Tobacco Aside, E-Cigarette Flavorings May Harm Blood VesselsFlavoring chemicals widely used in e-cigarettes and other tobacco products may be toxic to the cells that line and regulate blood vessel function. The adverse effects observed withchemical flavor additives on endothelial cells could be early warning signs of future heart disease, researchers say. The health effects of "combustible" tobacco products including tra-ditional cigarettes and hookah are well-established, but the potential dangers of e-cigarettes have not yet been extensively studied. E-cigarettes are battery-powered devices that heata liquid -- including tobacco-derived nicotine, flavoring and other additives -- and produce an aerosol that is inhaled. Nine chemical flavorings -- menthol (mint), acetylpyridine (burnt fla-vor), vanillin (vanilla), cinnamaldehyde (cinnamon), eugenol (clove), diacetyl (butter), dimethylpyrazine (strawberry), isoamyl acetate (banana) and eucalyptol (spicy cooling) -- whichare widely used in e-cigarettes, hookah, little cigars and cigarillos were tested for their short-term effects on endothelial cells, the cells which line the blood vessels and the inside of theheart. Researchers found all nine flavors were dangerous to cells in the laboratory at the highest levels tested and all the flavorings impaired nitric oxide production in endothelial cellsin culture (outside of the body). Several of the flavorings -- menthol, clove, vanillin, cinnamon and burnt flavoring -- resulted in higher levels of an inflammatory marker and lower levelsof nitric oxide, a molecule that inhibits inflammation and clotting, and regulates vessels' ability to widen in response to greater blood flow. "Increased inflammation and a loss of nitricoxide are some of the first changes to occur leading up to cardiovascular disease and events like heart attacks and stroke, so they are considered early predictors of heart disease,"said lead study author Jessica L. Fetterman, Ph.D., assistant professor of medicine at Boston University School of Medicine in Massachusetts. "Our findings suggest that these flavor-ing additives may have serious health consequences." Endothelialcells were collected from volunteers (nine non-smokers/non-e-cigaretteusers; six non-menthol and six men- thol cigarette smokers) and testedin the lab. Researchers found that both groups of smokers had a simi-lar deficit in nitric oxide production when stimulated by a chemicalcalled A23187. Nonsmokers' cells that were treated with menthol or aclove flavoring also had impaired nitric oxide production, suggestingthose flavorings cause damage like that found in active smokers. Theteam also exposed commercially- available human aortic endothelialcells to the flavorings. Burnt flavor, vanilla, cinnamon and clove flavorsimpaired nitric oxide production and boosted an inflammatory chemicalcalled interleukin-6 (IL-6) at all con- centrations tested, suggesting theendothelium is particularly sensitive to these flavors. Menthol applied tothe cells increased IL-6 at high con- centrations and reduced nitric oxideeven at low doses. In smokers, sci- entists don't see differences in heartdisease between menthol and non- menthol users -- probably becausecigarette smoke is overwhelmingly toxic, Fetterman said. "But mentholis certainly not a benign player, based upon our work." At the high-est levels tested, all nine chemicals caused cell death, while at lowerlevels cinnamon, clove, strawberry, banana and spicy cooling flavor did.Dimethylpyrazine/strawberry flavor had that effect even at very low lev-els, suggesting endothelial cells are especially sensitive to it. Vanillin and eugenol also increased oxidative stress in the cells. Three flavorings were tested when heated, to mimic whathappens in e-cigarettes. Nitric oxide production was impaired with vanillin and eugenol, but not with menthol. "Our work and prior research have provided evidence that flavorings inducetoxicity in the lung and cardiovascular systems. Flavorings are also a driver of youth tobacco use and sustained tobac-co use among smokers," Fetterman said. A key strength of the new research was that it directly tested effects of justthe flavorings, at levels likely to be reached in the body. Limitations include the fact that testing did not heat all the fla-vorings or include other chemicals used in e-cigarettes. Also, the study gauged just the flavorings' short-term effectsand captured these with cells outside the body, not inside. "We still don't know what concentrations of the flavoringsmake it inside the body," Fetterman said. Most adult e-cigarette users are current or former combustible cigarettesmokers who may use e-cigarettes as an aid in smoking cessation or as a harm-reduction tool. In addition, e-cigaretteuse by youth is rising rapidly with 37 percent of high schoolers reporting they have had an e-cigarette in 2015.Flavored tobacco products are a major driver of experimentation among youth. The American Heart Association cau-tions against the use of e-cigarettes, stating that e-cigarettes containing nicotine are tobacco products that should besubject to all laws that apply to these products. The Association also calls for strong new regulations to preventaccess, sales and marketing of e-cigarettes to youth, and for more research into the product's health impact.

