Family coverage includes the insured; spouse;and dependent, unmarried children to age 25

Policy contains a 30-day waiting period

Applicant diagnosed or treated for internal cancer within the last five years for residents of all states (two years for breast cancer for FL residents only)

Skin cancer exclusion rider if skin cancer diagnosed within the last five years

Applicant receiving hormonal therapy within the last 12 months

Any covered person who has had a previous diagnosis of cancer will not be eligible for a First-Occurrence Benefit under this policy for a recurrence, extension, or metastatic spread of that same cancer
