BUILDING CONVENIENT, COST-EFFECTIVE CARE MODELS

• When such telehealth tools are incorporated intotraditional health care settings, they help facilitatecoordination of care for patients with complexconditions.

• Family/ patients can now consult with a physician ornurse practitioner via video, phone, or email to helpdiagnose conditions that may require little more than amedical history to assess, such as pink eye, insectbites, and blad-der infections.

IMPROVING DECISION-MAKING BYCONSUMERS AND PROVIDERS

• Digital health technologies also can empower consumerswho have chronic and complex conditions by helping themsort through treatment options using tools that recognizeethnographic differences and varying levels of healthliteracy.

• These decision aids, including interactive tools and Web-based educational modules, focus on the benefits, risks,and uncertainties of medical procedures and have beenshown in a comprehensive review to increase patients’knowledge of and improve their satisfaction with care
