AFFIDAVIT OF LOSS I, Arianne Gloriane Cruz, of legal age, Filipino, single, and a resident of Blk 1, Lot 2 PH II, Balubad Settlement Nangka, Marikina City, after having been duly sworn to in accordance with law, depose and state that:

1. I am the owner of a Voters Identification Card (Voters ID) issued by the COMELEC;

2. Sometime in 2012, while working in the baggage area of Red Ribbon, I misplaced and lost my wallet which contained my Voters ID;

3. I have exerted diligent efforts to find my Voters ID but to no avail;

4. Because of this, I am now applying for a new Voters ID with the COMELEC;

5. This affidavit is being executed to attest to the veracity of the foregoing facts and for whatever legal purpose it may serve.
