March 5, 2016 test is

February 5, 2016

If envelope is not post-marked by that date, College Board will not complete registration for the SAT test.

To complete this registration information, print in CAPITAL letters and use a blue or black

This questionnaire (questions 1-34) deals with a student’s high school experience. Since you are in middle school, this does not pertain to you and you do not need to answer these questions.

Copy the statement in cursive and sign the form. Forms that are not signed cannot be processed

and will be returned.


Item 15a: Test Date

Parents- If your child cannot test on this date, please mark the date that he/she will be testing. If testing on an alternate date, you will be responsible for arranging testing with that high school.

Testing at the three middle school sites is only being offered on March 5, 2016.