New Target For Treating Heart FailureChanges in cellular struts called microtubules (MT) can affect thestiffness of diseased human heart muscle cells, and reversingthese modifications can lessen the stiffness and improve the beat-ing strength of these cells isolated from transplant patients withheart failure, found researchers from the Perelman School ofMedicine at the University of Pennsylvania. This Nature Medicinenew study is a continuation of research conducted two years agoon how MTs are involved in regulating the heartbeat. "These find-ings provide compelling evidence from human samples for a newtherapeutic target for heart disease," said senior author BenProsser, PhD, an assistant professor of Physiology. The Penninvestigators aim to develop therapies that seek out the damagedMTs to reverse their harmful influence. By suppressing impairedMTs, the team improved heart muscle cell function in damagedhuman cells. Normally, MTs of the cell's inner support system havediverse structural and signaling roles. Alterations in this networkhave been suggested to contribute to heart disease. Recent stud-ies suggest that chemical changes to the MTs, called detyrosina-tion (the removal of a tyrosine chemical group), control themechanics of heart beats. Detyrosinated MTs provide resistancethat can impede the motion of contracting heart muscle cells. ThePenn team used mass spectrometry and mechanical tests of sin-gle heart muscle cells to characterize changes to the MT networkand its consequences for normal heart function. Analysis of tissuefrom the left ventricle of heart transplant patients revealed a con-sistent upregulation of proteins that leads to the stiffening of MTs.Using super-resolution imaging, the team also saw a dense, heav-ily detyrosinated MT network in the diseased heart muscle cells,which is consistent with increased cell stiffness and decreasedability to contract. Proper cell elasticityand contraction is crucial for normalcirculation throughout the body. Usinga drug, the team suppressed the dety-rosinated MTs, which restored abouthalf of lost contractile function in thediseased cells. Genetically reducingthe MT detyrosination also softenedthe diseased cells and improved theirability to contract. Past clinical datafrom Penn showed a direct correlationbetween excess MT detyrosinationand a decline in heart function amongpatients with hypertrophic cardiomy-opathy, a condition in which thickenedheart muscle can cause problems inmaintaining proper blood pressurelevels and flow of blood through theheart. The team found that detyrosina-tion was greater in diseased hearts bycomparing human heart tissue donat-ed from heart transplant patients tonormal heart tissue from other donors,obtained from work with transplantcardiologist and coauthor KenMargulies, MD, a professor ofCardiovascular Medicine. Cells fromdiseased hearts have more MTs, andthese MTs have more detyrosination.This process correlated with impaired function within this patient populationin that their whole hearts, before thetransplant, had a lower ejection frac-tion that correlated with greater dety-rosination. Ejection fraction, an indica-tor of heart health, measures theamount of blood pumped out of ventri-cles with each contraction. The teamis now working on ways to target onlyheart muscle cell MTs. They are refin-ing gene therapy approaches with thePenn Gene Vector Core to deliver anenzyme to the heart that reversesdetyrosination in heart muscle cells.

