Before Your Surgery A member of our business office staff will call you prior to your surgery date to confirm personal and insurance information, and discuss your estimated charges for services that your physician has scheduled for you. In addition, our pre-admission nurse will call you 1 to 2 days before your surgery date to complete a clini- cal assessment, provide pre-operative instructions, and provide your arrival time on the day of surgery. Please ensure you have spoken with someone from our center or your surgery may be cancelled. Preparing for Surgery Please be sure to talk to your doctor about daily medication you are currently taking especially for heart diabetes or blood pressure problems; be sure to mention any over-the-counter drugs such as aspirin, acetaminophen Advil, Motrin, herbal supplements and vitamins. Your doctor will give you instructions about taking any medication prior to surgery. e Night Before Surgery: For your safety, please do not eat or drink anything aſter 12:00 midnight (including water) the night before your surgery. is includes hard candy, gum or prescription medications, unless ordered by your doctor. Un- digested food in the stomach can cause complica- tions, and your surgery is likely to be postponed if you forget to follow these instructions. e Day of Surgery Bathe or shower the morning of your surgery to mini- mize the risk of infection. You may brush your teeth or use mouthwash the morning of surgery; however, remember not to swallow any water. Refrain from smoking aſter midnight of the day pre- ceding your surgery. When to Notify to Notify Your Doctor: If for any reason you cannot keep your scheduled appointment for surgery or you notice the following, contact your doctor immediately: You suspect that you are pregnant. You experience any change in your health such as a cough, fever or a cold. e Day of Surgery: Here is a quick checklist for the day of your surgery: Be sure to have an adult present to drive you home when your surgery is completed. You can not drive yourself. Please leave all valuables, including jewelry and watches, at home. Be sure to remove all body piercings. Please bring co-pays, deductible, or coinsurance due Please bring your insurance card and photo ID so we may properly bill your insurance carrier. Wear comfortable, loose fitting clothing. Please do not wear contact lenses or make-up. Please leave children at home When You Arrive At Ponte Vedra Beach Surgery Center Plan to arrive promptly and check-in on arrival and have a seat in our waiting room. Our Front Office Representative will confirm and com- plete all necessary paperwork with you. Once complet- ed, one of our nurses will take you back to the pre-op area so you may prepare for surgery. e Procedure Itself Anesthesia Care Team: Anesthesia services are provided by a board certified anesthesiologist, highly qualified in all aspects of outpatient anesthesia. Your anesthe- siologist will examine you before surgery and answer any questions you may have. Be sure to talk to your anesthesiologist about any medications you are currently taking, even over-the-counter drugs. e appropriate type of anesthesia will be determined by your surgeon in consultation with the anesthesiolo- gist. Your Recovery What Happens Aſter My Surgery? While in the operating room, you will be transferred to a special bed, and then wheeled to the recovery room. ere, nurses frequently check your blood pressure, pulse and respiration rate. Medications for pain or nausea may be adminis- tered. Since many patients feel slightly disoriented aſter surgery, your nurse will first assure you that the surgery is completed and that you are safe in the recovery room. You will remain until you are ready to be discharged. If you have no nausea, you will be given water, or juice. How will I feel? How you feel depends on the complex- ity of your procedure, how long you were in the operat- ing room, your pain threshold and what type of anes- thesia you were given. Each person responds differently to surgery and the effects of anesthesia. It is important to communicate with your nurse and let her know if you need pain medication or have special concerns. A responsible person must drive you home and you must have a responsible person in attendance for the first 24 hours following surgery.

Before Your Surgery

A member of our business office staff will call you prior to your surgery date to confirm personal and insurance information, and discuss your estimated charges for services that your physician has scheduled for you.

How Are Finacial Arrangements Handled?

MAKING ARRANGEMENTS:Please contact the surgery center prior to surgery to make financial arrangements or to ask questions, and to be sure you are pre-admitted. In most cases, we should be able to estimate the cost of your surgery be-forehand. Our staff will help you finalize your finacial arrangements before your surgery.To avoid delays in processing tour account, please bring your State pic-tureIDordriver’slicense,andyourinsurancecards.

CHARGES AND PAYMENTS:Billing for the Center and physician is separate. We charge for the use of the Center, that includes the cost of the operating room, recovery room and supplies for surgery. Your surgeon, anesthesiologist, and patholo-gist (if applicable) will bill you separately for their services.

1. Uninsured and/or cash patients will be required to pay for services on or before the date of surgery.

2. Patients with verified insurance will be required to pay an estimated portion on or before the day of surgery.

3. The Surgery Center will bill your insurance com-pany as a courtesy, however, the balance due is the patient’sresponsibilityandpaymentwillbere-quested from you unless your insurance company pays within 60 days.

4. The facility participates in most managed care plans(HMOsandPPOs)Medicaid,Worker’sCompenstation, and commercial insurance plans.

5. All financial arrangements must be made prior to admission.

Merge onto FL-A1A S. toward Ponte Vedra/St. Augustine. We are located between Palm Valley Road and Mickler Road. Our office Build-ing will be 2.8 miles from Sawgrass Marriott on the right.

If you are unable to keep your appointment, please notify your doctor and the Surgery Center at (904) 285-1199 as soon as possible. This assists your doctor and the Surgery Center with daily scheduling. We appreciate your courtesy.

If the Surgery Center has not contacted you two days prior to your surgery, please call the Surgery Center.

We must speak to you prior to the day of surgery or your surgery may be cancelled.
