tile. They are as follows: (Refer Figure 34)
• QCP (QOPI Certification Program Track)
• PCT (Patient-Centered Track)
• MOQC (MOQC Track)
• FLT (Fellowship Track)
• NTQ (New to QOPI Track)
• Lung Track

• If you select QOPI® Certification or QOPI® Re-Certification in the Participation tab,
then QCP track automatically gets selected (Refer Figure 33) and is mandatory for
participation.
• All measures within the Track are mandatory when you select a specific Track.
• Clicking the track name displays the list of measures associated with the track (Refer
Figure 35).

4.4.2.3 MODULES
Modules are a grouping of like measures and are displayed based on the Type of Care
selected in the Basic Demographics tab. Module tab displays different modules such as:

• Symptom (Symptom/Toxicity Management)
• EOL (Care at End of Life)

• Colorectal
• NSCLC (Non-small cell lung cancer)
• NHL (Non-Hodgkin’s Lymphoma)
• Gyn-Onc
• Palliative Care

4.4.2.4 TEST MEASURES
Test Measures are also known as Research measures as they do not have enough data to
categorize them as regular measures.
Test Measures are displayed depending upon the selection of tracks and modules (Refer
Figure 39).
Select the applicable test measures and click Save & Finish.
You are redirected to the Participation Details page (Refer Figure 40).
Clicking Submit Round Registration completes the registration procedure for QOPI®.

On completion of round participation process, you receive an email which has a link to
access the QOPI® dashboard. (Refer Figure 41)

As you have submitted the configuration for the round participation, the Track/Module
details are updated.
NOTE: The Select/Change Participation Style is disabled once the practice begins
abstracting charts and you will have to contact the QOPI Help Desk if you would like to
change your participation style.
4.4.3 TRACK/MODULE DETAILS
Track/Module details section displays the name of the practice/site with the following icons.

• Participate
Auto-checked when the round participation for the selected practice/site is completed.
If you uncheck it, all participation details are deleted and you need to re-register.
• Target
Displays the target summary of charts to be filled for the selected practice/site. The
summary of the modules is displayed with the individual target level. (Refer Figure 42)

• Details
Shows the details of the configured Track/Module within the selected

Allows you to update the configured Track/Module details. Edits can be
performed before chart abstraction is initiated.

after you register and complete the QOPI® round registration. The dashboard can also be

6. COMMON FUNCTIONALITIES
6.1. SEARCH
The search functionality has been provided for every table included in all the modules. This
allows you to search for a particular record from the list.
The search boxes are placed below the columns names. (Refer Figure 46)
These boxes are not action sensitive and therefore you need to press enter to get the required
results.
6.2. SORT
The up and down arrows placed in front of every column name allow you to customize the
view of the list in ascending or descending order. By default, all the columns are in ascending
order. (Refer Figure 46)
