Cracked the Code

This value-based care leader – one of only two ACOs that achieved shared savings over the course of the Pioneer program – has cracked the code on home hospitalization, with a 30-day prospective bundled payment for CHF; COPD; deep vein thrombosis; UTI; cellulitis; and pneumonia. Marshfield executives will describe this year-old initiative, including its role in mitigating unwarranted admissions and re-admissions, and recommend how other health systems can replicate their model.

� 9:45 – 10:15 am The Virtual Hospital: Avera Health Redefines

Acute Telemedicine

Sioux Falls, SD-based Avera Health, a 33-hospital integrated network, will share a case study of Avera eCare. Health systems in a handful of markets across the nation are building very small, full-service hospitals that fill the gap between urgent care centers and large inpatient buildings. San Francisco-based Dignity Health will share the strategy behind its micro-hospitals, which offer 24/7 service through eight to ten ER bays, imaging and lab services, and fewer than a dozen inpatient beds.

Health System 2027: Four Bold Business Models Lincoln Healthcare Leadership is conducting a research study that will prescribe the four most promising business models for successful health systems of the future. We are interviewing more than 50 health system CEOs, healthcare strategy consultants, healthcare futurists, private equity investors, and disruptive entrepreneurs.

Over the next ten years, we can expect a continuing buildup of tectonic forces: increasing incentive to bend the healthcare cost curve downward, increasing consumerism, and increasing disruptors to health systems’ dominant position. The likely consequences: fee-for-service will wane, value-based care will dominate, acute care revenues will be lower than today, some fraction of health systems will close, and leading health systems’ business models will look substantially different. This project aims to peer into this future, describe the business models most likely to succeed, and recommend the sequential steps for getting there from the present.

� HealthPartners is a NFP integrated delivery system with seven hospitals, 1,700 physicians, 55 primary care clinics, 20 urgent care locations and numerous specialty practices in Minnesota and western Wisconsin. Its medical and dental health plans cover 1.8 million members.
