Urinary tract infections – UTI is a very common clinical problem. The spectrum of UTI varies from asymptomatic infection to a life threatening urosepsis.Broadly urinary tract infections can be devided in two parts 1. Lower UTI – cystitis and urethritis2. Upper tract infection – Pyelonephritis
Most common organisms for UTI are E.coli , kleibsella and pseudomonas . Enterococcus ,proteus, and staphylococcus are less common causes along with candida.Risk Factors;
Female sex , diabetes , stone disease , urological abnormalities like obstructive uropathy, prostate enlargement, neurogenic bladder, recent instrumentation, catheterization.Clinical symptoms and diagnosis:
Patients with cystitis and urethritis presents with increased frequency of urination and burning micturition-dysuria with lower abdominal pain. High grade fever with flank pain and dysuria suggests pyelonephritis and more aggressive treatment and admission .
For all pateints with symptoms of UTI, urine cultures should be sent and patients with suspected pyelonephritis RFT and ultrasound of kidney should be done. Urine quantitative cultures with colonies > 1 lac suggests clinicaly significant UTI and needs treatment with antibiotics.Treatment of UTI
Empirical treatment for cystitis and urethritis is oral antibiotics, cifixime / levofloxacin / nitrofurantoin / cotrimoxazole / amoxyclav for 5-7 days and to be modified based on culture report.
Patients with pyelonephritis usualy needs admission and parenteral antibiotics for 7-10 days followed by oral antibiotics. Cefoperazonesulbactum, ceftriaxone, levofloxacin, cefepime - tazobactum can be used f or empir ical t reatment .
Urinary tract Infections Carbapenemsshuld be used only on the basis of culture report or very severe sepsis.
Aminoglycosides are better avoided in view of possible nephrotoxicity.Which patients needs referral to kidney specialists:
Patients with recurrent cystitis, pyelonephritis and urosepsis and any patient with complicated UTI is better managed by a kidney specialist.
Complicated UTI is defined as any UTI in presence of anatomical or functional abnormality like ; diabetes, stone, obstructive uropathy, stricture, abcess, prostate enlargement, neurogenic bladder.How to manage recurrent UTI
Detailed evaluation should be done including USG with full blader and post void ,Xray KUB and sos CT scan, metabolic work up for diabetes and RFT, uroflowmetry and sos urodynamic study for neurogenic bladder.
Apart from correction of underlying cause these patients need long term antibiotic prophylaxis, in which a low dose antibiotic is give at bed time for months.
Cifixime, levofloxacin, nitrofurantoin, cotrimoxazole are agents for prophylaxis.Life style changes for UTI prevention:- Drink more liquids- Use more citrus fruits- Micturition after intercourse reduces chances of UTI in
females- Diabetes control and treatment of kidney stones
