Introduction

Amavata is the most crippling of the joint diseases It occurs throughout the world in all climates

First explained in Madhava Nidana by Madhavakara

Ama and vitiated Vata play the dominant role so it is called as Amavata

The incidence of Amavata (Rheumatoid Arthritis) is

reported to be 1-2 of general population with a

female to male ratio of 31

Nidana amp Samprapthi

Viruddahara means unwholesome foods or combination of foods which effects adverselyPoor digestion power defective metabolismSedentary life style and also too much exercise

So affected defective metabolism with the influence of nervous system also affects the normal functioning and the products of this reaches Dhamanis(blood vessels) and circulates all over the body and produces symptoms

BodyachePoor appetite feeling thirsty lethargicHeavyness in the bodyFever feelingNon inflammatory and no suppurative nature

Severe AMAVATA features Very difficult to tolerate when becomes severe Can

also be considered as difficult to treat when aggravates

Severe pain in hand foot head ankle neck low back knee and hip joints

Pain and swelling is seen in different parts of body

Pain will be severe as if bit by scorpion (burning sensation and pain as if hit by stick)

with the use of NSAIDrsquos (Non Steroidal

Anti Inflammatory Drugs) glucocorticoids DMARDrsquos

(Disease Modifying Anti Rheumatic Drugs) treatment

remains unsatisfactory through modern system of

medicine Patients are continuously looking with a hope

towards Ayurveda to overcome this challenge

So the Ayurvedic medicinal herbs are moving to

mainstream

Pippali (Piper longum Linn)

Pippali is one of the Rasayana drugs described in Ayurveda It has Agnidipana and Amapachana actions and is helpful in the alleviation of Ama from the body which is the main pathological factor in Amavata ie rheumatoid arthritis

Pippali alleviates Ama the factor responsible for Balabhrmsha or development of autoimmune disorders in the body resulting in the correction in immune system activities Pippali has rasayana and immunomodulator actions Thus because of all these actions together it plays an important role in the Samprapti-vighatana of the Amavata ie rheumatoid arthritis

This plant is popularly used for the treatment of arthritis as folk medicine The decoction of leaves and tender branches of the plant mixed with sugar and ghee is used every day for a month or so The seed oil has also been described to have anti-Rheumatic property

It is tree of Anacardiacae family

found in the lower level of Himalayan range

It is popularly known as Marking nut

This is one of the potent drugs of

Indian system of medicine used for various

conditions it contains a highly irritant oil of

Phenolic nature in the pulp of the fruit

This drug is supposed to have powerful

action on gastro-intestinal tract It stimulates the

digestive mechanism

According to ayurveda involvement of

gastro-intestinal tract is primary in the aetiology of

amavata hence this drug was given a trial to assess

its role in the treatment of the same