Easy Exercises For Back Pain ReliefBack pain problems are amongst the most common medical conditions faced by millions.Fortunately, they are treatable, especially if the causes are diagnosed. Some of the commoncauses are muscle strain, ruptured disc, constricted spinal canal and osteoporosis. Most peo-ple recover without medication or surgery by not straining the spine, using ice packs or hotwater bottles to ease the pain. An easy method of relief from back pain is with the help of exer-cises; some simple examples follow: Lying on the back:- Lie with your back on the floor withpillows under your head. Place a chair close enough to enable you to bend your knees andrest them on the seat of the chair. This takes pressure off the back. Bending from a chair:-Sit on a chair and lean forward to stretch, placing the palms on the feet, while keeping the headdown and the neck relaxed. Hold this position for ten to thirty seconds. Then use your handsto push yourself upright. This exercise is to be repeated thrice at a stretch. Cat bend:- Beginthis exercise by getting down on the hands and knees. Arch the upper back upwards, put yourhead down and contract your stomach muscles. This position should be held for five seconds.Continue by raising your head and dropping your back to the original position, taking care thatthe pelvis is not pushed forward to avoid a swayback. This position should be held for fifteenseconds and repeated three times at one go. Single knee to chest:- Lie flat on your back,then raise and pull your right knee forward towards your chest using your hands. Hold thisposition for ten seconds. This knee should then be put back on to the floor. Next, the left kneeshould be pulled forward to the chest in the same manner and held in that position; followingthis the legs are lowered. This exercise should be repeated thrice on each side. Mid backstretch:- In this exercise the person should stand upright with hands on the hips, and kneesshoulder width apart. Following this, twist from the waist towards the right and turn your neckin the same direction as far as you can manage with comfort. Hold for ten seconds whilebreathing normally. Next do the exact opposite, this time twisting from the waist towards theleft. Tips for people suffering from back pain:- When lifting heavy objects, do not lift bybending over, instead, bend the hips and squat l Lift heavy objects by keeping the backstraight and holding the object close to the body l Do not twist the body while lifting l Pushrather than pull while moving a heavy object l If you go for a long car journey or sit at a deskfor extended lengths of time, try and remember to consciously take breaks and stretch l Keepin mind that flat soles with low heels help in aavoiding back pain.

Blood Vitamin D LevelsLinked To Colorectal

Cance RiskA new study authored by scientists frommore than 20 medical centers and organiza-tions finds that higher circulating vitamin Dconcentrations are significantly associatedwith lower colorectal cancer risk. Vitamin D,known for its role in maintaining bone health,is hypothesized to lower colorectal cancerrisk via several pathways related to cellgrowth and regulation. Previous prospectivestudies have reported inconsistent resultsfor whether higher concentrations of circulat-ing 25-hydroxyvitamin D, the acceptedmeasure of vitamin D status, are linked tolower risk of colorectal cancer. The few ran-domized clinical trials of vitamin D supple-mentation and colorectal cancer completedthus far have not shown an effect; but studysize, supplementation duration, and compli-ance may have contributed to their null find-ings. "To address inconsistencies in priorstudies on vitamin D and to investigate asso-ciations in population subgroups, we ana-lyzed participant-level data, collected beforecolorectal cancer diagnosis, from 17prospective cohorts and used standardizedcriteria across the studies," said StephanieSmith-Warner, PhD, an epidemiologist at theHarvard T.H. Chan School of Public Healthand co-senior author on the article. Theanalysis included over 5,700 colorectal can-cer cases and 7,100 controls from theUnited States, Europe, and Asia. A single,widely accepted assay and laboratory wasused for new vitamin D measurements andcalibration of existing vitamin D measure-ments. "In the past, substantial differencesbetween assays made it difficult to integratevitamin D data from different studies," explained Regina G. Ziegler, PhD, a National Cancer Institute epidemiologistand co-senior author on the article. "This calibration approach enabled us to systematically explore risk over thebroad range of vitamin D levels seen internationally." Compared to participants with circulating vitamin D concen-trations considered sufficient for bone health, those with deficient concentrations of vitamin D had a 31% higher riskof colorectal cancer during follow-up, which averaged 5.5 years (range: 1 -- 25 years). Similarly, concentrationsabove bone health sufficiency were associated with a 22% lower risk. However, risk did not continue to decline atthe highest concentrations. These associations persisted even after adjusting for known colorectal cancer risk fac-tors. Protective associations were seen in all subgroups examined. However, the association was noticeablystronger in women than men at concentrations above bone health sufficiency. The lifetime risk of colorectal canceris 4.2% (1 in 24) in women and 4.5% (1 in 22) in men. Colorectal cancer is the third most common cancer and third

leading cause of cancer-related deaths in both men and women in the United States, with about 140,250 new casesand 50,630 deaths expected during 2018 "Currently, health agencies do not recommend vitamin D for the preven-tion of colorectal cancer," said Marji L. McCullough, ScD, American Cancer Society epidemiologist and co-firstauthor of the study. "This study adds new information that agencies can use when reviewing evidence for vitaminD guidance and suggests that the concentrations recommended for bone health may be lower than would be opti-mal for colorectal cancer prevention." Vitamin D can be obtained in the diet, particularly from fortified foods, fromsupplements, and from sun exposure. Experts recommend vitamin D be obtained through diet whenever possiblebecause excessive ultraviolet radiation is a major risk factor for skin cancer.
