Provider Compensation
CIGNA HealthCare is committed tokeeping you informed about mattersrelated to your health care plan. For thatreason, we offer the followingdescription of the way that CIGNAHealthCare compensates health careproviders (physicians, hospitals and otherhealth care practitioners and facilities)that participate in our managed carenetworks.
CIGNA HealthCare compensates healthcare providers in ways that are intendedto emphasize preventive care, promotequality care and ensure the appropriateand cost-effective use of covered medicalservices and supplies. CIGNAHealthCare reinforces this philosophythrough utilization management decisionsmade by its medical directors and HealthServices staff. CIGNA HealthCareemployees are encouraged to promoteappropriate utilization of covered healthcare services and to discourageunderutilization.
The same criteria apply for physicianseligible to receive additional paymentsbased on their performance. Physiciancompensation and incentives encouragethe provision of medically necessarycare. CIGNA HealthCare considers thephysician’s quality of care, quality ofservice and appropriate use of medicalservices prior to awarding any bonusesand incentives.
The methods by which participatinghealth care providers agree to becompensated are described generallyhere. The amount and type ofcompensation a health care provideragrees to accept may vary dependingupon the type of plan. For example, ahospital may agree to accept less forservices provided to patients enrolled inan HMO plan than to patients enrolled inother types of plans. In addition, CIGNAHealthCare may attempt in various waysto promote the use of those participatingproviders that are the most cost-effective,while assuring quality and access tocovered services and supplies.
DISCOUNTED FEE FOR SERVICE:Payment for services is based on anagreed-upon discounted amount from thehealth care provider’s bill.
CAPITATION:By mutual agreement, networkphysicians, provider groups orphysician/hospital organizations (PHOs)are paid a fixed amount (capitation) atregular intervals for each memberassigned to the physician, group or PHO,whether or not services are provided.This payment covers physician and/or,where applicable, hospital or otherservices covered under the benefits plan.Medical groups and PHOs may in turncompensate providers using a variety ofmethods.
Capitation offers health care providers apredictable income, encouragesphysicians to keep people well throughpreventive care, eliminates the financialincentive to provide services that will notbenefit the patient and reducespaperwork.
Providers paid on a capitation basis mayparticipate in a risk-sharing arrangementwith CIGNA HealthCare; they agree on atarget amount for the cost of certainservices and share all or some of theamount by which costs are over or underthe target. Provider services aremonitored using criteria that may includeaccessibility, quality of care, membersatisfaction and appropriate andcost-effective use of medical services andsupplies.
CIGNA HealthCare may also work withthird parties that administer payments toparticipating providers. Under thesearrangements, CIGNA HealthCare paysthe third party a fixed monthly amountfor these services. Providers arecompensated by the third party forservices provided to CIGNA HealthCareplan members from the fixed amount.Compensation arrangements vary butgenerally depend on overall utilization.
SALARY:Physicians and other providers who areemployed to work in a CIGNAHealthCare staff-model medical facilityare paid a salary. The salary is decided inadvance each year and is guaranteedregardless of the services provided.Physicians are eligible for a bonus at theend of the year based on performance,which is evaluated using criteria that mayinclude quality of care, quality of serviceand appropriate and cost-effective use ofmedical services and supplies.
BONUSES AND INCENTIVES:Some providers may receive additionalpayments based on their performance,which is measured using criteria that mayinclude quality of care, quality of serviceand appropriate and cost-effective use ofmedical services and supplies. Providersmay also receive financial and/ornonfinancial incentives that promoteutilization of cost-effective participatingproviders (such as hospitals, labs,specialists and vendors) and covereddrugs and supplies.
PER DIEM:A specific amount is paid to a hospitalper day for all health care received. Thepayment may vary by type of service andlength of stay.
CASE RATE:A specific amount is paid for all healthcare received in the hospital for a givenhospital stay (such as for a normalmaternity delivery).

CIGNA LIFESOURCETransplant Networksm
CIGNA HealthCare is committed to providing access to quality health care throughtoday’s innovative and technologically advanced medical options. As a CIGNAHealthCare member, you’ll have access to the CIGNA LIFESOURCE TransplantNetworksm, a network of participating organ and tissue transplant centers.Developed by a team of CIGNA HealthCare clinical professionals, the CIGNALIFESOURCE Transplant Networksm includes respected hospitals and medicalcenters throughout the United States.
As part of the credentialing program, each transplant facility is evaluated forfavorable rates of patient outcomes, as well as waiting periods, housingarrangements, and ‘‘patient friendly’’ environments, before it is included in theCIGNA LIFESOURCE Transplant Networksm.
CIGNA LIFESOURCE participants are managed by the Comprehensive TransplantCase Management Unit. This unit consists of Registered Nurses with clinicalexperience in transplant, hematology/oncology, home health care, dialysis, criticalcare and/or community care. They are specially trained to manage complextransplant cases.

In some instances a travel allowance is offered as a feature of the program. Please beaware that most of these expenses are usually considered taxable income.
