Draft of AFFIDAVIT / INDEMNITY

(To be prepared on Rs.100/- Non Judiciary Stamp paper and duly notarized)

I, Mr. /Ms. /Mrs. __________________________________ do hereby state on solemn affirmation as under,

I say that I am the occupant of residential premises as (status of occupant should be stated) wherein PNG connection bearing Business Partner (BP) Number ______________ is installed by Mahanagar Gas Limited (MGL) and the said PNG connection is presently registered in the name of Mr. ___________________

I say that due to __________________ (state reasons), I am not in a position to submit letter / NOC from the registered consumer namely, Mr. ________________________ for transfer of PNG connection to my name.

I say that to in lieu of letter / NOC I am submitting the following documents to substantiate my legal possession / occupation of the above said premises.

I say that apart from me no other person is authorized or entitled to occupy the above said premises either as legal heir or as occupant and in the event of any person claiming through or under the present registered customer, claims or challenges the transfer of PNG connection to my name, MGL may take appropriate action including disconnection & re-transfer PNG connection in the name of such person as MGL may deem fit.

I hereby undertake to Indemnity and keep MGL and its officers indemnified and harmless against any loss, cost, damage, suits, actions and liabilities that may accrue due to my representations herein and consequent transfer of PNG connection to my name.

I further say that if any information provided by me herein is found to be incorrect by MGL, in such an event MGL, may at its sole discretion disconnect my PNG supply without prior notice.

I say that MGL, its employees or authorized representatives at all times have the right to enter into the above premises for inspection, maintenance and disconnection of supply and I undertake not to obstruct their such entry in to my premises.

I say that the information provided by me herein above and documents produced by me are absolutely true and correct.

I say that I am swearing to this affidavit for the purpose of producing the same before the office of Mahanagar Gas Limited, Mumbai, in support application for transfer of PNG connection (BP No ________) to my name.
