2. Sign  and  date  the  membership  application    

3. Attach  annual  dues  check  made  out  to:  Northern  California  MCA    

4. Have  sponsoring  MCA  NorCal  member  contractor  sign  the  application    

5. Attach  the  sponsoring  MCA  NorCal  member  contractor’s  recommendation  statement  to  the  application  

 6. Mail  completed  membership  application,  dues  check  and  recommendation  statement  to:
