Nanosized filler particles abrade below the visible spectrum of light, allowing the polish retention typical of a microfill.The nanoclusters are lightly sintered so that they can break apart during the wear process. By doing so, the optical effect is the loss of a nanometer-sized particle rather than the loss of a micron (e.g. hybrid composite) sized particle, which is the reason for the long term polish retention.

Class III Composite Restoration using Filtek Z350

24 yr old young male with rampant caries, history of missing molars and root pieces leading to migration of teeth. He has bad oral hygiene with generalized periodontitis.

There are old composite restorations on 11,21,22,23 which were done without following the established protocol. Upon flicking off the loosely attached restorations, the presence of active caries was visible underneath. Caries was carefully excavated. Indirect pulpal exposure seen in 21,22 & 23. Gingival displacement cord was placed in all four teeth so as to avoid contamination with blood and sulcular fluid.

The Ketac N100 restoration was finished and polished using 3M ESPE's Soflex system. Esthetics comparable to composites is possible with the use of Ketac N100.

The right central incisor was also restored using Ketac N100 nanoionomer. Each increment was light cured for 20 secs.

The Ketac N100 restoration was completed by finishing and polishing with 3M ESPE's Soflex system.

Es the t i c Nano ionomer (RMGI ) restoration that blends well with the tooth seen on 11.

The tooth preparation involved a long bevel on the enamel to help blend the shade of the restoration with that of the tooth. It was decided to restore the right central incisor (11) and left canine (23) with Ketac N100 nanoionomer, due to the advantage of Flouride release. Esthetics given by Ketac N100 is a definitive advantage in anterior cervical restorations.Ketac N100 Primer application is done on canine, air-dryed and cured for 20 secs.

Ketac N100 was placed in increments and light cured for 20 secs.

In the upper left quadrant a deep distal cavity is visible on the first bicuspid and the second bicuspid had incipient caries on mesial aspect, which has been recontoured and polished. Wedge is inserted to control oozing from the interdental papilla. Caries excavated and the tooth was prepared for restoration.

After placement of sectional matrix band and a wedge, the primer of 3M ESPE Adper SE Plus (Self Etch Adhesive) Liquid A (rose bengal color) is applied on the prepared tooth. The color ensures proper coverage of the cavity preparation.

Next, the first coat of liquid B (yellow color) of Adper SE Plus is scrubbed on the primed surface for 20 seconds. On interacting with one another the bond layer becomes transparent ensuring proper activation of adhesive. It was then air-dryed. Next the second coat is applied and light cured which helps in forming a "hydrophobic overcoat".

Then the cavity was filled incremently with 3M ESPE's Filtek Z350 Nanocomposites and finishing and polishing was done with gold finishing burs and 3M ESPE's Soflex finishing and polishing system

Final restoration with Filtek Z350.The packability of Z350 helps in achieving the cusp & groove pattern of bicuspid.

High Caries Risk Patient: Multiple Restorative Requirements Answered.

It was decided to use Filtek Z350 nanocomposite to restore the left central and lateral incisors as layering with different opacities was required to get a natural tooth like restoration. Gingival displacement cord was placed for adequate visibility and to improve isolation. After completeing cavity preparation, Adper Easy One self etch system was applied and cured before composite placement.

After completion of the procedure, the displacement cord is removed. Restorations are checked for over-extension of the gingival margins. Thorough finishing and polishing of these areas are ensured.

To control the oozing of crevicular fluid in the interdental papillary area an anatomical wedge is placed. The first layer of composite (OA3-Opaque A3) was applied on both the central and lateral incisor. This was used to help improve the value of the restoration (to block the “show through” of the darkness of the oral cavity).

A second layer of Filtek Z350 A2 body shade was used followed by Filtek Supreme A2 enamel shade to complete the layering procedure.

Restorations seen on 21 & 22 after completion of the nanocomposite build up.

Occlusal equilibration is achieved with adjustments on the palatal aspect, which was then finished and polished.

Finishing and Polishing of labial aspect is done using 3M ESPE's Soflex composite finishing and polishing system.
